O tratamento de pacientes com reestenose de stents farmacológicos permanece um desafio.
Este estudo avaliará qual estratégia intervencionista (balão com eluição de paclitaxel versus stent com eluição de everolimus) é superior no tratamento de pacientes com reestenose de stent farmacológico.

O tratamento de pacientes com reestenose de stents farmacológicos continua sendo um grande desafio clínico e técnico. Atualmente, os stents farmacológicos são amplamente utilizados no manejo desses pacientes. Recentemente, foi sugerido o valor do balão com eluição de paclitaxel neste cenário clínico. No entanto, existem poucos dados observacionais sobre o valor desse novo balão em pacientes com reestenose de stent farmacológico.
Este ensaio clínico prospectivo, multicêntrico e randomizado irá comparar duas estratégias de intervenção diferentes (ou seja, balão eluidor de paclitaxel versus stent eluidor de everolimus) no tratamento de pacientes com reestenose de stent eluidor de drogas.

Critério de inclusão:
PACIENTE:
- Idade entre 20 e 85 anos
- Consentimento informado assinado
- Aceitação do controle angiográfico tardio (6-9 meses)
- Angina e/ou evidência objetiva de isquemia
LESÃO:
- Reestenose intra-stent (> 50% visual) qualquer stent farmacológico
- Conhecimento da localização anterior do stent
Critério de exclusão:
PACIENTE:
- Inclusão em outro protocolo de pesquisa clínica
- Mulheres em idade reprodutiva
- Doença sistêmica grave associada (incluindo insuficiência renal ou hepática)
- Depressão grave da fração de ejeção do ventrículo esquerdo (FEVE <25%)
- Doença que afeta a expectativa de vida
- Infarto do miocárdio recente (<7 dias)
- Tempo de implantação do SF anterior <1 mês
- Dificuldades severas esperadas para estudo angiográfico tardio
LESÃO:
- Falha angiográfica durante a implantação do stent inicial (estenose residual > 50%)
- Imagem de grande trombo no stent (> diâmetro do vaso)
- Tortuosidade ou Ca++ com dificuldades muito graves durante a implantação prévia do stent
- Diâmetro do vaso <2 mm (estimativa visual)
- Reestenose apenas "fora" do stent (A borda do stent não é afetada)
- Reestenose completamente oclusiva (100%, TIMI 0)
- Reestenose muito difusa (> 30 mm de comprimento)
