Czynniki psychospołeczne i choroby układu krążenia
Ocena roli czynników psychospołecznych w rozwoju nadciśnienia tętniczego i chorób układu krążenia.
Ocena częstości występowania maskowanego nadciśnienia tętniczego i jego potencjalnej roli w rozwoju subklinicznej choroby sercowo-naczyniowej. Porównanie wiarygodności pomiarów ciśnienia krwi w gabinecie lekarskim (BP), ciśnienia krwi w domu i ciśnienia ambulatoryjnego oraz ich powiązania z subkliniczną chorobą układu krążenia

NARRACJA PROJEKTOWA:
Badania miały kilka powiązanych ze sobą tematów. Jednym z nich było zbadanie związków między stresem środowiskowym a ciśnieniem krwi w różnych okresach czasu, przy czym na jednym krańcu znalazły się dokładne badania laboratoryjne, a na drugim długoterminowe badania prospektywne. Podstawowy model stresu uwzględniał trzy elementy: obiektywny charakter stresora, jego postrzeganie przez jednostkę oraz jej podatność fizjologiczną. Model ten zawierał elementy modelu Karaska Job Strain i modelu Frankenhauesera wysiłek-dystres, z dwoma komponentami odpowiadającymi popytowi i kontroli. Trzecim wymiarem było wsparcie społeczne. Modele te testowano zarówno w warunkach laboratoryjnych, jak i terenowych. Główną zmienną zależną było ciśnienie krwi, podobnie jak zmiany strukturalne w sercu i tętnicach szyjnych. Do konkursu włączono pięć projektów. W projekcie l oceniano zdolność ambulatoryjnego ciśnienia krwi i zmienności ciśnienia krwi do przewidywania zachorowalności na choroby układu krążenia, a także uwzględniono przekrojowe badanie porównujące związek między ciśnieniem krwi a uszkodzeniem narządów docelowych u osób rasy czarnej i białej. Zbadano także hipotezę, że nadciśnienie białego fartucha jest schorzeniem obarczonym niższym ryzykiem niż nadciśnienie utrwalone. W ramach projektu 2 (badanie ciśnienia krwi w miejscu pracy) oceniano prospektywnie wpływ napięcia zawodowego i wsparcia społecznego na ciśnienie krwi i inne zmienne wynikowe w populacji osób pracujących. W ramach projektu 3 zbadano czynniki psychospołeczne i hormonalne wpływające na dobowe wahania ciśnienia krwi u kobiet, w tym zmiany związane z miesiączką i menopauzą. W projekcie 4 (Badanie Masked Hypertension) oceniano częstość występowania maskowanego nadciśnienia tętniczego (prawidłowe ciśnienie krwi w warunkach klinicznych, ale podwyższone BP w zwykłym środowisku) i jego związek z subkliniczną chorobą sercowo-naczyniową (CVD); Około 50% próby poddano ponownej ocenie średnio po 6 latach. Projekt 5 (Poprawa wykrywania nadciśnienia tętniczego) obejmował powtórną ocenę BP w klinice, w domu i w warunkach ambulatoryjnych w celu oceny wiarygodności testu-powtórzenia każdego z nich i porównania ich powiązania z subkliniczną CVD.
Badanie wznowiono w roku budżetowym 1998, roku budżetowym 2003 i roku budżetowym 2009 w celu kontynuacji działań następczych i analiz.
