En ensidig pilotstudie av genomförbarheten och effektiviteten av elektroakupunktur hos patienter med kemoterapi-inducerad perifer neuropati
Denna studie kommer att fastställa genomförbarheten och effektiviteten av ett 10-behandlingsprogram med elektroakupunktur (EA) hos patienter med perifer neuropati orsakad av cellgifter (CIPN).
Forskarna antar att EA kommer att vara en genomförbar och effektiv behandling för CIPN.

Detaljerad beskrivning
Detta är en enarms-pilotstudie som utvärderar genomförbarheten och effektiviteten av EA hos 20 patienter med CIPN.
Det primära målet är att fastställa genomförbarheten av ett 10-behandlings EA-program hos patienter med CIPN.
Genomförbarhet kommer att definieras som ≥15 patienter som fullföljer ≥8 EA-behandlingar.
Sekundära mål inkluderar förändring i neuropatisk smärta och livskvalitet före och efter EA-behandling.
En explorativ korrelativ analys av mekanistiska biomarkörer kommer också att utföras.

Inklusionskriterier:
- Man eller kvinna i åldern ≥18 år
- Har fått kurativt syftande kemoterapi (dvs. paclitaxel, docetaxel, nab-paclitaxel, carboplatin, oxaliplatin, vinorelbine, ixabepilone eller vincristine) ≥3 månader före start av EA-behandling
- Bestående perifer neuropati av grad ≥2 i fingrar eller tår enligt National Cancer Institute Common Terminology Criteria for Adverse Events (NCI CTCAE) v3.0
- Eastern Cooperative Oncology Group prestandastatus på ≤2 (se avsnitt 13.3)
- Villig och kapabel att ge skriftligt informerat samtycke för studien.
Exklusionskriterier:
- Dokumenterad medicinsk historia av neuropati orsakad av nervkompression (t.ex. karpaltunnelsyndrom, radikulopati eller spinal stenos)
- Svår koagulopati eller blödningsrubbning, enligt behandlande läkares bedömning
- Förekomst av cellulit eller annan hudinfektion eller tillstånd som skulle förhindra placering av akupunkturnålar i händerna eller fötterna
- Diabetes om inte Hgb A1c <7,5%
- Instabil hjärtsjukdom
- Pacemaker
- Metallplattor
- Känd psykiatrisk störning som skulle störa samarbetet med studiens krav

Experimentell: Elektroakupunktur (EA)
EA är en icke-farmakologisk behandling som kombinerar traditionell akupunktur med elektrisk stimulering.
EA kommer att administreras totalt 10 gånger under en 7-veckorsperiod.
EA kommer att utföras två gånger i veckan under vecka 1-3 och sedan en gång i veckan under vecka 4-7.
Varje behandling tar ungefär 30 minuter.

Change in Neuropathic Pain After a 10-treatment EA Program

The change in neuropathic pain after a 10-treatment EA program in subjects with CIPN, was assessed by the Brief Pain Inventory Short Form (BPI-SF) at Baseline, 2-Week Post EA, and 3 Month Post EA.
Used to evaluate the severity of a subject's pain and the impact of this pain on the subject's daily functioning.
Pain and interference are rated on a 10-point scale (0 = no pain/interference and 10 = pain as bad as you can imagine/complete interference).
Higher scores mean worse outcomes.

Change in Quality of Life After a 10-treatment EA Program

Assessed by the Total Outcome Index (TOI), including the Functional Assessment of Cancer Therapy/Gynecologic Oncology Group-Neurotoxicity (FACT/GOG-NTX) subscale at Baseline, 2-Week Post EA, and 3 Month Post EA.
The Total Outcome Index (TOI) ranges from 0 to 100, with higher scores consistently indicating better outcomes.
Items are rated on a 0-4 Likert scale and negatively worded items are reverse scored so that higher values always reflect better health status.
The neurotoxicity subscale score is calculated by summing the scored responses to the neurotoxicity items (with standard prorating if some items are missing), and the TOI is calculated by summing the Physical Well-Being, Functional Well-Being, and Neurotoxicity subscale scores.
Lower scores on either measure indicate worse symptoms or functioning, while higher scores reflect improvement or better quality of life related to neurotoxicity.
