Effectiviteit van spectraal geoptimaliseerd licht op cognitieve stoornissen bij een ernstige depressieve stoornis en de neurobeeldvormingsmechanismenstudie

Deze studie heeft tot doel de therapeutische werkzaamheid en veiligheid van spectraal geoptimaliseerd licht (SOL) bij het verbeteren van cognitieve stoornissen (CI) bij een ernstige depressieve stoornis (MDD) te valideren, de functionele en structurele kenmerken van de hippocampus (HPC)-dorsolaterale prefrontale cortex (dlPFC) neurale circuit in MDD-patiënten met cognitieve stoornissen te karakteriseren en het bemiddelende effect van het HPC-dlPFC neurale circuit op de cognitieve verbeteringen veroorzaakt door SOL-behandeling bij MDD-CI-patiënten te onderzoeken. Patiënten met MDD-CI mogen alleen selectieve serotonineheropnameremmers (SSRI's) als primaire medicatie gedurende ten minste één week hebben ontvangen of geen enkele behandeling hebben ondergaan. Gedetailleerde beschrijving

Deze studie was ontworpen als een gerandomiseerde gecontroleerde studie. De focus ligt op patiënten met MDD-CI, met een experimentele groep en een controlegroep opgezet. De experimentele groep werd behandeld met één van zes SSRIs, waaronder Fluoxetine, Paroxetine, Sertraline, Citalopram, Fluvoxamine en Escitalopram. De medicatie bleef ongewijzigd gedurende de gehele interventie. De experimentele groep kreeg de interventie van BLT, en de controlegroep kreeg daarnaast de interventie van gedimd rood licht (placebo). De inclusie- en exclusiecriteria voor MDD-patiënten. 1、Inclusiecriteria:

  1. huidige diagnose van een ernstige depressieve stoornis, eerste episode of recidief, volgens de Diagnostic and Statistical Manual of Mental Disorders, vijfde editie (DSM-5);
  2. leeftijd 18-60 jaar; geslacht niet beperkt;
  3. de ernst van de MDD-symptomen moet >14 scores zijn op de Hamilton Depression Rating Scale-17 (HAMD-17);

Eligible MDD participants will be randomly subjected to the DRL control group. The light source intensity will be set at <100 lux and placed 0.6 meters away from the MDD participant. They will be required to staring at the light source for 2 seconds every 5 minutes. The DRL control group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for four weeks, 6 days per week.
Using a dim red light box as the placebo, with an intensity of <100lux and a main wavelength of 690.4nm
Experimenteel: BLT experimental group
Eligible MDD participants will be randomly subjected to the BLT experimental group. The source intensity of BLT experimental group will be set at 10000 lux and placed 0.45 meters away from the MDD participant. They will be required to staring at the light source for 2 seconds every 5 minutes. The BLT experimental group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for 4 weeks, 6 days per week.
