Karakterisering van vroege biologische en mechanische profielen in sepsis-geassocieerde ARDS voor het bestuderen van compartimentalisering (seriële bronchoalveolaire lavage en plasmabiomarkers) om inflammatoire en hybride subfenotypen te identificeren
Sepsis-geassocieerd acuut respiratoir distress syndroom (ARDS) is een van de dodelijkste en biologisch meest heterogene vormen van ademhalingsfalen. Ondanks uniforme diagnostische criteria vertonen patiënten met septische ARDS een grote variabiliteit in inflammatoire intensiteit, alveolair epitheel- en endotheelschade, alveolaire vloeistofsamenstelling, mechanische ventilatie-eigenschappen en klinische evolutie. Vroege identificatie van deze verschillen kan betere prognostiek en preciezere behandeling mogelijk maken.
Deze prospectieve observationele studie beoogt de vroegste fasen van septische ARDS diepgaand te karakteriseren door seriële bronchoalveolaire lavage (BAL) op 0, 24 en 72 uur te integreren met parallelle plasma biomarker-profiling en gedetailleerde mechanische ventilatiegegevens. Dit ontwerp legt het evoluerende biologische en fysiologische landschap van septische ARDS vast tijdens het meest dynamische venster. Het centrale doel is het identificeren van systemische, alveolaire en hybride bio-mechano-inflammatoire subfenotypes die gepersonaliseerde benaderingen voor ondersteuning, risicostratificatie en toekomstige interventiestudies kunnen informeren.

Acuut respiratoir distress syndroom veroorzaakt door sepsis is geen uniforme ziekte maar een syndroom dat meerdere biologische en mechanische toestanden omvat. De Berlijnse Definitie biedt een nuttig klinisch aanknopingspunt maar vangt niet de diepgaande heterogeniteit die wordt gevonden in moleculaire pathways, alveolaire immuunactivatie, epitheelverstoring, endotheeldysfunctie en mechanische beademingsresponsen.
De eerste 72 uur van ARDS vertegenwoordigen een kritiek, snel evoluerend biologisch landschap waar systemische cytokinerelease, alveolaire epitheelbeschadiging, endotheelactivatie en capillaire-alveolaire permeabiliteit allemaal dramatisch fluctueren. Deze vroege verschuivingen worden verondersteld de downstream trajecten te bepalen zoals beademingsafhankelijkheid, multiorgaandysfunctie en mortaliteit. Het vastleggen van dit dynamische proces vereist herhaalde bemonstering op vooraf bepaalde intervallen in plaats van de traditionele eenmalige meting.
ARDS ontstaat voornamelijk in de alveolaire ruimte; toch vertrouwen de meeste studies uitsluitend op plasmabiomarkers, die slechts een gedeeltelijk venster bieden op de alveolus. Het pulmonale compartiment gedraagt zich vaak onafhankelijk van de systemische circulatie vanwege de compartimentalisatie van inflammatoire mediatoren. BAL-bemonstering biedt daarom een unieke mogelijkheid om de long direct te onderzoeken. Aan de andere kant weerspiegelen ventilatoire mechanica - in het bijzonder drivende druk, plateaudruk, longcompliantie en ventilatoire ratio - de biomechanische stresscondities die aan de long worden opgelegd, die kunnen interageren met of zelfs biologische schade kunnen verergeren.
Zo vormen ontsteking, epitheelschade, endotheellekkage, alveolaire overstroming en mechanische stress onderling afhankelijke dimensies van het vroege ARDS-proces.
Deze studie integreert:
- Seriële BAL om alveolaire cytokinen, epitheel- en endotheelmarkers, eiwitten en permeabiliteitsmarkers te meten.
- Seriële plasmabiomarkers om systemische inflammatoire fenotypes te identificeren en ze te vergelijken met alveolaire fenotypes.
- Mechanische beademingsparameters om biomechanische stress en zijn rol in schadeversterking te kwantificeren.
- Klinische eindpunten en fysiologie om biologische patronen met werkelijke uitkomsten te verbinden.
Deze multidimensionale dataset is ontworpen om biologische, mechanische en hybride subfenotypes te onthullen die kunnen verklaren waarom patiënten die klinisch vergelijkbaar zijn uiteenlopen in sterk verschillende trajecten.

- Invasieve beademing
- Verwacht ≥72 uur geïntubeerd te blijven

Exclusiecriteria:
- Contra-indicaties voor bronchoscopie/BAL
- Refractaire hemodynamische instabiliteit
- Zwangerschap
- Patiënten met longtransplantatie

Sepsis or Septic shock requiring mechanical ventilation
Adult ICU patients with sepsis or septic shock requiring invasive mechanical ventilation within 72 hours from sepsis diagnosis will be consecutively enrolled and followed for 96 hours after sepsis diagnosis.

At baseline (T0), blood samples and standardized respiratory physiological measurements, including gas exchange and ventilatory mechanics, will be obtained from all participants. Bronchoalveolar lavage (BAL) will be performed only in invasively mechanically ventilated patients meeting predefined safety criteria. T1 will occur 24 hours after T0 in patients already intubated or at the time of endotracheal intubation if this occurs within 72 hours of sepsis diagnosis. T2 will be performed 96 hours after sepsis diagnosis and at least 24 hours after the previous assessment. At each study point plasma sampling and physiological measurements will be repeated, while BAL will be repeated only when predefined safety criteria are fulfilled. All clinical management will remain at the discretion of the treating ICU team. |
