Neuroplastische veranderingen van de motorische cortex door cafeïne: verschillen tussen cafeïne- en niet-cafeïnegebruikers en invloed van waakzaamheid tijdens stimulatie
Cafeïne is een psychostimulerend middel. Het werkt als een competitieve antagonist bij adenosinereceptoren, die ook de corticale prikkelbaarheid moduleren. Bij diepe hersenstimulatie (DBS) verklaart de productie van adenosine na het vrijkomen van adenosinetrifosfaat (ATP) de vermindering van tremor. Binding van adenosine aan adenosine A1-receptoren onderdrukt excitatoire transmissie in de thalamus en vermindert hierdoor zowel tremor- als DBS-geïnduceerde bijwerkingen. Ook werd het effect van adenosine verzwakt na toediening van de 8-cyclopentyl-1,3-dipropylxanthine (DPCPX) adenosine A1-receptorantagonist. Daarom werd gesuggereerd dat de aanwezigheid van een receptorantagonist zoals cafeïne de effectiviteit van diepe hersenstimulatie (DBS) bij de behandeling van tremor en andere bewegingsstoornissen zou verminderen.
Op basis van deze bevinding veronderstellen de onderzoekers dat het antagonistische effect van cafeïne voorlopig de stimulerende effecten van transcraniële wisselstroomstimulatie (tACS) kan blokkeren. De plasticiteitseffecten kunnen verschillen tussen cafeïnegebruikers en niet-cafeïnegebruikers, afhankelijk van de beschikbaarheid van receptorbindingsplaatsen.
Afgezien daarvan is een belangrijk probleem in NIBS-onderzoeken, inclusief die die motor-evoked potentials bestuderen, de responsvariabiliteit zowel binnen als tussen individuen. De variabiliteit van proef tot proef van motor evoked potentials (MEP's) kan door vele factoren worden beïnvloed. Inherent aan cafeïne is het effect ervan op de waakzaamheid. In deze studie zal de onderzoeker de waakzaamheid van de deelnemer volgen door middel van pupillometrie om (1) de factoren beter te begrijpen die variabiliteit kunnen veroorzaken in onderzoeken naar transcraniële prikkelbaarheidsinductie en (2) om het directe farmacologische effect te scheiden van het indirecte aandachtseffect van cafeïne.

Beschrijving
Inclusiecriteria:
- Mannelijke en vrouwelijke gezonde deelnemers tussen de 18 en 45 jaar.
- Rechtshandig (Oldfield 1971).
- Vrijwillige deelname en schriftelijke, geïnformeerde toestemming van alle proefpersonen verkregen voorafgaand aan de start van het onderzoek.
- Het gewicht van de deelnemer is meer dan 60 kg
Uitsluitingscriteria:
- Leeftijd < 18 of > 45 jaar;
- Linkerhand dominant;
- Bewijs van een chronische ziekte of geschiedenis met een aandoening van het zenuwstelsel
- Geschiedenis van epileptische aanvallen;
- Pacemaker of diepe hersenstimulatie;
- Metalen implantaten in het hoofdgebied (metaal gebruikt in het hoofdgebied, bijvoorbeeld clips na de operatie van een intracerebraal aneurysma (bloedvatverzakking in het gebied van de hersenvaten), implantatie van een kunstmatige gehoorgang);
- Hersentrauma met bewustzijnsverlies in de prehistorie;
- Bestaan van een ernstige interne (interne organen) of psychiatrische (geestesziekte)
- Alcohol-, medicatie- of drugsverslaving;
- Receptieve of globale afasie (stoornis van het spraakverstaan of daarnaast van spraak);
- Deelname aan een ander wetenschappelijk of klinisch onderzoek in de afgelopen 4 weken;
- Zwangerschap
- Borstvoeding
- Intolerantie voor cafeïne of koffieproducten
- Deelnemer die abnormale hartactiviteit heeft door een elektrocardiografie (ECG) bevinding
- Gewicht is minder dan 60 kg

Deelnemers krijgen een cafeïnetablet en alle elektrische stimulaties in willekeurige volgorde (tACS 140 Hz bij 1 mA en sham tACS).
De waakzaamheidsstatus van de deelnemer wordt gecontroleerd op basis van actieve waakzaamheid of passieve waakzaamheid.

Deelnemers krijgen een placebo-tablet en alle elektrische stimulaties in willekeurige volgorde (tACS 140 Hz bij 1 mA en nep-tACS).

De plasticiteit van de motorcortex wordt gemeten aan de hand van de veranderingen in de amplitude van de motor evoked potentials (MEP's) op verschillende tijdstippen.
Transcraniële magnetische stimulatie (TMS) zal worden gebruikt om MEP-amplitudes te meten.
