Deze studie wordt uitgevoerd om het effect van thoracale mobiliteit versus heupmobiliteitsoefeningen op kernstabilisatie te bepalen op pijnintensiteit
, functionele beperking, lumbale bewegingsuitslag, spinale mobiliteit, levenskwaliteit en bewegingsangst bij de behandeling van patiënten met lumbale spondylitis.

Inclusiecriteria:
- 1. Patiënten worden verwezen door een orthopedisch chirurg met een bevestigde diagnose van lumbale spondylose met beeldvormende bevindingen (MRI, CT of röntgenrapporten van de afgelopen 6 maanden) (Battié et al., 2014). 2. De duur van de symptomen van de patiënt is langer dan 3 maanden (Oliveira et al., 2022). 3. De leeftijd van de patiënten ligt tussen 40 en 60 jaar (Kalichman & Hunter, 2008). 4. Gelokaliseerde rugpijn en pijn die uitstraalt naar de onderste ledematen zonder distale betrokkenheid onder de knie met een NPRS hoger dan 3 (Thompson et al., 2021).
Exclusiecriteria:
- Voorgeschiedenis van rug- of heupchirurgie (Williams et al., 2022).
- Rode vlaggen: Cauda equina-syndroom, fractuur, infectie, maligniteit (Finucane et al., 2020).
- Neurologische aandoeningen: Parkinsonisme, multiple sclerose of andere neurologische uitval (Odzimek et al., 2023).
- Ontstekingsaandoeningen: Reumatoïde artritis, ankyloserende spondylitis of infectieuze rugaandoeningen (Lee et al., 2018).
- Cognitieve beperking: Niet in staat om instructies te begrijpen of op te volgen (Martinez et al., 2024).
- Zwangerschap.

Experimenteel: Kernstabilisatieoefeningen (controlegroep)
Voordat met de oefening wordt begonnen, wordt patiënten geleerd de lumbale lordose te verminderen door de buikspieren aan te spannen en in te trekken en de neutrale positie van de lumbale wervelkolom en het bekken te vinden door het bekken naar voren en naar achteren te bewegen (bekkenkanteling, brug, vogel-hond en plank etc.).
In elke sessie wordt eerst de neutrale positie gevonden en wordt erop gelet de neutrale positie gedurende de hele oefening te behouden.
Daarnaast worden oefeningen voor de kernstabiliteit gegeven, waaronder gelijktijdige contracties van de multifidus- en bekkenbodemspieren, in verschillende houdingen zoals rugligging, buikligging, kruipen, brug, knielen, zitten en staan.
De oefeningen bestaan uit 3 niveaus (van gemakkelijk tot moeilijk).
Bij de oefeningen op gevorderd niveau wordt patiënten gevraagd de neutrale kromming van de lumbale wervelkolom te behouden, worden weerstandsoefeningen voor de ledematen toegevoegd en worden de oefeningen geleidelijk voltooid.
Elke oefening wordt uitgevoerd in 3 sets van 8-10 herhalingen.

In elke sessie wordt eerst de neutrale positie gevonden en wordt erop gelet deze neutrale positie gedurende de hele oefening te behouden.
Daarnaast worden core stability-oefeningen gegeven, waaronder gelijktijdige contracties van de multifidus en bekkenbodemspieren, in verschillende houdingen zoals rugligging, buikligging, kruiphouding, brug, knielend, zittend en staand.
De oefeningen bestaan uit 3 niveaus (van makkelijk tot moeilijk).
Bij de gevorderde oefeningen wordt van patiënten gevraagd de neutrale kromming van de lumbale wervelkolom te behouden, worden weerstandsoefeningen voor de ledematen toegevoegd en worden de oefeningen geleidelijk afgerond.
Elke oefening wordt uitgevoerd in 3 sets van 8-10 herhalingen.
De oefening zal bestaan uit bewegingen in alle richtingen van thoracale wervelkolomflexie, extensie, laterale flexie en rotatie.
In de thoracale wervelkolomextensieoefening zullen beide handen achter de polsen worden vastgezet en zal de rug op een schuimroller worden geplaatst met de voeten plat op de vloer op 25.
De knie zal op 90° worden gehouden om de extensieoefening op de schuimroller uit te voeren.
De handen zullen met de elleboog op een stoel worden vastgezet.
Na het knielen zal de heup naar de hielen worden bewogen om de thoracale wervelkolom te strekken.
In de thoracale wervelkolomflexieoefening zal de thoracale wervelkolom worden gebogen door achteruit te bewegen totdat de heup de hielen raakte in een viervoetige positie.De rotatieoefening zal worden uitgevoerd met patiënten op hun zij liggend.De ellebogen zullen recht zijn en de handpalmen zullen tegen elkaar worden gehouden.
Het been dat naar het plafond wijst, zal worden gebogen tot het niveau van de maag.
Vervolgens zal de arm die naar het plafond wijst, achterwaarts in een grote boog worden bewogen om de thoracale wervelkolom te roteren.
De patiënten zullen 8 verschillende heupmobiliteitsoefeningen uitvoeren; reverse v-ups, vlinder, kikker, interne rotatie met voetverhoging, pike, gedraaide halvemaan uitval, duif, uitval op de knie.
Elke oefening wordt uitgevoerd voor 2 sets van 20 seconden.

