Efectividad de la Fracción Vascular Estromal (SVF) y el Plasma Rico en Plaquetas (PRP) en Osteoartritis y Tendinopatía (SPOT)
Efectividad de la Fracción Vascular Estromal (SVF) y el Plasma Rico en Plaquetas (PRP) en Osteoartritis y Tendinopatía: Protocolo de Estudio para un Estudio Multicéntrico Prospectivo, Aleatorizado y Controlado de Fase III: (Estudio SPOT).
En un ensayo controlado multicéntrico, aleatorizado, triple ciego, se separarán 195 pacientes en 2 subgrupos: 130 individuos con artrosis y 65 con tendinopatías. La principal pregunta a responder es el efecto de SVF en:
- La mejoría clínica
- La evolución del grosor del cartílago en caso de artrosis
- La curación del tendón en caso de thedinopatía.
Los pacientes recibirán una sola inyección inicial de PRP o PRP + SVF seguida de dosis de PRP de uno y dos meses. Paralelamente, se beneficiarán de un adecuado plan de rehabilitación con fisioterapias activas.

Antecedentes:
La artrosis y las tendinopatías son dos enfermedades frecuentes y de alto impacto social e individual. Ambos tienen una etiología multifactorial y el desarrollo de opciones terapéuticas es una prioridad de salud pública. La osteoartritis es la enfermedad articular más común, y más del 30% de las lesiones relacionadas con el deporte tienen un componente de tendinopatía.
Los tratamientos conservadores más habituales para el tratamiento de la artrosis incluyen analgésicos, fisioterapias activas, ortesis, infiltraciones de corticoides, ácido hialurónico (HA) y plasma rico en plaquetas (PRP).
El PRP puede ser beneficioso tanto en la tendinopatía como en la osteoartritis al interferir con los eventos catabólicos e inflamatorios y, posteriormente, promover las respuestas anabólicas. La activación de PRP libera componentes biológicamente activos, incluido el factor de crecimiento derivado de plaquetas (PDGF), el factor de crecimiento transformante-β (PGF-β), el factor de crecimiento similar a la insulina tipo I (IGF-1) y el factor de crecimiento endotelial vascular (VEGF). Estas proteínas son responsables de una variedad de funciones críticas de curación de tejidos, como la proliferación de condrocitos y células madre mesenquimales, la remodelación de huesos y vasos, la modulación inflamatoria y la síntesis de colágeno.
Para la artrosis, se establece claramente una mejora de los resultados clínicos, presumiblemente asociada al efecto condroprotector del PRP. Sin embargo, aún no se ha demostrado un efecto in vivo sobre la regeneración del cartílago humano a pesar de los numerosos estudios que abordan el tema.
Los tratamientos conservadores más comunes para las tendinopatías incluyen analgésicos, aparatos ortopédicos, terapias físicas activas, terapia de ondas de choque extracorpóreas, otras terapias específicas (crioterapia, terapia con luz roja, trinitrato de glicerol tópico, PRP y alargamiento de tendones). Actualmente se han demostrado beneficios clínicos claros con ejercicios activos y terapias de ondas de choque extracorpóreas. La punción del tendón, también conocida como fenestración del tendón o tenotomía percutánea con aguja, tiene como objetivo interrumpir los procesos inflamatorios y degenerativos crónicos a través del sangrado localizado, la proliferación fibroblástica y la síntesis organizada de colágeno. El tendón intervenido también puede alargarse en algunos casos.
Los modelos preclínicos dilucidaron cómo las células madre mesenquimales derivadas de tejido adiposo (AD-MSC) inyectadas coordinan el proceso de regeneración del cartílago a través de mecanismos paracrinos, produciendo citocinas y factores bioactivos tróficos que estimulan la proliferación celular, reducen la inflamación, la fibrosis, el estrés oxidativo y la senescencia de los condrocitos.
La Fracción Vascular Estromal (SVF), un producto del procesamiento específico del tejido adiposo, contiene células madre mesenquimales, células precursoras endoteliales, células T reguladoras, macrófagos, células de músculo liso, pericitos y preadipocitos. Las técnicas de extracción e inyección de SVF se han utilizado recientemente como una alternativa para recolectar AD-MSC debido a su simplicidad logística y factibilidad en la práctica clínica.
Las inyecciones de SVF producen un efecto clínicamente significativo en el tratamiento de la artrosis de rodilla y una posible mejora en la calidad del cartílago. Se observaron resultados prometedores en el tendón de Aquiles.
Este ensayo clínico tiene como objetivo evaluar la eficacia clínica de SVF como terapia adyuvante al PRP en la funcionalidad y regeneración tisular en osteoartritis y tendinopatías.

