Einschlusskriterien:
- Alter 18 Jahre oder älter zum Zeitpunkt der Unterzeichnung der Einverständniserklärung (ICF).
- Für Phase 1b: DIPSS-Score (Dynamic International Prognostic Scoring System) von 3 bis 4 (mittleres Risiko 2) oder ≥ 5 (hohes Risiko) primäre MF, Post-PV-MF und/oder Post-ET-MF, wie in bestätigt der neueste lokale Knochenmarkbiopsiebericht gemäß den Kriterien der Weltgesundheitsorganisation (WHO) von 2016.
- Auswaschen von mindestens 28 Tagen vor dem Screening der folgenden Behandlungen: Androgene, Erythropoietin, Cladribin, Immunmodulatoren (Lenalidomid, Thalidomid), Interferon alpha-2a oder jede andere MF-gerichtete Therapie. Systemische Kortikosteroide sind bei nicht hämatologischen Zuständen zulässig, wenn die Dosis für ≥ 28 Tage vor dem Screening stabil oder abnehmend ist und in den 28 Tagen unmittelbar vor dem Screening ein Äquivalent von ≤ 10 mg Prednison erhalten wurde.
- Anämie: Für Phase 1b: Hämoglobin (Hgb) < 10 g/dL bei ≥ 3 Bewertungen über 84 Tage vor dem Screening, ohne Erythrozyten-Transfusion, oder Hgb < 10 g/dL und regelmäßig Erythrozyten-Transfusionen erhalten, aber die Kriterien für TD-Teilnehmer nicht erfüllen für die TD-Kohorte definiert. Der Ausgangs-Hgb-Wert für diese Teilnehmer ist der niedrigste Hgb-Wert während der 84 Tage vor dem Screening oder die Erythrozyten-Transfusionsabhängigkeit, definiert als eine RBC-Transfusionshäufigkeit von ≥ 6 Einheiten verpackter Erythrozyten (PRBC) in den 84 Tagen unmittelbar vor dem Screening. Innerhalb des 84-Tage-Zeitraums darf es keinen aufeinanderfolgenden 42-Tage-Zeitraum ohne Erythrozyten-Transfusion geben, und die letzte Transfusion muss innerhalb von 28 Tagen vor dem Screening erfolgen. Für Phase 2a: Erythrozytentransfusionsabhängigkeit, definiert als eine Erythrozytentransfusionshäufigkeit von ≥ 6 Einheiten PRBC über die 84 Tage unmittelbar vor dem Screening. Innerhalb des 84-Tage-Zeitraums darf es keinen aufeinanderfolgenden 42-Tage-Zeitraum ohne Erythrozyten-Transfusion geben, und die letzte Transfusion muss innerhalb von 28 Tagen vor dem Screening erfolgen.
- Stabile Dosis von JAK-Inhibitor und/oder Hydroxyharnstoff oder, wenn eine andere Behandlung gegen MF eingenommen wird, stabil für mindestens 4 Monate vor dem Screening.
- Leistungspunktzahl der Eastern Cooperative Oncology Group (ECOG) ≤ 2.
- Die Infusion einer hämatopoetischen Stammzelltransplantation wird nicht innerhalb von 8 Monaten nach dem Screening erwartet.
- Eisenkonzentration in der Leber gemäß MRT < 7 mg/g Trockengewicht.
- Serumferritin ≥ 30 μg/l beim Screening.
- Thrombozytenzahl ≥ 25.000/μl und < 1.000.000/μl; Neutrophile ≥ 1.000/μl; und Gesamtzahl der weißen Blutkörperchen (WBC) < 50.000/μl beim Screening.
- Geschätzte glomeruläre Filtrationsrate (eGFR) ≥ 30 ml/min/1,73 m2 nach der Formel der Chronic Kidney Disease-Epidemiology Collaboration (CKD-EPI).
- Aspartat-Aminotransferase (AST) und Alanin-Transaminase (ALT) < 3,0 x Obergrenze des Normalwerts (ULN) beim Screening.
- Direktes Bilirubin < 2x ULN beim Screening. Höhere Konzentrationen sind akzeptabel, wenn diese vom Prüfarzt auf eine ineffektive Erythropoese zurückgeführt werden können.
Ausschlusskriterien:
Krankengeschichte:
- Hereditäre Hämochromatose
- Hämoglobinopathie oder intrinsischer Erythrozytendefekt in Verbindung mit Anämie
- Splenektomie
- Hämatopoetische Zelltransplantation
- Aktuelle Anämie durch Eisenmangel, Vitamin-B12- oder Folatmangel, Infektion oder Blutung
- Aktive immunvermittelte hämolytische Anämie
- Symptomatische Blutungen, die nicht im Zusammenhang mit einer Operation stehen, in einem kritischen Bereich oder Organ und/oder Blutungen, die eine Abnahme des Hgb um ≥ 2 g/dl verursachen oder zu einer Transfusion von ≥ 2 Einheiten Erythrozyten in den 6 Monaten vor dem Screening führen
- Größere Operation innerhalb von 8 Wochen vor dem Screening oder unvollständige Genesung von einer früheren Operation
Malignität innerhalb der letzten 3 Jahre, außer primärer MF, Post-ET oder Post-PV MF. Die folgenden historischen oder gleichzeitigen Bedingungen sind zulässig:
- Basal- oder Plattenepithelkarzinom
- Carcinoma in situ des Gebärmutterhalses oder der Brust
- Histologischer Befund von Prostatakrebs (T1a oder T1b unter Verwendung des klinischen Staging-Systems Tumor, Knoten, Metastasen [TNM])
- Schlaganfall, tiefe Venenthrombose oder Lungen- oder Arterienembolie innerhalb von 6 Monaten vor dem Screening
- Bekannte allergische Reaktion auf einen Hilfsstoff des Studienmedikaments oder Anaphylaxie auf ein Lebensmittel oder Medikament
- Eine Geschichte der Bildung von Anti-Drogen-Antikörpern
- Unzureichend kontrollierte Herzerkrankung (Klassifikation 3 oder 4 der New York Heart Association) und/oder bekanntermaßen linksventrikuläre Ejektionsfraktion < 35 %
- Aktive Hepatitis B oder C oder Humanes Immunschwächevirus (HIV) mit nachweisbarer Viruslast
Unkontrollierte Pilz-, Bakterien- oder Virusinfektion (anhaltende Anzeichen/Symptome im Zusammenhang mit der Infektion, ohne Besserung trotz angemessener Behandlung)
Behandlungshistorie:
- Gleichzeitige oder geplante Behandlung mit Mometinib während des Studienzeitraums
- Eisenchelattherapie in den 3 Monaten vor dem Screening
Änderung des Antikoagulans-Therapieschemas innerhalb von 8 Wochen vor dem Screening
Laborausschlüsse:
- Myeloblasten im peripheren Blut ≥ 10 % der WBC-Differenz bei der letzten Bewertung vor dem Screening
- Positiver direkter Antiglobulintest in Verbindung mit einem reaktiven RBC-Eluat beim Screening
