The study intervention involves administering nebulized tranexamic acid (TXA) to pediatric patients with traumatic hemorrhage (PTH). The intervention consists of three consecutive doses of nebulized TXA.
The dosage of nebulized TXA is adjusted based on the child's weight. For children weighing more than 25 kg, each dose is 500 mg. For children weighing less than 25 kg, each dose is 250 mg.
Frequency: The three doses of nebulized TXA are administered consecutively over the course of approximately an hour. Administration Method: Nebulized TXA is delivered through a nebulizer device. A nebulizer converts the liquid medication into a fine mist or aerosol, which is then inhaled by the patient. This method allows the medication to be delivered directly to the respiratory tract, where it can exert its effect on the bleeding site. Delivery Setting: The intervention may take place in a clinical setting, such as a hospital or outpatient clinic, where nebulizer devices and medical supervision are readily available. Each patient receives three nebulized independent doses of TXA in succession. The delivery of the intervention is carried out by healthcare professionals trained in administering nebulized medications.

Inclusion Criteria:
- Received a tonsillectomy
- Presents to the ED with secondary* post-tonsillectomy hemorrhage
- Children between age of 2 to 17 years of age (i.e., before their 18th birthday) *Secondary post-tonsillectomy hemorrhage is defined as greater than 24 hours from their primary tonsillectomy operation (arrival in recovery/PACU).
Exclusion Criteria:
- Known and documented bleeding or clotting disorder.
- Known pregnancy.
- Patients with known hypersensitivity or allergic response to tranexamic acid.
- Parents or guardians who cannot communicate in English or Spanish.
- Intubation prior to enrollment.
- Previously enrolled patients.

Experimentell: Nebuliserad tranexamsyra

Deltagarna kommer att få tre doser av TXA 500 mg (5 ml TXA 100 mg/ml) nebuliserade med en PARI LC D disponibel nebulisator eller motsvarande under 10-15 minuter med ett gasflöde på 8 eller mer liter/minut.

Placebo-jämförare: Nebuliserad saltlösning
Försökspersonerna kommer att få nebuliserad saltlösning efter tonsillektomi om de återvänder till akuten med blödning efter operationen

Deltagarna kommer att få tre 5 ml doser placebo (normal koksaltlösning) nebuliserade med en PARI LC D disponibel nebulisator eller motsvarande under 10-15 minuter med 8 eller mer liter/minut gasflöde.

Indirekt lokal koncentration av nebuliserad TXA

Begränsade data om nebuliserad TXA systematisk absorption.
Aktuella PK-studier av TXA dokumenterar en signifikant minskning av systematiska nivåer men samma hemostaseffekt.
Samling av två blodprover från varje deltagare.
Detta kommer att verifiera en lungfysiologisk baserad PK-modell (PBPK) (d.v.s. näshåla, svalg och lunga) som indirekt förutsäger den orofaryngeala och systematiska koncentrationen av nebuliserad TXA.

Farmakokinetikprover kommer att samlas in efter att den sista nebuliserade behandlingen som erhållits inom sextio minuter har avslutats.
En andra tidpunkt bör sedan samlas in efter sextio minuter upp till åtta timmar från den senaste nebuliseringen, separerad från föregående tidpunkt med minst sextio till nittio minuter.
Serum-TXA-nivåerna kommer att användas för att verifiera en TXA Fysiologisk-baserad farmakokinetisk modell och bestämma populationsvariabiliteten.
Denna PBPK-modell är byggd av vår forskningsapotekare baserad på omfattande forskning som redan genomförts om TXA-distribution och metabolism.
När modellen väl är byggd behöver utredarna bara ett till två prover för att avgöra om modellen korrekt återspeglar insamlade prover.
Utredarna kommer att utveckla en basmodell för att bestämma en kompartmentmodell, fördelnings- och elimineringskinetik som passar bäst.
Utredarna kommer också att använda stokastiska modeller för att utvärdera variabiliteten mellan individer i PK-parametrar.

Blood product transfusion volume will be measured at discharge or 24 hours (whichever comes first).
