Fattori psicosociali e malattie cardiovascolari
Valutare il ruolo dei fattori psicosociali nello sviluppo dell’ipertensione e delle malattie cardiovascolari.
Stimare la prevalenza dell’ipertensione mascherata e il suo potenziale ruolo nello sviluppo di malattie cardiovascolari subcliniche. Confrontare l’affidabilità della pressione arteriosa ambulatoriale (PA), della PA domiciliare e della PA ambulatoriale, e la loro associazione con la malattia cardiovascolare subclinica.

NARRAZIONE DEL PROGETTO:
Gli studi avevano diversi temi correlati. Il primo consisteva nell'esplorare le relazioni tra stress ambientale e pressione arteriosa in una serie di periodi temporali, con studi di laboratorio acuti da un lato e studi prospettici a lungo termine dall'altro. Il modello base dello stress comprendeva tre elementi: la natura oggettiva del fattore stressante, la percezione che l'individuo ne ha e la sua suscettibilità fisiologica. Questo modello incorporava elementi del modello Karasek Job Strain e del modello sforzo-distress di Frankenhaueser, con due componenti corrispondenti alla domanda e al controllo. Una terza dimensione era il supporto sociale. Questi modelli sono stati testati sia in condizioni di laboratorio che sul campo. La pressione sanguigna era la principale variabile dipendente, insieme ai cambiamenti strutturali nel cuore e nelle arterie carotidi. Sono stati inclusi cinque progetti. Il progetto 1 ha valutato la capacità della pressione arteriosa ambulatoriale e della variabilità della pressione arteriosa di predire la morbilità cardiovascolare e ha incluso anche uno studio trasversale che confrontava le relazioni tra pressione arteriosa e danno agli organi bersaglio in soggetti bianchi e neri. È stata inoltre testata l’ipotesi che l’ipertensione da camice bianco sia una condizione associata a un rischio inferiore rispetto all’ipertensione sostenuta. Il Progetto 2 (Work Site Blood Pressure Study) ha valutato in modo prospettico gli effetti dello stress lavorativo e del supporto sociale sulla pressione sanguigna e su altre variabili di risultato in una popolazione attiva. Il progetto 3 ha studiato i fattori psicosociali e ormonali che influenzano le variazioni diurne della pressione sanguigna nelle donne, compresi i cambiamenti associati alle mestruazioni e alla menopausa. Il Progetto 4 (Masked Hypertension Study) ha valutato la prevalenza dell'ipertensione mascherata (pressione normale in ambito clinico, ma pressione elevata nell'ambiente abituale) e la sua associazione con la malattia cardiovascolare subclinica (CVD); Circa il 50% del campione è stato rivalutato in media 6 anni dopo. Il Progetto 5 (Migliorare la rilevazione dell'ipertensione) prevedeva valutazioni ripetute della PA clinica, della PA domiciliare e della PA ambulatoriale al fine di valutare l'affidabilità test-retest di ciascuna e confrontare le loro associazioni con la CVD subclinica.
Lo studio è stato rinnovato nell'anno fiscale 1998, nell'anno fiscale 2003 e nell'anno fiscale 2009 per continuare il follow-up e l'analisi.
