DESIGNOVÝ PŘÍBĚH:
Studie měla několik vzájemně propojených témat. Jedním z nich bylo prozkoumat vztahy mezi environmentálním stresem a krevním tlakem v řadě časových průběhů, přičemž akutní laboratorní studie na jednom extrému a dlouhodobé prospektivní studie na druhém. Základní model stresu zahrnoval tři prvky: objektivní povahu stresoru, jeho vnímání jedincem a jeho fyziologickou vnímavost. Tento model zahrnoval prvky modelu Karásek Job Strain a Frankenhauserův model úsilí-distress, se dvěma komponentami odpovídajícími požadavku a kontrole. Třetím rozměrem byla sociální podpora. Tyto modely byly testovány v laboratorních i polních podmínkách. Krevní tlak byl hlavní závislou proměnnou spolu se strukturálními změnami v srdci a karotických tepnách. Bylo zahrnuto pět projektů. Projekt I hodnotil schopnost ambulantního krevního tlaku a variability krevního tlaku predikovat kardiovaskulární morbiditu a zahrnoval také průřezovou studii srovnávající vztahy mezi krevním tlakem a poškozením cílových orgánů u černošských a bílých jedinců. Byla také testována hypotéza, že hypertenze bílého pláště je stav spojený s nižším rizikem než trvalá hypertenze. Projekt 2 (The Work Site Blood Pressure Study) prospektivně hodnotil účinky pracovního vypětí a sociální podpory na krevní tlak a další výstupní proměnné u pracující populace. Projekt 3 studoval psychosociální a hormonální faktory ovlivňující denní variace krevního tlaku u žen, včetně změn spojených s menstruací a menopauzou. Projekt 4 (Masked Hypertension Study) hodnotil prevalenci maskované hypertenze (normální TK na klinickém pracovišti, ale zvýšený TK v obvyklém prostředí) a její souvislost se subklinickým kardiovaskulárním onemocněním (CVD); ~50 % vzorku bylo přehodnoceno v průměru o 6 let později. Projekt 5 (Improving the Detection of Hypertension) měl opakovaná hodnocení klinického TK, domácího TK a ambulantního TK, aby se vyhodnotila spolehlivost testu a opakovaného testu u každého z nich a porovnala se jejich asociace se subklinickým KVO.
Studie byla obnovena ve fiskálním roce 1998, fiskálním roce 2003 a ve fiskálním roce 2009, aby pokračovala ve sledování a analýze.
