Téměř 2 miliony lidí v USA podstoupily amputaci a až 90 % lidí s amputací končetiny zažívá přetrvávající pocit chybějící končetiny, fenomén známý jako „fantomová končetina“ (Weeks et al., 2010). Navíc značná část jedinců s fantomovou končetinou – až 85 % v některých studiích – pociťuje přetrvávající a vysilující bolest v chybějící končetině, stav známý jako fantomová bolest končetin (dále PLP). Ačkoli existující terapie v některých případech poskytují úlevu od bolesti, existuje široká shoda, že současné přístupy nedosahují úlevy většině jedinců s PLP (Weeks et al., 2010).
Vyšetřovatelé nedávno dokončili studii proof-of-concept (Ambron et al., 2021), ve které osm subjektů s podkolenní amputací (BKA) podstoupilo dvě léčby PLP ve virtuální realitě (VR). Při léčbě aktivní VR hráli subjekty různé aktivní hry vyžadující pohyby nohou a zároveň dostávaly vysoce kvalitní vizuální zpětnou vazbu o chybějící bérci. Zpětná vazba o poloze nohou byla poskytnuta prostřednictvím elektromagnetického systému využívajícího senzory nohou (trackSTAR, Ascension Technologies Inc) a program vygeneroval obrázek chybějící bérce, viditelný jako avatar z první osoby. Tato léčba byla v kontrastu s léčbou "Distractor", ve které byli účastníci zapojeni do vizuálně pohlcujícího zážitku virtuální reality, který nevyžadoval pohyby nohou (Cool!TM). Obě léčby byly spojeny s významným snížením intenzity bolesti, ale léčba aktivní VR byla také spojena se snížením interference bolesti, deprese a úzkosti.
Konkrétní cíle současné studie jsou (i) replikovat naše předchozí pozorování účinnosti léčby VR na větším vzorku jedinců s BKA; (ii) testovat terapii VR u pacientů s amputacemi nad kolenem; (iii) porovnat účinnost léčby aktivní VR s léčbou distraktorovou VR pro PLP na měření bolesti, stejně jako na psychické zdraví a kvalitu života.

Kritéria pro zařazení:
- Věk 18 - 100 let
- Schopnost poskytnout informovaný souhlas
- Jednostranná amputace nad nebo pod kolenem více než 3 měsíce před zařazením
- Absence kognitivní poruchy, operativně definovaná jako Montreal Cognitive Assessment skóre 24 nebo vyšší
- Bolest zprůměrovaná za předchozí 1 měsíc ve fantomové končetině hodnocená jako větší než 40 na vizuální analogové škále McGill Pain Questionnaire (Melzack, 1987)
Kritéria vyloučení:
- Anamnéza významné lékařské nebo neurologické poruchy, jako je mrtvice nebo středně těžké až těžké traumatické poranění mozku (operačně definované jako ztráta vědomí po dobu delší než 30 minut)
- Historie významných nebo špatně kontrolovaných psychiatrických poruch
- Současná významná deprese nebo úzkost podle hodnocení Hospital Anxiety and Depression Scale (HADS) (Zigmond & Snaith, 1983).
- Současné zneužívání alkoholu nebo drog, na předpis nebo jinak
- Kojení dítěte, těhotné nebo záměr otěhotnět během studie

Experimentální: Active VR treatment
Subjects assigned to the Active VR treatment began by selecting an avatar, with features such as gender and skin color that could be chosen according to preference.
During treatment, they participated in a variety of games and activities developed and used by our teams including Kick, Dog Food, Quest for Fire, Chess, Checkers, Sudoku, and surfing the internet.
Subjects had substantial flexibility to select games according to their interests but were required to spend at least 30 minutes in each session in games that required forceful, large-amplitude movements of the amputated lower limb (e.g., Quest for Fire, Kick).

Experimentální: Distractor VR treatment
Subjects assigned to the Distractor VR treatment participated in the REAL i-Series immersive VR experience (REAL system), which had been demonstrated to reduce pain in several studies but lacked the hypothesized "active ingredients" of our Active VR treatment (visual and auditory feedback of movement of an extrapolated amputated limb).
Subjects navigated through pleasant and relaxing VR environments; they did not see any rendering of their body and made no movements with their legs.

McGill Short Form Questionnaire (Melzack, 1987): a series of 4-point scales (0 none - 3 severe) assessing the intensity of 15 qualitative characteristics of pain (e.g., throbbing, shooting, cramping, etc.), where higher scores are worse (more severe).
For every participant we computed the total pain score (maximum 45 points indicating severe pain).
Then, we computed the difference in the total scores between time points (e.g.
post treatment vs baseline).
Therefore, negative scores indicate improvement in pain qualitative characteristics.
