SAbR para Carcinoma de Células Renais Oligometastático
Ensaio Fase II de SAbR para Pacientes com Carcinoma de Células Renais Oligometastático
Hipótese:
A radiação corporal ablativa estereotáxica (SAbR) prolonga a sobrevida livre de progressão para pacientes com câncer renal oligometastático (RCC) e atrasa o início da terapia sistêmica.
Objetivos primários:
• Avaliar o atraso no tempo para o início da terapia sistêmica (TTST) como substituto da sobrevida livre de progressão (PFS), definida como o tempo desde o primeiro dia de SAbR até o início da terapia sistêmica.
Objetivo Secundário:
- Avaliar a sobrevida livre de progressão modificada (mPFS) para pacientes com carcinoma de células renais oligometastáticos tratados com SAbR.
- Para avaliar a sobrevida global (OS)
- Para avaliar a sobrevida específica do câncer (CSS)
- Avaliar a taxa de controle local de lesões irradiadas.
- Medir a qualidade de vida relacionada à saúde (QVRS).

O estudo é um estudo prospectivo de fase II de instituição única, de braço único, aberto, avaliando SAbR em pacientes com CCR oligometastático recém-diagnosticado.
Declarações do problema:
- A terapia local (SAbR) pode retardar com segurança o início da terapia sistêmica?
- Atrasar com segurança o início da terapia sistêmica pode trazer benefícios significativos para a qualidade de vida dos pacientes, uma vez que a terapia sistêmica tem efeitos colaterais significativos.
- O SAbR pode ser curativo em pacientes com CCR verdadeiramente oligometastáticos?
Ponto final primário:
• Tempo para início da terapia sistêmica (TTST) definido como o tempo desde o primeiro dia de SAbR até o início da terapia sistêmica.
Ponto final secundário:
- A sobrevida livre de progressão modificada (mPFS) é definida como o intervalo de sobrevivência sem desenvolvimento de > 3 locais de novas metástases, novos locais de metástases que não são passíveis de tratamento com SAbR, um total de > 6 locais de metástase que exigiram SAbR, falha local no local tratado com SAbR ou desenvolvimento de metástase cerebral.

Descrição
Critério de inclusão:
- Carcinoma de células renais metastático com metástases extracranianas mensuráveis limitadas (metástases limitadas ou oligometástases, definidas como ≤3 locais de metástase).
- Evidência radiográfica de doença metastática. A TC deve ser realizada até 30 dias após o registro.
- Confirmação Patológica de Carcinoma de Células Renais.
- Cirurgia prévia ou radiação são permitidas.
- Idade ≥ 18 anos.
- As mulheres com potencial para engravidar e os homens devem concordar em usar contracepção adequada (método hormonal ou de barreira de controle de natalidade; abstinência) antes da entrada no estudo e por 90 dias após o término do tratamento com radiação. Caso uma mulher engravide ou suspeite estar grávida durante a participação neste estudo, ela deve informar seu médico assistente imediatamente.
Uma mulher com potencial para engravidar é qualquer mulher (independentemente da orientação sexual, de ter feito uma laqueadura ou de permanecer celibatária por opção) que atenda aos seguintes critérios:
- Não foi submetida a histerectomia ou ooforectomia bilateral; ou
Não esteve naturalmente na pós-menopausa por pelo menos 12 meses consecutivos (ou seja, teve menstruação em qualquer momento nos 12 meses consecutivos anteriores).
- Capacidade de entender e vontade de assinar um consentimento informado por escrito e concorda em se submeter a estudos de imagem e acompanhamento
Critério de exclusão:
- Indivíduos com metástase cerebral avaliados por ressonância magnética ou tomografia computadorizada com contraste (contraste recomendado).
- Indivíduos com história prévia de metástase cerebral.
- Indivíduos que receberam terapia sistêmica anterior para câncer renal no último 1 ano, exceto uma linha de imunoterapia ou terapia com citocinas (por exemplo, anterior IL-2); a terapia sistêmica para outros tipos de câncer não se aplica a este critério de exclusão
- Indivíduos com ≥3 fatores prognósticos desfavoráveis definidos por Motzer et al. (1999), (KPS 1, Hgb < LLN, LDH >1,5x normal, cálcio sérico corrigido >10mg/dl e ausência de nefrectomia prévia), pacientes com 0, 1-2 e ≥3 fatores tiveram tempo até a morte de 20 meses, 10 meses e 4 meses.
- Indivíduos com expectativa de vida < 6 meses.
- Indivíduos recebendo quaisquer outros agentes de investigação
- As participantes não devem estar grávidas devido ao potencial de anormalidades congênitas e ao potencial deste regime de prejudicar lactentes.

