Hjernebestråling ved behandling av pasienter med småcellet lungekreft i begrenset stadium

En fase II randomisert studie med to doseplaner for å gi profylaktisk kranial bestråling for pasienter med begrenset sykdom småcellet lungekreft
BAKGRUNN: Strålebehandling bruker høyenergirøntgenstråler for å skade tumorceller. Det er foreløpig ikke kjent hvilket strålebehandlingsregime som er mer effektivt for å forebygge hjernemetastaser hos pasienter med småcellet lungekreft i begrenset stadium samtidig som det tas hensyn til kronisk nevrotoksisitet fra strålebehandling.
FORMÅL: Denne randomiserte fase II-studien sammenligner forekomsten av kronisk nevrotoksisitet mellom tre forskjellige hjernebestrålingsregimer. Den tilsvarende fase III-komponenten som tar for seg forebygging av hjernemetastaser ble kjørt av EORTC og rapportert separat (NCT00005062).

MÅL:
- Sammenlign forekomsten av kronisk nevrotoksisitet hos pasienter behandlet med disse regimene.
- Sammenlign livskvaliteten til pasienter behandlet med disse regimene. (fase III stengt for periodisering per 31.12.05)
OVERSIKT: Dette er en randomisert, multisenterstudie. Pasientene er stratifisert etter alder (60 og under vs over 60) og tid siden induksjonsterapi (90 dager eller mindre vs 91-180 dager vs 181-240 dager). Pasientene er randomisert til 1 av 3 behandlingsarmer.
- Arm I: Pasienter gjennomgår profylaktisk kranial bestråling (PCI) en gang daglig 5 dager i uken. Behandlingen fortsetter i 2 uker i fravær av uakseptabel toksisitet.
- Arm II: Pasienter gjennomgår PCI én gang daglig 5 dager i uken. Behandlingen fortsetter i 2,6 uker i fravær av uakseptabel toksisitet.
- Arm III: Pasienter gjennomgår PCI to ganger daglig 5 dager i uken. Behandlingen fortsetter i 3,4 uker i fravær av uakseptabel toksisitet.
Livskvalitet vurderes ved baseline, hver 6. måned i 1 år, og deretter årlig i 3 år.
Pasientene følges hver 6. måned i 1 år og deretter årlig i 3 år.
PROSJERT PASSERING: Totalt 264 pasienter vil bli påløpt for denne studien innen 3,5 år.

SYKDOMENS KARAKTERISTIKA:
Histologisk eller cytologisk bekreftet småcellet lungekreft ved finnålsaspirasjon, biopsi eller 2 positive sputa
Begrenset stadium (I-IIIB)
- Begrenset til 1 hemithorax
- Ingen T4 basert på ondartet pleural effusjon, eller N3 sykdom basert på kontralateral hilar eller kontralateral supraklavikulær involvering
- Fullstendig respons etter induksjonskjemoterapi (med eller uten thoraxstrålebehandling)
- Konsolidativ bryststrålebehandling kan startes før studien
Ingen radiografisk bevis for noen av følgende:
Hjernemetastaser
- Normal hjerne-CT-skanning eller MR mindre enn 1 måned før studien
- Ipsilaterale lungemetastaser
Ondartet pleural effusjon
- Minimal pleural effusjon ved CT-skanning av thorax tillatt, men ikke ved røntgen av thorax

Forekomst av kronisk nevrotoksisitet

Kronisk nevrotoksisitet er definert som et fall i én standard målefeil (SEM) i en av de tre testene som omfatter det nevropsykologiske testbatteriet (Hopkins Verbal Learning Test, Controlled Word Association Test og Trail-Making Test) uten utvikling av hjernemetastase med ett år.
SEM beregnes som standardavviket til grunnlinjetesten multiplisert med kvadratroten av én minus den publiserte pålitelighetskoeffisienten for testen.
