Inklusionskriterier:
- Mænd og kvinder i alderen 18 til 70 år
- Bor i staterne Missouri eller Illinois
- Klager over lugte af visse ting eller alt er forvrænget
- Olfaktorisk dysfunktion, der har varet ved i >2 måneder efter mistanke om COVID-19-infektion
- Evne til at læse, skrive og forstå engelsk
Ekskluderingskriterier:
- Anamnese med olfaktorisk dysfunktion før COVID-19-infektion
- Enhver brug af samtidige terapier specifikt til behandling af olfaktorisk dysfunktion
- Anamnese med olfaktorisk dysfunktion i mere end 12 måneder
- Anamnese med bipolar lidelse, da SSRI'er teoretisk kan destabilisere bipolar lidelse
- Deltagere med symptomer på depression, som det fremgår af en score på 10 eller højere på Patient Health Questionnaire-9 (PHQ-9).95 PHQ-9 er et spørgeskema med ni punkter designet til at vurdere og hjælpe med at diagnosticere patienter med depression i kliniske og lokale miljøer.
- Anamnese med neurodegenerativ sygdom (dvs. Alzheimers demens, Parkinsons sygdom, Lewy body demens, frontotemporal demens)
- Anamnese med kronisk rhinosinusitis eller sinuskirurgi
- Gravide eller ammende mødre.
- Allerede tilmeldt et andet COVID 19 medicinforsøg eller modtagelse af monoklonal antistofinfusion.
- Indtagelse af donepezil eller fluoxetin (rationale: disse lægemidler er S1R-agonister) eller sertralin (en S1R-antagonist).
- Deltagere, der tager theophyllin, tizanidin, clozapin eller olanzapin (lægemidler med et snævert terapeutisk indeks, der primært metaboliseres af CYP 1A2, som hæmmes af fluvoxamin.
- Indtagelse af en anden SSRI, medmindre en lav dosis (f.eks. escitalopram 5 mg), således at tilføjelse af fluvoxamin ikke vil sætte patienten i fare for serotonergt syndrom.
- Indtagelse af coumadin baseret på teoretisk risiko for øget blødning med fluvoxamin.
- Kan ikke give informeret samtykke.

Eksperimentel: Fluvoxamin
Denne arm vil få den aktive behandling, orale fluvoxaminkapsler på 25 mg hver. De første seks uger vil være gradvis titrering (uge 1 & 2 25mg BID, uge 3 & 4 75mg BID, uge 5 & 6 100mg BID). De følgende seks uger vil være en fast dosis på 100 mg dagligt. De sidste to uger vil være en nedtrapning (første uge 50mg BID og anden uge 25mg BID) Der vil være 14 ugers aktiv behandling, og vurderinger vil blive udført efter afslutningen af uge 12, før nedtrapningsperioden påbegyndes. |
Fluvoxamin er et SSRI, der bruges til depression og angstlidelser.
Denne undersøgelse vil undersøge effektiviteten af fluvoxamin til forbedring af olfaktorisk dysfunktion hos personer med post-Covid-19 parosmi.

Placebo komparator: Placebo
Placebo-kapsler, der ser ud, lugter og smager som fluvoxamin-kapsler, gives til placeboarmen. For at bevare dobbeltblindningen af undersøgelsen vil forsøgspersonerne modtage en kapsel BID i løbet af de første seks uger efter titreringsskemaet og en kapsel TID i løbet af de næste seks uger i den faste dosisperiode. Forsøgspersonerne vil derefter nedtrappe placebo for at efterligne fluvoxamin-armen i to uger. Vurderinger vil blive udført 12 uger efter afslutningen af den faste dosisperiode, før nedtrapningsperioden påbegyndes. |

Clinical Global Impression Scale (CGI)

CGI'en har to komponenter - CGI-Severity og CGI-Improvement. Scorer på CGI-sværhedsskalaen går fra 1 til 7 (1 er normal, 7 er fuldstændig lugttab) og giver information om patientens opfattede sværhedsgrad af deres dysfunktion ved baseline. Scoren på CGI-forbedringsskalaen går fra 1 til 7 (1 er meget forbedret, 7 er meget forværret). Hver vurdering er veldefineret for at maksimere nøjagtigheden. Deltagere, der rapporterer 3 som minimalt forbedret, 2 som meget forbedret eller 1 som meget forbedret i CGI-I, vil blive betragtet som respondere på behandlingen, og antallet af respondere på non-responders vil blive sammenlignet mellem de to arme. Clinical Global Impression-Severity Scale for Parosmia (CGI-P) vil også blive brugt. UPSIT er sammensat af 40 strimler af mikroindkapslede lugtstoffer, som findes nederst på hver side lige under et fire-alternativ multiple-choice spørgsmål.

Depression, angst og stress-skalaen - 21 punkter (DASS-21)

DASS-21 er et sæt af tre selvrapporteringsskalaer designet til at måle den følelsesmæssige tilstand af depression, angst og stress.
Hver af de tre DASS-21 skalaer indeholder 7 emner, opdelt i underskalaer med lignende indhold.
Depressionsskalaen vurderer dysfori, håbløshed, devaluering af livet, selvironiskhed, manglende interesse/involvering, anhedoni og inerti.
Angstskalaen vurderer autonom ophidselse, skeletmuskelpåvirkninger, situationsangst og subjektiv oplevelse af angstpåvirkning.
Stressskalaen er følsom over for niveauer af kronisk ikke-specifik ophidselse og vurderer problemer med at slappe af, nervøs ophidselse og let at være ked af det / ophidset, irritabel / overreaktiv og utålmodig.
