SMILE: Klinisch onderzoek om mindfulness te evalueren als interventie voor raciale en etnische bevolkingsgroepen tijdens COVID-19 (SMILE)
Digitaal gezondheidsplatform (DHP) gaat mindfulness leveren als stressbeheersingsinterventie door gebruik te maken van elektronische (SMILE) gezondheidsdossiers voor raciale en etnische bevolkingsgroepen tijdens de COVID-19-pandemie: klinische proef
Het doel van deze klinische proef is het evalueren van de SMILE-app, een Digital Health Platform (DHP), dat een mindfulness-interventie zal leveren, ontworpen om COVID-gerelateerde stress te verminderen. Bovendien zal de SMILE-app op afstand zelfgerapporteerde psychologische en fysiologische gegevens over geestelijke gezondheid en autonome regulatie verzamelen. Deelnemers aan het onderzoek zijn volwassenen die zichzelf identificeren als Afro-Amerikaans, zwart en/of Latino, en die klinisch significante angstniveaus hebben.
De doelstellingen van de studie zijn:
- Doel 1: Vaststellen van de effectiviteit en duurzaamheid van een 8 weken durende Mindfulness DHP-interventie. De onderzoekers zullen zich concentreren op twee constructen die belangrijk zijn voor de geestelijke gezondheid en veronderstellen dat: A) Angst, zelfrapportage van stress en kwaliteit van leven aanzienlijk zullen verbeteren bij het vergelijken van: A.1) Pre-to-post interventie, en; A.2) Controle- versus interventiegroepen gedurende 8 weken en bij follow-up van 1 maand. B) Opwinding, autonome indices van HRV (die parasympathische activering weerspiegelen) zullen aanzienlijk verbeteren bij vergelijking van: B.1) Pre-to-post interventie, en; B.2) Controle- versus interventiegroepen gedurende 8 weken en bij follow-up van 1 maand.
- Doel 2: Vaststellen van de duurzaamheid van twee Mindfulness DHP-interventies, waarbij gebruik wordt gemaakt van retentie, gebruik (frequentie) en tevredenheid van de deelnemers.
- Doel 3: Onderzoek de associaties tussen aan COVID-19 gerelateerde stress, resultaten op het gebied van de geestelijke gezondheid en HRV. Onderzoek de mate waarin aan COVID-19 gerelateerde stress en geestelijke gezondheidssymptomen verband houden met HRV bij aanvang en hoe die relatie in de loop van de tijd verandert.
Deelnemers worden toegewezen aan 1 van de 3 armen van het onderzoek: MTIA-interventie, MAPP-interventie of wachtlijstcontrole. Alle deelnemers krijgen een apparaat toegestuurd waarop de SMILE-app is geïnstalleerd, en de apparatuur voor het opnemen van hartgegevens in huis. Alle deelnemers voltooien de psychometrische basismetingen en de fysiologische stresstest met behulp van de instructies in de SMILE-app. Degenen die zijn toegewezen aan de MTIA- of MAPP-interventiegroepen zullen vervolgens gedurende de daaropvolgende 8 weken deelnemen aan de hun toegewezen interventie. Gedurende deze 8 weken worden voor alle deelnemers tweewekelijks psychometrische en fysiologische gegevens ingevuld. Drie maanden na de initiële basislijn voltooien alle deelnemers een laatste psychometrische/fysiologische evaluatie.