Experimenteel: Thoracale mobiliteitsoefeningen (alleen 1e experimentele groep)
De oefening zal bestaan uit bewegingen in alle richtingen van thoracale wervelkolomflexie, extensie, laterale flexie en rotatie.
In de thoracale wervelkolomextensie-oefening zullen beide handen achter de polsen worden vergrendeld en wordt de rug op een schuimroller geplaatst met de voeten plat op de vloer op 25.
De knie wordt op 90° gehouden om de extensie-oefening op de schuimroller uit te voeren.
De handen worden vergrendeld met de elleboog op een stoel.
Na het knielen wordt de heup richting de hielen bewogen om de thoracale wervelkolom te strekken.
In de thoracale wervelkolomflexie-oefening wordt de thoracale wervelkolom gebogen door achterwaarts te bewegen totdat de heup de hielen raakt in een viervoetige positie.De rotatie-oefening wordt uitgevoerd met patiënten die op hun zij liggen.De ellebogen zijn gestrekt en de handpalmen worden tegen elkaar gehouden.
Het been dat naar het plafond wijst, wordt gebogen tot op het niveau van de maag.

Experimenteel: Heupmobiliteitsoefeningen (alleen 2e experimentele groep)
De patiënten zullen 8 verschillende heupmobiliteitsoefeningen uitvoeren; reverse v-ups, vlinder, kikker, interne rotatie met voetheffing, pike, gedraaide halvemaan uitval, duif, uitval op de knie.
Elke oefening wordt uitgevoerd voor 2 sets van 20 seconden.

Met behulp van de Arabische MODI (Bijlage III) zullen patiënten worden geïnstrueerd om het beste antwoord te kiezen uit de mogelijke antwoorden dat het niveau van functioneren en beperkingen tijdens dagelijkse activiteiten beschrijft voor elk item van de vragenlijst (Alnahdi, 2025).

Lumbale bewegingsuitslag

Het Back Range of Motion (BROM)-apparaat zal worden gebruikt om de lumbale bewegingsuitslag (ROM) te meten, als een valide en betrouwbare maat voor spinale beweging (Kumar & Singh, 2021; Chen et al., 2022), volgens gevestigde procedures (Madson et al., 1999). Na palpatie en markering van de doornuitsteeksels van T12 en S1 met kleefstofstippen, zullen patiënten een warming-up uitvoeren voor elk bewegingsvlak. Het BROM-apparaat zal worden gebruikt om actieve ROM te meten in de volgorde van flexie/extensie, laterale flexie rechts/links en rotatie rechts/links. Zodra de gegevens voor elke beweging waren genoteerd, werd het BROM-apparaat verwijderd en werden de huidmarkeringen na een rustperiode van 10-15 seconden opnieuw aangebracht. Elke beweging zal drie keer worden uitgevoerd, en de gegevens van de drie pogingen zullen worden gemiddeld en gebruikt voor analyse om de betrouwbaarheid te maximaliseren. |

Spinale mobiliteit:

Bij de aangepaste Schober-test staat de patiënt.
De onderzoeker markeert beide achterste superieure iliacale stekels (PSIS) en trekt vervolgens een horizontale lijn in het midden van beide markeringen.
Een tweede lijn wordt gemarkeerd 5 cm onder de eerste lijn.
Een derde lijn wordt gemarkeerd 10 cm boven de eerste lijn.
De patiënt krijgt de instructie om naar voren te buigen alsof hij/zij zijn/haar tenen probeert aan te raken.
De onderzoeker meet opnieuw de afstand tussen de bovenste en onderste lijn (Rezvani et al., 2012).
Er worden drie pogingen uitgevoerd en het gemiddelde van de drie pogingen wordt gekozen voor de gegevensanalyse.

Angst voor beweging

Met behulp van de Arabische Tampa (Bijlage V) zullen patiënten worden gevraagd naar eventuele buitensporige, irrationele en invaliderende angst voor fysieke beweging en activiteit als gevolg van een gevoel van kwetsbaarheid voor pijnlijk letsel of hernieuwd letsel (Al Shudifat et al., 2020).
