Effecten van pravastatine op cholesterol, ontsteking en cognitie bij schizofrenie
Fase 4-onderzoek naar de effecten van pravastatine op cholesterolwaarden, ontsteking en cognitie bij schizofrenie
Bij deze studie zijn mensen met schizofrenie of schizoaffectieve stoornis betrokken, die momenteel antipsychotica gebruiken. Sommige antipsychotica kunnen een verhoging van het cholesterolgehalte veroorzaken, wat kan leiden tot ontstekingen in het lichaam. Ontsteking vormt een risico bij het ontwikkelen van hartaandoeningen, diabetes en problemen met de hersenfunctie. Het doel van deze studie is om te zien of pravastatine:
- Lagere cholesterol
- Verminder ontstekingen
- Verbeter de cognitie bij patiënten met schizofrenie

Beschrijving
Inclusiecriteria:
- Man of vrouw
- Leeftijd 18-68 jaar
- Diagnose van schizofrenie, elk subtype, schizoaffectieve stoornis, elk subtype of schizofreniforme stoornis
- Goed gevestigde naleving van poliklinische medicatie, inclusief hun antipsychotische medicatie
Uitsluitingscriteria:
- Onvermogen om geïnformeerde toestemming te geven
- Actueel middelen- en alcoholmisbruik
- Aanzienlijke medische ziekte, waaronder congestief hartfalen, ernstige hart- en vaatziekten, nierziekte (serumcreatinine > 1,5), ernstige leverfunctiestoornis of actieve leverziekte, bloedarmoede (hemoglobine < 11,0 gm/dl), voorgeschiedenis van ernstig hoofdletsel en niet-behandelde spier ziekte.
- Psychiatrisch instabiel
- Vrouwen in de vruchtbare leeftijd die zwanger zijn, borstvoeding geven of die gedurende het gehele onderzoek geen effectieve vorm van anticonceptie willen of kunnen gebruiken
- Onderwerpen die worden behandeld met ontstekingsremmende geneesmiddelen (waaronder dagelijkse aspirine en ibuprofen), thiazidediuretica; middelen die gewichtsverlies veroorzaken, en sint-janskruid zullen van het onderzoek worden uitgesloten
- Huidige geschiedenis van onbehandelde schildklierziekte
- Huidige behandeling met insuline
- Onderwerpen die worden behandeld met geneesmiddelen zoals: colchicine, azol-antischimmelmiddelen (fluconazol, ketoconazol, itraconazol); macrolide-antibiotica (claritromycine, erytromycine); HIV-proteaseremmers (ritonavir, indinavir, saquinavir, nelfinavir) die het CYP 450 3A-leverenzym remmen
- Bekende overgevoeligheid voor pravastatine of een van de bestanddelen ervan

Het meet- en behandelingsonderzoek ter verbetering van de cognitie bij schizofrenie (MATRICS) Consensus Cognitive Battery meet het cognitief functioneren binnen 7 domeinen: verwerkingssnelheid, aandacht/waakzaamheid, werkgeheugen (non-verbaal en verbaal), verbaal leren, visueel leren, redeneren en probleemoplossing en sociale cognitie. De samengestelde score wordt berekend door het computerprogramma MATRICS, dat elk van de 7 domeinscores even zwaar weegt. Het bereik van samengestelde scores is 20-80. Hogere scores duiden op hogere niveaus van cognitief functioneren, terwijl lagere scores op lagere niveaus van cognitief functioneren duiden. |
