SMILE: Badanie kliniczne mające na celu ocenę uważności jako interwencji w populacjach rasowych i etnicznych podczas COVID-19 (SMILE)
Cyfrowa platforma zdrowia (DHP) zapewniająca uważność jako interwencję w zakresie zarządzania stresem z wykorzystaniem elektronicznej dokumentacji zdrowotnej (SMILE) dla populacji rasowych i etnicznych podczas pandemii COVID-19: badanie kliniczne
Celem tego badania klinicznego jest ocena aplikacji SMILE, cyfrowej platformy zdrowia (DHP), która zapewni interwencję uważności mającą na celu łagodzenie stresu związanego z Covid-19. Ponadto aplikacja SMILE będzie zdalnie gromadzić zgłaszane przez siebie psychologiczne i fizjologiczne wskaźniki zdrowia psychicznego i regulacji autonomicznej. Uczestnikami badania są osoby dorosłe, które identyfikują się jako Afroamerykanie, osoby czarnoskóre i/lub Latynosi i które wykazują klinicznie istotny poziom lęku.
Celem badania jest:
- Cel 1: Ustalenie skuteczności i trwałości 8-tygodniowej interwencji DHP Mindfulness. Badacze skoncentrują się na dwóch konstruktach ważnych dla zdrowia psychicznego i postawią hipotezę, że: A) Lęk, poziom stresu i jakość życia znacznie się poprawią w wyniku porównania: A.1) Interwencja przed i po; A.2) Grupa kontrolna vs. grupy interwencyjne w ciągu 8 tygodni i po 1 miesiącu obserwacji. B) Pobudzenie, autonomiczne wskaźniki HRV (odzwierciedlające aktywację układu przywspółczulnego) znacznie się poprawią, porównując: B.1) Interwencję przed i po; B.2) Grupa kontrolna vs. grupy interwencyjne w ciągu 8 tygodni i po 1 miesiącu obserwacji.
- Cel 2: Ustalenie trwałości dwóch interwencji DHP Mindfulness, wykorzystujących zachowanie, wykorzystanie (częstotliwość) i satysfakcję uczestników.
- Cel 3: Zbadanie powiązań między stresem związanym z Covid-19, skutkami zdrowia psychicznego i HRV. Zbadaj, w jakim stopniu stres i objawy zdrowia psychicznego związane z Covid-19 są powiązane z HRV na początku badania i jak ta zależność zmienia się w czasie.
Uczestnicy zostaną przydzieleni do 1 z 3 ramion badania: interwencja MTIA, interwencja MAPP lub kontrola na liście oczekujących. Wszyscy uczestnicy otrzymają pocztą urządzenie z zainstalowaną aplikacją SMILE oraz sprzęt do rejestracji danych kardiologicznych w domu. Wszyscy uczestnicy przeprowadzą podstawowe pomiary psychometryczne i test stresu fizjologicznego, korzystając z instrukcji podanych w aplikacji SMILE. Osoby przydzielone do grup interwencyjnych MTIA lub MAPP będą następnie uczestniczyć w przypisanej im interwencji przez kolejne 8 tygodni. W ciągu tych 8 tygodni dane psychometryczne i fizjologiczne dla wszystkich uczestników będą uzupełniane co dwa tygodnie. 3 miesiące po początkowym punkcie odniesienia wszyscy uczestnicy przejdą ostateczną ocenę psychometryczną/fizjologiczną.
Przegląd badań
Szczegółowy opis
Całość gromadzenia danych będzie odbywać się zdalnie, a sesje interwencyjne będą wirtualne (tj. w domach uczestników). Uczestnicy otrzymają tablet z aplikacją SMILE i czujnikiem tętna oraz będą mogli obejrzeć wirtualny samouczek wprowadzający na temat korzystania ze sprzętu lub wziąć w nim udział. Wszystkie dane psychologiczne i fizjologiczne będą zbierane za pośrednictwem aplikacji SMILE. Uczestnicy zostaną przydzieleni do 1 z 3 grup (MTIA, MAPP, WLC), a wszyscy uczestnicy ukończą każdą zaplanowaną ocenę, niezależnie od przydziału grupowego.
Ocena wyjściowa (tydzień 0). Uczestnicy wypełnią kwestionariusze demograficzne i psychologiczne, a następnie przeprowadzą protokół oceny HRV, który polega na poproszeniu uczestników o podłączenie czujnika tętna i przestrzeganiu instrukcji/modelu w aplikacji w celu ukończenia poznawczego testu wysiłkowego i testu ortostatycznego.
Oprócz kwestionariusza demograficznego, pomiary psychologiczne uwzględnione w ocenie wyjściowej to: GAD-7, Skala Stresu COVID (COVID-SS), Skala Odporności Connora-Davidsona (CD RISC), Jakość Życia w zakresie Zdrowia Psychicznego (MHQoL), Radzenie sobie Inwentarz doświadczanych problemów (krótki COPE), skala odczuwanego stresu (PSS), inwentarz wzrostu potraumatycznego (PTGI), zaburzenia snu (skrócony formularz systemu informacji o wynikach zgłaszanych przez dorosłych pacjentów (PROMIS), lista kontrolna PTSD, Centrum Badań Epidemiologicznych Depresja Skala (CES-D), poprawiona skala uważności poznawczej i afektywnej (CAMS-R) oraz zdrowie fizyczne (krótka ankieta dotycząca stanu zdrowia, SF-12).
Interwencja (tygodnie 1-8). Uczestnicy przydzieleni do grup MTIA i MAPP wezmą udział w 8-tygodniowej interwencji. Obydwa programy interwencyjne są uważnymi praktykami medytacyjnymi, których celem jest kultywowanie regulacji uwagi na świadomość chwili obecnej, a także rozwijanie umiejętności uważności i świadomości w celu poprawy radzenia sobie i redukcji stresu, zmniejszając w ten sposób lęk i zwiększając dobrostan.
MTIA będzie prowadzona przez instruktora, dostarczana przez Internet (za pośrednictwem Zoom), interaktywna, grupowa interwencja w zakresie treningu uważności, która obejmie szkolenie dla około 9 osób w formacie grupowym, z zadaniami poza sesją. MindfulnessAPP (MAPP) to interwencja realizowana samodzielnie, dostarczana przez Internet, opracowana przez zespół badawczy SMILE.
Co dwa tygodnie w tygodniach 1-6 uczestnicy wszystkich grup zostaną poproszeni o wypełnienie ocen obejmujących kwestionariusze psychologiczne (GAD-7, COVID-SS, CD RISC, CAMS-R) i ten sam protokół oceny HRV, co w badaniu wyjściowym. Uczestnicy obu grup uważności zostaną poproszeni o dostarczenie dokumentacji dotyczącej praktyki uważności.

