El objetivo de este ensayo clínico es evaluar la aplicación SMILE, una plataforma de salud digital (DHP), que ofrecerá una intervención de atención plena, diseñada para mitigar el estrés relacionado con COVID. Además, la aplicación SMILE recopilará de forma remota métricas psicológicas y fisiológicas de salud mental y regulación autónoma autoinformadas. Los participantes del estudio son adultos que se identifican como afroamericanos, negros y/o latinos, y que tienen niveles de ansiedad clínicamente significativos.
Los objetivos del estudio son:
- Objetivo 1: Establecer la eficacia y durabilidad de una intervención DHP de Mindfulness de 8 semanas. Los investigadores se centrarán en dos constructos importantes para la salud mental y plantearán la hipótesis de que: A) Las medidas de ansiedad, estrés autoinformado y calidad de vida mejorarán significativamente al comparar: A.1) Intervención previa a posterior, y; A.2) Grupos de control versus intervención durante 8 semanas y al mes de seguimiento. B) La excitación, los índices autonómicos de HRV (que reflejan la activación parasimpática) mejorarán significativamente, al comparar: B.1) Intervención antes y después, y; B.2) Grupos de control versus intervención durante 8 semanas y al mes de seguimiento.
- Objetivo 2: Establecer la sostenibilidad de dos intervenciones de Mindfulness DHP utilizando retención, uso (frecuencia) y satisfacción de los participantes.
- Objetivo 3: examinar las asociaciones entre el estrés relacionado con COVID-19, los resultados de salud mental y la VFC. Examinar hasta qué punto el estrés y los síntomas de salud mental relacionados con COVID-19 están relacionados con la VFC al inicio del estudio y cómo esa relación cambia con el tiempo.
Los participantes serán asignados a 1 de 3 brazos del estudio: intervención MTIA, intervención MAPP o control en lista de espera. A todos los participantes se les enviará por correo un dispositivo con la aplicación SMILE instalada y el equipo para registrar datos cardíacos en el hogar. Todos los participantes completarán las medidas psicométricas de referencia y la prueba de estrés fisiológico utilizando las instrucciones proporcionadas en la aplicación SMILE. Aquellos asignados a los grupos de intervención MTIA o MAPP participarán en la intervención asignada durante las siguientes 8 semanas. Durante estas 8 semanas, se completarán datos psicométricos y fisiológicos quincenalmente para todos los participantes. 3 meses después de la línea de base inicial, todos los participantes completarán una evaluación psicométrica/fisiológica final.

Descripción detallada
Toda la recopilación de datos se realizará de forma remota y las sesiones de intervención serán virtuales (es decir, en los hogares de los participantes). Los participantes recibirán una tableta cargada con la aplicación SMILE y un monitor de frecuencia cardíaca y verán o participarán en un tutorial introductorio virtual sobre el uso del equipo. Todos los datos psicológicos y fisiológicos se recopilarán a través de la aplicación SMILE. Los participantes serán asignados a 1 de 3 grupos (MTIA, MAPP, WLC) y todos los participantes completarán cada evaluación programada, independientemente de la asignación de grupo.
Evaluación inicial (semana 0). Los participantes completarán cuestionarios demográficos y psicológicos seguidos de un protocolo de evaluación de la VFC, que consiste en pedirles a los participantes que conecten el monitor de frecuencia cardíaca y sigan las instrucciones/modelo de la aplicación para completar una prueba de estrés cognitivo y una prueba ortostática.
Además del cuestionario demográfico, las medidas psicológicas incluidas en la evaluación inicial son: GAD-7, COVID Stress Scale (COVID-SS), Connor-Davidson Resilience Scale (CD RISC), Mental Health Quality of Life (MHQoL), Coping Inventario de orientación a problemas experimentados (COPE breve), Escala de estrés percibido (PSS), Inventario de crecimiento postraumático (PTGI), alteraciones del sueño (formato abreviado del Sistema de información de medición de resultados informados por pacientes adultos (PROMIS), Lista de verificación de trastorno de estrés postraumático, Centro de estudios epidemiológicos, depresión Escala (CES-D), Escala Cognitiva y Afectiva de Atención Plena Revisada (CAMS-R) y salud física (Encuesta de salud de formato abreviado, SF-12).
Intervención (semanas 1-8). Los participantes asignados a los grupos MTIA y MAPP participarán en la intervención de 8 semanas. Ambos programas de intervención son prácticas de meditación consciente, diseñadas para cultivar la regulación de la atención a la conciencia del momento presente, así como desarrollar habilidades de atención plena y conciencia para mejorar el afrontamiento y reducir el estrés, reduciendo así la ansiedad y aumentando el bienestar.
La MTIA será una intervención de entrenamiento de atención plena interactiva, grupal e interactiva, dirigida por un instructor, impartida por Internet (a través de Zoom) que incorporará la capacitación para aproximadamente 9 personas en un formato grupal, con tareas fuera de la sesión. MindfulnessAPP (MAPP) es una intervención autoadministrada a través de Internet desarrollada por el equipo de estudio SMILE.
Cada dos semanas durante las semanas 1 a 6, se pedirá a los participantes de todos los grupos que completen evaluaciones que incluyen cuestionarios psicológicos (GAD-7, COVID-SS, CD RISC, CAMS-R) y el mismo protocolo de evaluación de la VFC que la línea de base. A los participantes de ambos grupos de atención plena se les pedirá que proporcionen documentación de la práctica de la atención plena.
- 8 semanas y evaluación de seguimiento en la semana 12: a los participantes de todos los grupos se les pedirá que completen evaluaciones que incluyen cuestionarios psicológicos (GAD-7, COVID-SS, CD RISC, MHQoL, COPE, PSS, PTGI, PROMIS, PTSD Checklist, CES-D, CAMS-R y SF-12) y el mismo protocolo de evaluación de la VFC que la línea de base.

