Beschreibung
Einschlusskriterien:
- Alter ≥ 18 Jahre
- Unterschrift der Einverständniserklärung
- Patienten mit Angina pectoris und klinischer Indikation zur koronaren Revaskularisation mit einem medikamentenfreisetzenden Stent
Vorhandensein von mindestens 2 zu revaskularisierenden Läsionen mit ähnlichen angiographischen Merkmalen
- Referenzläsionsdurchmesser mit höchstens 0,5 mm Unterschied
- Maximaler Unterschied in der Läsionslänge von 10 mm
- Läsionen können mit nur 1 Stent behandelt werden
Ausschlusskriterien:
- Läsionen aufgrund von Restenose
- Läsionen in Stammvenentransplantaten
- STE-ACS (ST-Hebung – akutes Koronarsyndrom)
- Kardiogener Schock
- Kontraindikation für eine duale Thrombozytenaggregationshemmung für > 3 Monate
Nachsorgekatheterisierung kontraindiziert
- Chronisches Nierenversagen mit Kreatinin > 2 mg/dL
- Allergie gegen jodhaltige Kontrastmittel
- Schwerwiegende Komplikation durch Gefäßzugang bei vorheriger Katheterisierung
Nicht zur Auswertung mittels optischer Kohärenztomographie geeignet
- Läsion lokalisiert
- Starke proximale Angulation >90º
- Bifurkationsläsion, die eine Strategie mit zwei Stents erfordert
Angiographische Merkmale, die eine Fehlpositionierung des Stents begünstigen
- Aneurysma oder Ektasie der Koronararterie
- Vorhandensein eines Thrombus, der als TIMI-Fluss (Thrombolyse bei Myokardinfarkt)2 oder höher klassifiziert ist
- Starke Verkalkung, insbesondere bei Kalziumspitzen
- Schwere Läsionswinkelung
- Unfähigkeit, nach Meinung des Ermittlers eine zuverlässige Nachuntersuchung durchzuführen