Alle gegevensverzameling zal op afstand plaatsvinden en de interventiesessies zullen virtueel zijn (dat wil zeggen bij de deelnemers thuis). Deelnemers krijgen een tablet met de SMILE-app en een hartslagmeter en kunnen een virtuele introductieles over het gebruik van de apparatuur bekijken of eraan deelnemen. Alle psychologische en fysiologische gegevens worden verzameld via de SMILE-app. Deelnemers worden toegewezen aan 1 van de 3 groepen (MTIA, MAPP, WLC) en alle deelnemers voltooien elke geplande beoordeling, ongeacht de groepsopdracht.
Basisbeoordeling (week 0). Deelnemers vullen demografische en psychologische vragenlijsten in, gevolgd door een HRV-beoordelingsprotocol, dat erin bestaat de deelnemers te vragen de hartslagmeter aan te sluiten en de instructies/het model op de app te volgen om een cognitieve stresstest en een orthostatische test te voltooien.
Naast de demografische vragenlijst zijn de psychologische metingen die in de basisbeoordeling zijn opgenomen: GAD-7, COVID Stress Scale (COVID-SS), Connor-Davidson Resilience Scale (CD RISC), Mental Health Quality of Life (MHQoL), Coping Oriëntatie op problemen Ervaren inventarisatie (Brief COPE), Perceived Stress Scale (PSS), Posttraumatic Growth Inventory (PTGI), slaapstoornissen (Adult Patient-Reported Outcomes Measurement Information System (PROMIS) Short Form), PTSD-checklist, Centrum voor Epidemiologische Studies Depressie Schaal (CES-D), Cognitieve en Affectieve Mindfulness Schaal-Herzien (CAMS-R), en lichamelijke gezondheid (Short Form Health Survey, SF-12).
Interventie (week 1-8). Deelnemers toegewezen aan de MTIA- en MAPP-groepen zullen deelnemen aan de 8 weken durende interventie. Beide interventieprogramma's zijn bewuste meditatieve praktijken, ontworpen om de regulering van de aandacht op het bewustzijn van het huidige moment te cultiveren, en om mindfulness- en bewustzijnsvaardigheden te ontwikkelen om het omgaan met stress te verbeteren en stress te verminderen, waardoor angst wordt verminderd en het welzijn wordt verhoogd.
De MTIA zal een door een instructeur geleide, via internet geleverde (via Zoom), interactieve, groepsgebaseerde mindfulness-trainingsinterventie zijn die de training voor ongeveer 9 personen in groepsvorm zal omvatten, met opdrachten buiten de sessie. De MindfulnessAPP (MAPP) is een zelfbeheerde, via internet geleverde interventie, ontwikkeld door het SMILE-studieteam.
Gedurende de weken 1-6 wordt deelnemers in alle groepen tweewekelijks gevraagd om beoordelingen in te vullen, waaronder psychologische vragenlijsten (GAD-7, COVID-SS, CD RISC, CAMS-R) en hetzelfde HRV-beoordelingsprotocol als de basislijn. Deelnemers aan beide mindfulnessgroepen wordt gevraagd documentatie over de beoefening van mindfulness te verstrekken.
- 8 weken en vervolgbeoordeling in week 12: deelnemers in alle groepen wordt gevraagd beoordelingen in te vullen, waaronder psychologische vragenlijsten (GAD-7, COVID-SS, CD RISC, MHQoL, COPE, PSS, PTGI, PROMIS, PTSD-checklist, CES-D, CAMS-R en SF-12) en hetzelfde HRV-beoordelingsprotocol als de basislijn.

Inclusiecriteria:
- zichzelf identificeren als Afro-Amerikaans, Zwart, Spaans en/of Latino
- symptomen van angst vertonen, zoals bepaald op basis van de GAD-7-screeningsmaatregel (score tussen 8-14)
Uitsluitingscriteria:
- Huidige of geschiedenis van hartziekten
- Geschiedenis van een beroerte of dementie
- Diagnose van bewegingsstoornissen, zoals de ziekte van Parkinson, of verlamming
- Diagnose van genetische aandoeningen, zoals het Downsyndroom of het Fragile-X-syndroom
- Diagnose van autisme
- Diagnose van schizofrenie, psychose, dissociatieve stoornis, manie/bipolaire stoornis, ernstige depressie of een persoonlijkheidsstoornis
- Voorgeschiedenis van ernstige geestelijke of gedragsmatige gezondheidsproblemen waarvoor een verblijf in een ziekenhuis of behandelcentrum nodig was in de afgelopen 12 maanden
- Hartmedicijnen gebruiken (anders dan bloeddrukmedicijnen)
- Medicatie tegen epilepsie nemen

