Avaliar o papel dos fatores psicossociais no desenvolvimento da hipertensão e das doenças cardiovasculares.

NARRATIVA DO PROJETO:
Os estudos tiveram vários temas inter-relacionados. Uma delas era explorar as relações entre o stress ambiental e a pressão arterial ao longo de vários períodos de tempo, com estudos laboratoriais agudos num extremo e estudos prospectivos a longo prazo no outro. O modelo básico de estresse incluía três elementos: a natureza objetiva do estressor, a percepção que o indivíduo tem dele e sua suscetibilidade fisiológica. Este modelo incorporou elementos do modelo Karasek Job Strain e do modelo Frankenhaueser esforço-distress, com dois componentes correspondentes à demanda e ao controle. Uma terceira dimensão foi o apoio social. Esses modelos foram testados em condições de laboratório e de campo. A pressão arterial foi a principal variável dependente, juntamente com alterações estruturais no coração e nas artérias carótidas. Cinco projetos foram incluídos. O Projeto l avaliou a capacidade da pressão arterial ambulatorial e da variabilidade da pressão arterial de prever a morbidade cardiovascular e também incluiu um estudo transversal comparando as relações entre a pressão arterial e danos a órgãos-alvo em indivíduos negros e brancos. A hipótese de que a hipertensão do avental branco é uma condição associada a menor risco do que a hipertensão sustentada também foi testada. O Projeto 2 (Estudo de Pressão Arterial no Local de Trabalho) avaliou prospectivamente os efeitos da tensão no trabalho e do apoio social sobre a pressão arterial e outras variáveis de resultado em uma população trabalhadora. O Projeto 3 estudou fatores psicossociais e hormonais que influenciam as variações diurnas da pressão arterial em mulheres, incluindo as alterações associadas à menstruação e à menopausa. O Projeto 4 (Estudo de Hipertensão Mascarada) avaliou a prevalência de hipertensão mascarada (PA normal no ambiente clínico, mas PA elevada no ambiente habitual) e sua associação com doença cardiovascular subclínica (DCV); Cerca de 50% da amostra foi reavaliada em média 6 anos depois. O Projeto 5 (Melhorando a Detecção de Hipertensão) repetiu avaliações da PA clínica, PA domiciliar e PA ambulatorial, a fim de avaliar a confiabilidade teste-reteste de cada uma e comparar suas associações com DCV subclínica.
O estudo foi renovado no ano fiscal de 1998, no ano fiscal de 2003 e no ano fiscal de 2009 para continuar o acompanhamento e a análise.
