Multicentrická, otevřená studie fáze 1/2 monoterapie IMGN632 podávané intravenózně u pacientů s CD123-pozitivní akutní myeloidní leukémií a jinými CD123-pozitivními hematologickými malignitami

Toto je otevřená, multicentrická studie fáze 1/2 ke stanovení MTD a posouzení bezpečnosti, snášenlivosti, PK, imunogenicity a antileukemické aktivity IMGN632 při podávání jako monoterapie pacientům s onemocněním CD123+.

Do studie je zařazena klíčová kohorta pacientů s BPDCN v první linii a kohorta pacientů s relabujícím/refrakterním BPDCN.

Studie dokončila fázi eskalace dávky a nyní se zařazuje do fáze expanze dávky, aby se dále charakterizoval bezpečnostní profil a aby se vyhodnotila účinnost IMGN632 u pacientů s BPDCN. IMGN632 se podává IV v den 1 každého cyklu, přičemž cykly se opakují každých 21 dní.

Kritéria pro zařazení:

  1. Charakteristika onemocnění:

    A. Potvrzení pozitivity CD123 průtokovou cytometrií nebo IHC. Účastníkům, kteří dříve dostávali činidla cílená na CD123, bude povoleno, pokud blasty stále mají detekovatelnou expresi CD123.

  2. Zahrnutí rozšíření:

    • kohorta 1 – účastníci s relabujícím nebo refrakterním blastickým plazmocytoidním dendritickým novotvarem (BPDCN) s 1–3 předchozími liniemi terapie
    • Kohorta 6 – Účastníci s přední linií de novo BPDCN ve screeningu, kteří nedostali předchozí systémovou léčbu a účastníci s přední linií BPDCN, kteří mají PCHM a nedostali předchozí systémovou léčbu.

Poznámka: Účastníci kohorty 6 mohli podstoupit lokální terapii (radioterapii, chirurgickou excizi, fotodynamickou terapii). Způsobilí účastníci musí mít recidivu nebo progresi v oblasti lokální terapie NEBO onemocnění mimo oblast lokální terapie.

Kritéria vyloučení:

  1. Účastníci, kteří podle úsudku jejich ošetřujícího lékaře mají odpovídající standardní terapie, budou vyloučeni z kohort 1 až 5.
  2. Účastníci frontové linie BPDCN s onemocněním centrálního nervového systému (CNS) budou vyloučeni. Lumbální punkce musí být provedena během 28denního screeningového období před podáním léku. Účastníci BPDCN s relapsem nebo refrakterní se známou anamnézou onemocnění CNS musí být léčeni lokálně, musí mít alespoň 1 lumbální punkci bez známek onemocnění CNS a musí být před první dávkou klinicky stabilní. Souběžná léčba profylaxe CNS nebo pokračování léčby kontrolovaného onemocnění CNS je povoleno se souhlasem sponzora.
  3. Účastníci s anamnézou venookluzivního onemocnění jater.
  4. Účastníci s anamnézou syndromu kapilárního úniku 4. stupně nebo nekardiálního edému 4. stupně nejsou vhodní, např. související s tagraxofusp-erzs nebo jinou etiologií.
  5. Interval od předchozí léčby rakoviny: 1. U účastníků BPDCN v první linii s předchozí lokální terapií (např. radioterapií) nesměli účastníci v této studii dostat léčbu během 14 dnů před podáním léku. 2. Účastníci BPDCN s relapsem nebo refrakterní nesmějí během 14 dnů před podáním léku v této studii podstoupit žádnou protirakovinnou terapii včetně chemoterapie, imunoterapie, radioterapie, hormonálních, biologických nebo jakýchkoliv zkoumaných látek. Účastníci se musí zotavit na výchozí hodnotu ze všech akutních toxicit z této předchozí terapie.

Poznámka: Výjimka, že účastníci, kteří dostali inhibitor kontrolního bodu, nesměli dostat tuto terapii během 28 dnů před podáním léku v této studii.
