在牙科手术过程中,必须对下颌神经支配的结构进行麻醉,文献中描述了许多局部麻醉技术。 通常,浸润丛技术通常用于上颌手术,而 IANB 用于下颌磨牙部位,使用 2% 的局部麻醉剂。 IANB 的失败率很高,使用这种技术的下切牙有时会达到 80%,并且已经描述了许多实施 IANB 新技术的方法。 其中,4%阿替卡因已成功用于局部麻醉,下颌骨面部浸润麻醉技术疗效显着,效果令人鼓舞,可与IANB技术重叠。 Articaine (4-methyl-3-[2-(propylamino)-propionamido]-2-thiophene-carboxylic acid, methyl ester hydrochloride) 含有噻吩环而不是苯和酯基。 噻吩环允许更高的脂溶性,因此更大部分的给药剂量可以进入神经元。 Gazal 发现,与单独使用 IANB 进行下颌第一磨牙牙髓麻醉相比,神经阻滞麻醉、颊侧浸润和韧带内注射的组合导致更深的麻醉(P = 0.003)和更高的成功率。 其他研究报告了阿替卡因在种植干预中的使用,其中它与骨膜下技术一起用于颊侧和前庭侧。

最近对该麻醉技术的研究发现,当下颌面部皮层厚度<2.0-3.0 mm时,1.8 cc 4%阿替卡因面部浸润下颌骨可以有效,需要5-10分钟才能充分麻醉。
据报道,许多乳磨牙的牙科手术可以通过在儿科人群中单独浸润 articaine 来完成。
如上所述,面部局麻药浸润成人下颌第三磨牙难以实现可靠麻醉与骨皮质厚度和无法始终如一地实现下牙槽神经有关。
理想的局麻药应具有以下特点:
- 延迟时间短;
- 强烈效果的强度;
- 长期作用的持续时间;
- 没有有害的局部和全身作用。 目前,甲哌卡因是牙科中使用和研究最多的麻醉剂之一;然而,如今阿替卡因因其易管理、无副作用和高麻醉效力而越来越多地被使用。 甲哌卡因是一种酰胺类麻醉剂,起效迅速,持续时间为 30-120 分钟。 其最大允许剂量为 300 毫克与肾上腺素或单独 500 毫克。 儿童剂量为 4-6 mg/kg/剂量(最大:270 mg),不含肾上腺素。
由于其化学特性,阿替卡因可快速溶解并从脂肪/脂质组织中快速释放。 阿替卡因的酯侧链被血浆酯酶水解,使分子失去活性。 有证据表明,在软组织和硬组织中扩散最好的是局部麻醉剂。 然后,与需要肝脏清除的其他酰胺大约 90 分钟(甲哌卡因为 115 分钟)相比,它对具有较短半衰期 25 分钟的延长手术有不利影响。 因此,有人认为阿替卡因不具有任何相关的副作用或全身毒性。