Experimental: Radiação corporal ablativa estereotáxica (SABR)
A radiação corporal ablativa estereotáxica (SAbR) para todos os locais de metástases mensuráveis (≤3) será tratada por SAbR.
Novos locais de metástase serão avaliados para tratamento continuado, se considerados apropriados por oncologistas médicos e radioterapeutas com SAbR.

The percentage of participants with more than 1 year time to systemic therapy (TTST) will be reported for this outcome.
Initiation of systemic therapy >1 year (delay in time) is a surrogate of progression free survival (PFS) and is defined as the time from the first day of SAbR to start of systemic therapy.

Modified Progression-free Survival (mPFS) for Patients Who Are Treated With SAbR
Prazo: 5.7 years
|
Modified progression-free survival (mPFS) for patients with oligometastatic renal cell carcinoma who are treated with SAbR.
Modified progression-free survival (mPFS) is defined as the survival interval without development of >3 sites of new metastasis, new sites of metastases that are not amenable to SAbR treatment, a total of >6 sites of metastasis that required SAbR, local failure at SAbR-treated site, or development of brain metastasis, with the definition of new metastasis and local failure (progression) as defined by RECIST.
"Metastasis" in this entire protocol refers to extra-cranial metastasis unless otherwise specified.

Progression-free Survival on Systemic Therapy (PFS-ST)

To evaluate progression on next line of systemic therapy as defined by RECIST 1.1 starting from mPFS (modified progression free survival).
Response and progression will be evaluated in this study using the new international criteria proposed by the Response Evaluation Criteria in Solid Tumors (RECIST v 1.1) Committee [Eur J Cancer.
2009;45(2):228-247].
Changes in only the largest diameter (unidimensional measurement) of the tumor lesions are used in the RECIST v1.1 criteria.

Severity or Toxicity will be assessed according to the National Cancer Institute (NCI) Common Toxicity Criteria for Adverse Events (CTCAE), version 5.0.
Events have been counted if they are considered related to the investigational intervention included "possible," "probable," and "definite."
To ensure transparency, all AEs with "unlikely" or greater attribution are included.

FACT-G is a measure that sums the functional wellbeing (FWB), physical wellbeing (PWB), the social/family well-being (S/FWB), and emotional wellbeing (EMB).FACT-G total score range (0-108); Higher values indicate a better HRQOL.

The EQ-VAS is a global measure of patient perception of their health ranging from 0-100, with 0 being the worst health they can imagine, and 100 being the best health they can imagine.

HRQOL will be measured using FKSI questionnaire at baseline and at certain specific timepoints (3,9,15,21,27,33 months) until the completion of follow ups.
FKSI adds to the FACT-G (27 items) by including 15 items specific to kidney cancer patients.
FKSI is a questionnaire for FACT-Kidney Symptom Index used to assess QoL/participant-reported outcomes for participants diagnosed with renal cell cancer.
The FKSI contained 15 questions each ranging from 0 (not at all) to 4 (very much) so that FKSI ranged between 0-60 where higher scores reflects better functioning and fewer symptoms.

Health care utilization data needed to assess costs will be obtained from the cost & convenience questionnaire, which includes costs of hospitalization, treatment, ER visits, physician and clinic visits and medications.
Markov modeling with probabilistic sensitivity analysis will be used to correlate quality-adjusted survival and cost.
Cost-effective analysis (cost & convenience questionnaire) will be given prior to treatment and at the time of progression.
