Tiamina (witamina B1) jest rozpuszczalną w wodzie witaminą, która odgrywa kluczową rolę w tlenowym metabolizmie glukozy. Tiamina jest kofaktorem dehydrogenazy pirogronianowej (PDH), enzymu, który musi zostać aktywowany, aby wejść w cykl Krebsa dla metabolizmu tlenowego. Aktywność PDH jest zmniejszona w stanach niedoboru tiaminy, co powoduje przesunięcie metabolizmu pirogronianu do szlaku beztlenowego. Prowadzi to do zwiększonej produkcji mleczanu i kwasicy. Utrata tiaminy z moczem, z wynikającym z tego niedoborem tiaminy, nie jest rzadkością w cukrzycy. Wstępne badania badaczy wykazały, że niedobór tiaminy występuje aż u 39% pacjentów z CKK oraz że poziom tiaminy jest odwrotnie proporcjonalny do mleczanu i kwasicy. Badacz stawia hipotezę, że leczenie pacjentów z CKK dożylną tiaminą doprowadzi do szybszego ustąpienia kwasicy i poprawy metabolizmu tlenowego. Drugorzędną hipotezą badacza jest to, że leczenie tiaminą skróci pobyt na OIOM-ie i w szpitalu oraz doprowadzi do mniejszego wykorzystania zasobów szpitalnych.
W tym randomizowanym, podwójnie zaślepionym, kontrolowanym placebo badaniu pacjenci przyjęci do szpitala z CKK, którzy zostali włączeni do badania, zostaną losowo przydzieleni do grupy otrzymującej dożylnie tiaminę (200 mg w 0,9% roztworze soli fizjologicznej) dwa razy dziennie przez dwa dni lub identyczną objętość 0,9% soli fizjologicznej w tym samym schemacie. Głównym wynikiem badacza jest zmiana stężenia wodorowęglanów w ciągu 24 godzin po włączeniu, z pomiarami po 0, 6, 12, 18, 24 godzinach, przy użyciu liniowego modelu efektów mieszanych. Po drugie, pacjenci zostaną podzieleni na stratyfikację według typu I i typu II DM. Dodatkowo zostanie przeprowadzona wcześniej zaplanowana analiza podrzędna osób z niedoborem tiaminy.

Kryteria przyjęcia:
- Wodorowęglan ≤15 mEq/L
- Luka anionowa > 12 mEq/l
- pH krwi ≤ 7,24 (jeśli zostało już uzyskane przez zespół kliniczny)
- Ketony w moczu (jakościowo) lub ketony w surowicy (kwas β-hydroksymasłowy) > 3 mmol/L
- Zapisy w ciągu 6 godzin od prezentacji
Kryteria wyłączenia:
- Obecna suplementacja tiaminy ≥ 6 miligramów dziennie (tj. więcej niż multiwitamina)
- Konkurencyjne przyczyny ciężkiej kwasicy, w tym drgawki, zatrucie tlenkiem węgla, toksyczność cyjanków, zatrzymanie akcji serca, dysfunkcja wątroby (szczególnie zdefiniowana jako znana marskość wątroby)
- Znana alergia na tiaminę
- Konkurencyjne wskazanie do podania tiaminy w ocenie zespołu klinicznego (np. alkoholik)
- Populacje chronione badaniami (kobiety w ciąży, więźniowie, osoby niepełnosprawne intelektualnie)

SOFA Score (Sequential Organ Failure Assessement Score)

The SOFA score is a validated measure of organ dysfunction commonly used in critically ill patients, particularly those with sepsis.
It evaluates the function of six organ systems-respiratory, cardiovascular, neurologic, hepatic, renal, and coagulation-based on routinely collected clinical and laboratory data.
Each organ system is assigned a score from 0 (normal function) to 4 (most severe dysfunction), resulting in a total score ranging from 0 to 24.
Higher SOFA scores indicate greater severity of organ failure and a higher risk of adverse outcomes.
SOFA score in this study uses a modification in which the arterial oxygen saturation/fraction of inspired oxygen (SaO2 /FiO2) ratio is substituted for the partial pressure of arterial oxygen/fraction of inspired oxygen (PaO2 /FiO2 ratio).

ATP-linked respiration represents oxygen consumption coupled to ATP production, while proton leak represents oxygen consumption not linked to ATP synthesis.
Maximal respiration represents the maximal capacity of the electron transport chain after uncoupling.
Spare respiratory capacity represents the difference between maximal and basal respiration and indicates the ability to respond to increased energy demand.
Non-mitochondrial respiration represents oxygen consumption independent of mitochondrial activity.

Pyruvate Dehydrogenase Activity

Secondary outcome of change in PDH specific activity over 72 hours or at discharge if patient's hospital length of stay was less than 72 hours.
PDH activity and total PDH protein quantity were measured in isolated peripheral blood mononuclear cells (PBMCs) following selective disruption of the mitochondrial membrane, using a previously validated immunocapture and microplate-based enzymatic assay protocol.
PDH-specific activity was calculated as the ratio of measured PDH enzymatic activity to the natural logarithm of PDH protein quantity (PDH activity / ln[PDH quantity]), providing a normalized metric that reflects the functional efficiency of the enzyme independent of its expression level.
Higher values suggest higher enzymatic efficiency.