Descripción
Criterios de inclusión:
- Consentimiento informado documentado por firma (Apéndice Formulario de consentimiento informado)
- Edad mayor de 16 años,
- Artrosis sintomática de cadera, rodilla, tobillo, codo, hombro confirmada por resonancia magnética o tendinopatía sintomática confirmada por ultrasonografía,
- Fracaso de manejo conservador de primera línea en los últimos 3 meses incluyendo tratamiento médico o infiltrativo, uso de ortesis, plan de rehabilitación activa, adaptación de hábitos deportivos y laborales.
Criterio de exclusión:
- El paciente está familiarizado con el proceso de lipoaspiración.
- En caso de tendinopatía: pinzamiento significativo a juicio del investigador
- Pinzamiento subacromial del tendón del supraespinoso, enfermedad de Haglund con erosión de la parte anterior del tendón de Aquiles
- Enfermedad significativa del miembro contralateral con una función evaluada con puntaje SANE por debajo del 80%
- En caso de artrosis: enfermedad microcristalina (es decir, gota, seudogota),
- Trastornos reumáticos inflamatorios activos,
- Necesidad de tratamiento antiinflamatorio regular (ya sea AINE o corticosteroides),
- Alergia a anestésicos locales o epinefrina
- Trastornos hemorrágicos o tratamiento anticoagulante actual
- Pacientes con insuficiencia renal descompensada, disfunción hepática o enfermedad pulmonar o cardiovascular grave,
- Pacientes con un estado inmunocomprometido
- Mujeres que están embarazadas o tienen la intención de quedar embarazadas durante el estudio
- Incapacidad para seguir los procedimientos del estudio, por ejemplo, debido a problemas de lenguaje, trastornos psicológicos, demencia, etc. del participante,
- Incumplimiento conocido o sospechado, abuso de drogas o alcohol
- Inscripción previa en el estudio actual,
- Participación en otro estudio con fármaco o procedimiento en investigación dentro de los 30 días anteriores y durante el presente estudio
- Inscripción del investigador, sus familiares, empleados y otras personas dependientes
Si hay una enfermedad bilateral y ambos lados requieren la intervención experimental o de control, solo se estudiará el lado más sintomático.

Experimental: SVF (fracción vascular estromal)
Los pacientes recibirán una venopunción para obtener PRP y un lipoaspirado para obtener SVF. Luego se realizará una inyección de PRP+SVF guiada por ultrasonografía. Los pacientes recibirán consecutivamente dos inyecciones mensuales de PRP. Los pacientes en el subgrupo de tendinopatías además se someterán a la punción del tendón concomitantemente a cada inyección. |
Procedimiento para preparar SVF: En el quirófano y previa técnica aséptica, se aplica anestesia local en el sitio de la incisión de la liposucción con lidocaína al 1% sin epinefrina por vía subcutánea.
Se inyectan 60 ml de solución tumescente.
Después de 15-20 minutos de espera, se recogen 15 ml de lipoaspiración por lado en una jeringa doble.
Esta se centrifuga durante 4 minutos a 2.500 rpm y la grasa restante se separa de las demás fracciones.
Se unen dos jeringas GEMS de 1,4 mm y la grasa se transfiere al menos 30 veces de una jeringa a la otra.
El contenido de la jeringa se centrifuga de nuevo durante 4 minutos.
El aceite se desecha y queda aproximadamente 1,5 ml de fracción SVF.

Comparador activo: PRP (plasma rico en plaquetas)
Los pacientes recibirán una venopunción para obtener PRP y un lipoaspirado simulado. Luego se realizará una inyección de PRP guiada por ultrasonografía. Los pacientes recibirán consecutivamente dos inyecciones mensuales de PRP. Los pacientes en el subgrupo de tendinopatías además se someterán a la punción del tendón concomitantemente a cada inyección. |
Procedimiento para preparar PRP: Se centrifugan 15 cm de sangre periférica obtenida por venopunción a 5000 RPM durante 5 minutos.
Usando el kit PRP Arthrex, las plaquetas se desechan y el plasma pobre se desecha y 1-3 mm de PRP están listos para ser inyectados.
