Factores psicosociales y enfermedades cardiovasculares
Evaluar el papel de los factores psicosociales en el desarrollo de hipertensión y enfermedad cardiovascular.
Estimar la prevalencia de la hipertensión enmascarada y su papel potencial en el desarrollo de la enfermedad cardiovascular subclínica. Comparar la confiabilidad de la presión arterial (PA) en el consultorio, la PA en el hogar y la PA ambulatoria, y sus asociaciones con la enfermedad cardiovascular subclínica.

NARRATIVA DE DISEÑO:
Los estudios tenían varios temas interrelacionados. Uno de ellos era explorar las relaciones entre el estrés ambiental y la presión arterial a lo largo de una serie de períodos de tiempo, con estudios de laboratorio agudos en un extremo y estudios prospectivos a largo plazo en el otro. El modelo básico de estrés incluía tres elementos: la naturaleza objetiva del factor estresante, la percepción que el individuo tiene del mismo y su susceptibilidad fisiológica. Este modelo incorporó elementos del modelo Karasek Job Strain y del modelo Frankenhaueser esfuerzo-angustia, con dos componentes correspondientes a la demanda y el control. Una tercera dimensión fue el apoyo social. Estos modelos fueron probados tanto en condiciones de laboratorio como de campo. La presión arterial fue la principal variable dependiente, junto con los cambios estructurales en el corazón y las arterias carótidas. Se incluyeron cinco proyectos. El proyecto l evaluó la capacidad de la presión arterial ambulatoria y la variabilidad de la presión arterial para predecir la morbilidad cardiovascular y también incluyó un estudio transversal que compara las relaciones entre la presión arterial y el daño a órganos diana en individuos blancos y negros. También se probó la hipótesis de que la hipertensión de bata blanca es una condición asociada con un riesgo menor que la hipertensión sostenida. El Proyecto 2 (el Estudio de presión arterial en el lugar de trabajo) evaluó prospectivamente los efectos de la tensión laboral y el apoyo social sobre la presión arterial y otras variables de resultado en una población trabajadora. El proyecto 3 estudió los factores psicosociales y hormonales que influyen en las variaciones diurnas de la presión arterial en las mujeres, incluidos los cambios asociados con la menstruación y la menopausia. El Proyecto 4 (el Estudio de Hipertensión Enmascarada) evaluó la prevalencia de hipertensión enmascarada (PA normal en el entorno clínico, pero PA elevada en el entorno habitual) y su asociación con enfermedad cardiovascular subclínica (ECV); ~50% de la muestra fue reevaluada un promedio de 6 años después. El Proyecto 5 (Mejora de la detección de hipertensión) tuvo evaluaciones repetidas de la PA clínica, la PA domiciliaria y la PA ambulatoria para evaluar la confiabilidad test-retest de cada una y comparar sus asociaciones con la ECV subclínica.
El estudio se renovó en el año fiscal 1998, el año fiscal 2003 y el año fiscal 2009 para continuar con el seguimiento y el análisis.
