Opis
Kryteria przyjęcia:
- Duże zaburzenie depresyjne (MDD) rozpoznane na podstawie wywiadu klinicznego w oparciu o Affective Disorders and Schizophrenia Agenda dla dzieci w wieku szkolnym – wersja aktualna i dożywotnia K-SADS-PL dla jego akronimu w języku angielskim, z pojedynczym lub nawracającym epizodem trwającym ≥4 tygodnie.
- Obecny epizod depresyjny musi trwać krócej niż 5 lat i być co najmniej dłuższy niż 2 miesiące, w których pacjent spełnia operacyjną definicję MDD z DSM-V.
- Skala CID z wartościami równymi 17 lub wyższymi w ocenie klinicznej.
- Zakaz stosowania innego leku w dniach interwencji.
- Nie powinni przyjmować leków przeciwdepresyjnych na początku badania (ostatnie 4 tygodnie) lub musi upłynąć odpowiedni czas „wypłukania”/klirensu leków przeciwdepresyjnych ustalony jako 5 lub więcej biologicznych okresów półtrwania lub 5 dni przed randomizacją.
- Aby móc i chcieć wyrazić świadomą zgodę, zarówno rodzica lub opiekuna, jak i uczestnika w wieku od 14 do 17 lat. W grupie wiekowej 10-13 lat zgoda pacjenta i zgoda rodzica lub opiekuna.
- Stosowanie benzodiazepin (np. lorazepamu) równoważne 20 mg diazepamu i które pozostaje stabilne podczas badania.
- Pacjenci z niezmienionym leczeniem farmakologicznym MDD w ciągu ostatnich 4 tygodni i którzy zgodzą się na zmianę fluoksetyny w momencie randomizacji, spełniający wcześniej opisane kryteria przeciwdepresyjne.
Kryteria wyłączenia:
- Poprzednia operacja czaszki.
- Niedawny uraz czaszki (6 miesięcy).
- Stosowanie środków pobudzających ośrodkowy układ nerwowy.
- Infekcje układu oddechowego.
- NIE podpisuje lub nie może podpisać świadomej zgody lub opiekunowie prawni/rodzice nie wyrażają zgody.
- Stan kliniczny lub anomalia, które zdaniem badacza zagrażają bezpieczeństwu pacjenta lub jakości danych.
- Niekontrolowana współistniejąca choroba, postępujące zaburzenie mózgowe, ciężkie choroby ogólnoustrojowe, choroba objawowa, choroba serca, przewlekłe choroby skóry lub uszkodzona skóra głowy, które mogą zakłócać stymulację tDCS.
- Wszelkie metalowe implanty czaszki (z wyjątkiem wypełnień dentystycznych lub płytek tytanowych) lub urządzenia medyczne (rozrusznik serca, głęboki stymulator mózgu, pompa infuzyjna, implant ślimakowy, stymulator nerwu błędnego).
- Bezpośredni krewni badaczy lub personel bezpośrednio zaangażowany w badanie.
- Ryzyko ostrego samobójstwa mierzone za pomocą skali oceny depresji Montgomery Asberg (MADRS) > 4 punktów lub pozytywność 4. lub 5. pozycji w skali oceny ciężkości samobójstw Columbia (C-SSRS).
- Depresja oporna na leczenie >4 dobrze wskazanymi lekami w trakcie obecnego epizodu.
- Historia choroby afektywnej dwubiegunowej lub obecne objawy manii.
- Leczenie elektrowstrząsami jednocześnie iw ciągu ostatnich 12 miesięcy.
- Inne istotne zaburzenia psychiczne zgodnie z DSM 5 (z wyjątkiem zespołu lęku uogólnionego, fobii specyficznej lub zespołu lęku separacyjnego).
- Wcześniejsze leczenie tDCS z powodu innej choroby niż duże zaburzenie depresyjne.
- Wcześniej udokumentowane zaburzenie rozwoju intelektualnego lub ocena kliniczna spełniająca kryteria CIE-F79.
- Ze względu na to, że leczenie tDCS ma przeciwwskazania, wykluczone zostaną: osoby z napadami niesprowokowanymi w wywiadzie, zaburzeniami drgawkowymi, drgawkami gorączkowymi w wywiadzie, wywiadem rodzinnym w kierunku padaczki lub guza mózgu, ciążą.
- Przeciwwskazanie lub historia działań niepożądanych fluoksetyny od łagodnych do ciężkich.
Kryteria eliminacji:
- Pacjenci, którzy nie ukończyli testów oceny neuropsychologicznej.
- Przerwanie interwencji o mniej niż 50% tego, co było zaplanowane.
- Początek aktywnych myśli samobójczych.
- Aktywna psychoza.
- Rozpoczęcie leczenia pojawiającej się manii.
- Ciąża.

Eksperymentalny: ramię tDCS
Uczestnicy otrzymują 10 kolejnych sesji, po których następuje 1 sesja na tydzień 10 tygodni tDCS (30 minut i 2 mA) + fluoksetyna (10 mg dziennie przez 2 tygodnie, a następnie 20 mg dziennie).

Stymulacja anodowa zostanie zastosowana na F3 zgodnie z zamierzonym układem 10/20, a katoda zostanie zastosowana w pozycji kontralateralnej (F4).

Jest to narzędzie do ilościowego określania objawów manii, które składa się z 11 pozycji, z których każda ma 5 opcji odpowiedzi, które odzwierciedlają wyższy wynik przy dużym nasileniu. Stosowany jest przez klinicystę i opiera się na subiektywnej relacji pacjenta w ciągu 48 godzin przed oceną oraz na obserwacji zachowania pacjenta podczas wywiadu. Pacjent wybiera poziom intensywności objawu dla każdej pozycji z opcją od 0 do 4.
