Plaveiselcelcarcinoom van het hoofd en de hals (HNSCC) is wereldwijd een van de belangrijkste doodsoorzaken door kanker en staat op de vierde plaats in Taiwan als gevolg van kankergerelateerde sterfgevallen bij mannen.
In HNSCC dragen invasiviteit en uitzaaiing van kankercellen bij tot de belangrijkste doodsoorzaak; daarom is opheldering van het mechanisme en de ontwikkeling van nieuwe strategieën tegen metastase van het grootste belang bij de behandeling van gevorderde HNSCC.
Epitheliale-mesenchymale overgang (EMT) is een proces waarbij epitheelcellen hun polariteit verliezen en worden omgezet in een mesenchymaal fenotype, en wordt recentelijk beschouwd als het belangrijkste mechanisme voor uitzaaiing van kanker.
De initiatie van EMT wordt gekenmerkt door onderdrukking van het intercellulaire junctionele eiwit E-cadherine door een verscheidenheid aan transcriptionele factoren, waaronder Twist1, Snail, Slug, SIP1, Zeb1 en E47.
Tijdens de metastatische evolutie is het samenspel tussen verschillende EMT-inductoren echter beperkt onderzocht.
Eerder onderzoek toonde aan dat hypoxie EMT van HNSCC induceert door inductie van Twist1-expressie.
Bovendien bleek dat Twist1 EMT en tumor-initiërend vermogen bevordert door opregulatie van Bmi1, en co-expressie Twist1 en Bmi1 voorspelt een slechtere prognose van HNSCC-gevallen.
Geaccumuleerd bewijs suggereert dat microRNA's een essentiële rol spelen bij de progressie en metastase van kanker.
Daarom willen we de rol van microRNA in Twist1-gemedieerde kankermetastase en de wisselwerking tussen Twist1 en andere EMT-regulatoren onderzoeken.
Onze voorlopige gegevens toonden aan dat de expressie van de miR-29-familie, inclusief miR-29a, b en c, was verhoogd in Twist1-tot overexpressie komende HNSCC-cellen.
Verder ontdekten we dat SIN3A, een co-repressor van een andere EMT-regulator Snail, een doelwit is van miR-29's.
We speculeren daarom dat Twist1 de functie van Snail wijzigt door middel van microRNA-machines.
In dit voorstel zullen we het regulerende mechanisme van de Twist1-miR29s-SIN3A-as afbakenen.
We zullen ook het moleculaire samenspel tussen Twist1 en Snail onderzoeken via de signaalroute Twist1-miR29s-SIN3A.
Verder zullen we de moleculaire basis en pathofysiologische betekenis van Twist1-Snail-interactie in een hypoxische omgeving toelichten.
Ten slotte zullen we de in vitro bevinding bevestigen door middel van in vivo dierstudie en HNSCC-monsters.
Deze resultaten zullen cruciale informatie opleveren voor het begrijpen van de moleculaire basis van HNSCC-metastase, en zullen waardevol zijn voor het ontwikkelen van nieuwe therapeutische strategieën tegen geavanceerde HNSCC.