Actieve vergelijker: "MTIA"-interventie
De Mindfulness Training Instructor Administered ("MTIA") interventie zal de volgende elementen omvatten: training in een aangepast mindfulnessprogramma van 8 weken, 90 minuten per week, dat extra nadruk legt op training die haalbaar en relevant is voor ras/etnische groepen , inclusief: a) didactiek over de relevantie van stress, coping en veerkracht, b) bewust medeleven met zichzelf en anderen; c) aandachtige communicatie, waaronder non-verbale aandacht, aandachtig luisteren en aandachtig spreken.
De MTIA zal een door een instructeur geleide, via internet geleverde (via Zoom), interactieve, groepsgebaseerde mindfulness-trainingsinterventie zijn die de training voor ongeveer 9 personen in groepsvorm zal omvatten, met opdrachten buiten de sessie.

Mindfulness-meditatie is beschreven als een gedragstechniek die betrekking heeft op de opzettelijke zelfregulering van de aandacht op de ervaring van het huidige moment, gecombineerd met het loslaten van cognitieve fixatie op gedachten (of het nu gaat om eenvoudige beelden of complexe verhaallijnen) over het verleden of de toekomst.
Door training in mindfulness leren individuen een niet-oordelende staat van bewustzijn van het huidige moment op te roepen en te behouden.

Actieve vergelijker: "MAPP"-interventie
De MindfulnessAPP ("MAPP") is een zelfbeheerde interventie ontwikkeld door het SMILE-studieteam.
De MAPP is voor individueel gebruik, met acht MAPP-sessies samengesteld uit mindfulness-oefeningen en didactiek die aansluiten bij de MTIA-sessies.
Omdat de wekelijkse MTIA-les 90 minuten zal duren, wordt bij de MAPP-opdrachten aanbevolen om ongeveer 90 minuten per week aan de toegewezen les te besteden, maar in een flexibel formaat dat handig is voor de deelnemer.
Bovendien zal elke sessie op mindfulness gebaseerde oefenopdrachten bevatten die over het algemeen variëren van 10 tot 30 minuten per dag.
Het totale aantal voorgestelde dagen voor voltooiing bedraagt 49 dagen, vergelijkbaar met de tijd van start tot finish van een traditionele MTIA-sessie van 8 weken; er zal echter flexibiliteit zijn binnen dit geïndividualiseerde programma.

Geen tussenkomst: Wachtlijstcontrole
De wachtlijstcontrolegroep (WLC) zal deelnemen aan alle onderzoeksbeoordelingssessies, maar zal de Mindfulness-interventie pas aangeboden krijgen nadat hun rol in het onderzoek is voltooid.

This instrument includes 8 items rated on a 5-point Likert scale (1 = not at all to 5 = very much). Raw scores are summed and converted to standardized T-scores based on the PROMIS adult reference population. T-scores have a mean of 50 and a standard deviation of 10, where 50 represents the average level of sleep disturbance in the general U.S. population. Higher T-scores indicate greater sleep disturbance (worse outcome), while lower T-scores indicate less sleep disturbance (better outcome). Clinically, T-scores of approximately 60 or higher are considered indicative of elevated sleep disturbance, and scores of 70 or higher may reflect severe sleep disturbance. |

Client Satisfaction Questionnaire-internet at the End of the Intervention

The Client satisfaction questionnaire-internet (CSQ-I) is an 8-item measure developed in Germany to assess satisfaction with telehealth interventions. the CSQ-I consists of seven statements that address satisfaction with an online program and willingness to refer others.
The investigators have adjusted the 4 response categories for an American sample to use the standard agreement scale (1=strongly disagree, 2= mildly disagree, 3=mildly agree, 4= strongly agree).
Mean scores are calculated which can range from 1-4 with higher scores representing greater satisfaction.
The questionnaire will be assessed among participants in the intervention arms at the end of the interventions.