Criterios de inclusión:
- identificarse como afroamericano, negro, hispano y/o latino
- demostrar síntomas de ansiedad, según lo determinado según la medida de detección GAD-7 (puntuación entre 8 y 14)
Criterio de exclusión:
- Enfermedad cardíaca actual o previa.
- Historia de accidente cerebrovascular o demencia.
- Diagnóstico de trastornos del movimiento, como la enfermedad de Parkinson o parálisis.
- Diagnóstico de trastornos genéticos, como el síndrome de Down o el síndrome de X frágil
- Diagnóstico de autismo
- Diagnóstico de esquizofrenia, psicosis, trastorno disociativo, manía/trastorno bipolar, depresión mayor o trastorno de la personalidad.
- Historial de problemas graves de salud mental o conductual que requirieron estadía en un hospital o centro de tratamiento dentro de los últimos 12 meses
- Tomar medicamentos para el corazón (que no sean medicamentos para la presión arterial)
- Tomar medicamentos para las convulsiones
- Actualmente tomando medicamentos o suplementos opioides

Comparador activo: Intervención "MTIA"

Comparador activo: Intervención "MAPP"
MindfulnessAPP ("MAPP") es una intervención autoadministrada desarrollada por el equipo de estudio SMILE.
El MAPP es de uso individual, con ocho sesiones MAPP compuestas por ejercicios de mindfulness y didácticas que corresponden a las sesiones MTIA.
Como la clase semanal de MTIA tendrá una duración de 90 minutos, las tareas de MAPP recomendarán dedicar aproximadamente 90 minutos por semana a cubrir la lección asignada, pero en un formato flexible y conveniente para el participante.
Además, cada sesión contendrá tareas de práctica basadas en la atención plena que generalmente oscilarán entre 10 y 30 minutos por día.
El número total de días sugeridos para completar será 49 días, comparable al tiempo desde el principio hasta el final de una sesión tradicional de MTIA de 8 semanas; sin embargo, habrá flexibilidad dentro de este programa individualizado.

The Patient-Reported Outcomes Measurement Information System (PROMIS) Sleep Disturbance 8b Short Form assesses self-reported perceptions of sleep quality, sleep depth, and restoration over the past 7 days. This instrument includes 8 items rated on a 5-point Likert scale (1 = not at all to 5 = very much). Raw scores are summed and converted to standardized T-scores based on the PROMIS adult reference population. T-scores have a mean of 50 and a standard deviation of 10, where 50 represents the average level of sleep disturbance in the general U.S. population. Higher T-scores indicate greater sleep disturbance (worse outcome), while lower T-scores indicate less sleep disturbance (better outcome). Clinically, T-scores of approximately 60 or higher are considered indicative of elevated sleep disturbance, and scores of 70 or higher may reflect severe sleep disturbance. |

Client Satisfaction Questionnaire-internet at the End of the Intervention

The Client satisfaction questionnaire-internet (CSQ-I) is an 8-item measure developed in Germany to assess satisfaction with telehealth interventions. the CSQ-I consists of seven statements that address satisfaction with an online program and willingness to refer others.
The investigators have adjusted the 4 response categories for an American sample to use the standard agreement scale (1=strongly disagree, 2= mildly disagree, 3=mildly agree, 4= strongly agree).
Mean scores are calculated which can range from 1-4 with higher scores representing greater satisfaction.
The questionnaire will be assessed among participants in the intervention arms at the end of the interventions.
