Een fase 3, gerandomiseerde, dubbelblinde, placebo-gecontroleerde studie van Felzartamab bij volwassenen met IgA-nefropathie (Prevail)

In deze studie zullen onderzoekers meer leren over het gebruik van Felzartamab bij deelnemers met immunoglobuline A nefropathie (IGAN). Deze studie zal zich richten op deelnemers die eiwitten in hun urine (proteïnurie) hebben als gevolg van beschadigde nieren.

Het belangrijkste doel van de studie is om te leren over het effect dat Felzartamab heeft op proteïnurie. De belangrijkste vraag die onderzoekers willen beantwoorden is:

• Hoeveel verandert de hoeveelheid eiwitten in de urine van het begin van de studie naar week 36?

Onderzoekers zullen leren over het effect dat Felzartamab heeft op het vermogen van de nieren om bloed te filteren. Ze zullen ook meer leren over de veiligheid van Felzartamab en hoe het wordt verwerkt door het lichaam.

De studie zal als volgt worden uitgevoerd:

  • Deelnemers worden gescreend om te controleren of ze deelnemen aan de studie.
  • Deelnemers worden gerandomiseerd om Felzartamab of een placebo te ontvangen. Een placebo ziet eruit als het studiemedicijn, maar bevat geen echt medicijn.
  • Noch de onderzoekers noch de deelnemers zullen weten wat de deelnemers zullen ontvangen.
  • Deelnemers ontvangen Felzartamab of placebo als intraveneuze (IV) infusies. De behandelingsperiode duurt 24 weken.
  • Daarna gaan de deelnemers een vervolgperiode in die 80 weken duurt.
  • In totaal zullen deelnemers 17 studiebezoeken hebben. Deelnemers blijven ongeveer 2 jaar in het onderzoek.

Het primaire doel van de studie is om de werkzaamheid van Felzartamab te evalueren in vergelijking met placebo op proteïnurie bij deelnemers met immunoglobuline A nefropathie (IGAN). Het belangrijkste secundaire doel van de studie is om de werkzaamheid van Felzartamab te evalueren in vergelijking met placebo op nierfuncties bij deelnemers met IGAN. De extra secundaire doelstellingen zijn het evalueren van de werkzaamheid van Felzartamab in vergelijking met placebo op aanvullende klinische eindpunten en om de farmacokinetiek (PK) en immunogeniteit van felzartamab te beoordelen.

Belangrijkste inclusiecriteria:

  • Biopsie-bevestigde diagnose van IgAN in de afgelopen 10 jaar voorafgaand aan de handtekening van het geïnformeerde toestemmingsformulier (ICF). Voor deelnemers met diabetes mellitus type 2 moet de biopsiebevestiging van de IGAN -diagnose worden gedaan binnen de afgelopen 24 maanden voorafgaand aan het ondertekenen van de ICF.
  • Een EGFR ≥ 30 ml/min/1.73m^2 bij screening zoals berekend met behulp van de creatinineformule van de 2021 chronische nierziekte-epidemiologie (CKD-EPI). Een EGFR van ≥ 20 en <30 ml/min/1.73m^2 is acceptabel voor de cohorten 3 en 4.
  • Proteïnurie van ≥ 1,0 gram per dag (g/dag) of UPCR ≥0,8 gram per gram (g/g) zoals beoordeeld door een adequate 24-uurs urinecollectie.
  • Klinisch stabiel op een maximaal getolereerde dosis of maximaal goedgekeurde dosis angiotensine-converterende enzymremmers (ACEI) of angiotensinereceptorblokkers (ARB) gedurende ten minste 12 weken voorafgaand aan screening, of intolerant van ACEI of ARB. Indien intolerant, moet dit worden besproken met de medische monitor voorafgaand aan randomisatie. Deelnemers kunnen ook gebruik maken van natriumglucose Cotransporter-2-remmers (SGLT2IS), endothelinereceptorantagonisten (ERA's) goedgekeurd voor de behandeling van IGAN, en/of mineralocorticoïde receptorantagonisten (MRA's) zolang de dosis stabiel is voor ten minste 12 weken voorafgaand aan screening. Deelnemers moeten voor de duur van het onderzoek in stabiele doses van deze achtergrondmedicijnen blijven. Zodra de ICF is ondertekend en daarna kunnen de doses niet worden gewijzigd tijdens de studie, noch de geneesmiddelen die worden stopgezet, behalve als ze worden geacht met een AE. Deelnemers die Sparrentan gebruiken, is niet toegestaan ​​om gelijktijdige ACEI- of ARB -medicatie te gebruiken.

Belangrijkste uitsluitingscriteria:

  • Secundaire vormen van IgAN, aangegeven door de aanwezigheid van een andere systemische ziekte die mogelijk leidt tot IGA -afzettingen zoals bepaald door de onderzoeker.
  • Geschiedenis van de snel progressieve variant van IgAN, gedefinieerd als EGFR-verlies met> 50% per 3 maanden en niet verklaard door veranderingen in blokkade van Renin-Angiotensines System (RAS) of andere factoren.
  • Nefrotisch syndroom waarvan wordt aangenomen dat het te wijten is aan minimale variant voor veranderingenziekten (MCD).
  • Gelijktijdig andere progressieve glomerulonefritis of niet-immunologische glomerulaire aandoeningen zoals diabetische nefropathie.
  • Type 2 diabetes mellitus met hemoglobine A1C (HbA1C)> 8% bij screening, of bewijs van diabetische nefropathie op biopsie, geschiedenis van diabetische microvasculaire of macrovasculaire aandoeningen (bijv. Diabetische retinopathie, perifere neuropathie).
  • Elke gediagnosticeerde of vermoedelijke immunosuppressieve of immunodeficiënte toestand zoals Asplenia, Human Immunodeficiency Virus (HIV), primaire immunodeficiënties, orgaan- of beenmergtransplantatie, met uitzondering van hoornvliestransplantaties.
  • Eerder behandeld met immunosuppressieve of andere immunomodulerende middelen zoals maar niet beperkt tot cyclofosfamide, rituximab, infliximab, eculizumab, canakinumab, mycophenolaat mofetil (mmf) of mycofenolaat -natrium (mps), mps), tacrolimus, sirolimus, agolimus, of systeem corticic corticic corticicus of systeem corticicus (> 7,5 milligram per deciliter [mg/d] prednison/prednisolon equivalent) binnen 4 maanden (of 12 maanden voor rituximab) voorafgaand aan screening.
  • Deelnemers die momenteel worden behandeld met orale budesonide. Deelnemers die deze therapie hebben gestopt ≥ 4 maanden voorafgaand aan de screening, kunnen in aanmerking komen.
  • Actieve klinisch significante infecties, bekende geschiedenis van terugkerende klinisch significante infectie, of screening laboratoriumbewijs in overeenstemming met een actieve infectie, of IV anti-infectieven (antibacterials, antivirale of antischimmelmiddelen). Deelnemers met een geschiedenis van opportunistische infecties zijn uitgesloten.
  • Hypogammaglobulinemie: serum immunoglobine G (IgG) <6,0 gram per liter (g/l), bij screening.

Opmerking: andere protocol-gedefinieerde inclusie/uitsluitingscriteria kunnen van toepassing zijn.