Kryteria przyjęcia:

Interwencja prowadzona przez instruktora treningu uważności („MTIA”) będzie obejmować następujące elementy: szkolenie w ramach 8-tygodniowego, 90-minutowego tygodniowo zmodyfikowanego programu uważności, który kładzie dodatkowy nacisk na trening wykonalny i odpowiedni dla grup rasowych/etnicznych , w tym: a) dydaktyka dotycząca znaczenia stresu, radzenia sobie i odporności, b) uważnego współczucia dla siebie i innych; c) uważna komunikacja, w tym uważność niewerbalna, uważne słuchanie i uważne mówienie.
MTIA będzie prowadzona przez instruktora, dostarczana przez Internet (za pośrednictwem Zoom), interaktywna, grupowa interwencja w zakresie treningu uważności, która obejmie szkolenie dla około 9 osób w formacie grupowym, z zadaniami poza sesją.

Medytację uważności opisano jako technikę behawioralną polegającą na celowej samoregulacji uwagi na doświadczenia chwili obecnej, połączoną z uwolnieniem poznawczej fiksacji na myślach (czy to prostych obrazach, czy złożonych fabułach) dotyczących przeszłości lub przyszłości.
Poprzez trening uważności ludzie uczą się wywoływać i podtrzymywać nieoceniający stan świadomości chwili obecnej.

Aktywny komparator: Interwencja „MAPP”.
Aplikacja MindfulnessAPP („MAPP”) to interwencja do samodzielnego stosowania opracowana przez zespół badawczy SMILE.
MAPP jest przeznaczony do użytku indywidualnego i obejmuje osiem sesji MAPP składających się z ćwiczeń uważności i dydaktyki odpowiadających sesjom MTIA.
Ponieważ cotygodniowe zajęcia MTIA będą trwać 90 minut, zadania MAPP będą zalecać spędzanie około 90 minut tygodniowo na przypisanej lekcji, ale w elastycznym formacie dogodnym dla uczestnika.
Ponadto każda sesja będzie zawierać ćwiczenia oparte na uważności, trwające zazwyczaj od 10 do 30 minut dziennie.
Całkowita liczba sugerowanych dni na ukończenie wyniesie 49 dni, co jest porównywalne z czasem od rozpoczęcia do zakończenia tradycyjnej 8-tygodniowej sesji MTIA; jednakże w ramach tego zindywidualizowanego programu będzie zapewniona elastyczność.

T-scores have a mean of 50 and a standard deviation of 10, where 50 represents the average level of sleep disturbance in the general U.S. population. Client Satisfaction Questionnaire-internet at the End of the Intervention

The Client satisfaction questionnaire-internet (CSQ-I) is an 8-item measure developed in Germany to assess satisfaction with telehealth interventions. the CSQ-I consists of seven statements that address satisfaction with an online program and willingness to refer others.
The investigators have adjusted the 4 response categories for an American sample to use the standard agreement scale (1=strongly disagree, 2= mildly disagree, 3=mildly agree, 4= strongly agree).
Mean scores are calculated which can range from 1-4 with higher scores representing greater satisfaction.
The questionnaire will be assessed among participants in the intervention arms at the end of the interventions.
