A stromális vascularis frakció (SVF) és a vérlemezkékben gazdag plazma (PRP) hatékonysága osteoarthritisben és tendinopathiában: III. fázisú, prospektív, randomizált, kontrollált többközpontú vizsgálat vizsgálati protokollja: (SPOT vizsgálat).
Egy multicentrikus, randomizált, hármas-vak, kontrollos vizsgálatban 195 beteget 2 alcsoportra osztanak: 130 osteoarthritisben és 65 tendinopathiában szenvedő beteget. A legtöbb megválaszolandó kérdés az SVF hatása a következőkre:
- A klinikai javulás
- A porc vastagságának alakulása osteoarthritis esetén
- Az inak gyógyulása thendinopathia esetén
A betegek kezdetben egyszeri PRP vagy PRP + SVF injekciót kapnak, majd egy és két hónapos PRP adagokat kapnak. Ezzel párhuzamosan egy megfelelő rehabilitációs tervben részesülnek aktív fizikai terápiákkal.

Háttér:
Az osteoarthritis és az tendinopathia két gyakori betegség, amelyek társadalmi és egyéni hatással is rendelkeznek. Mindkettő többtényezős etiológiájú, és a terápiás lehetőségek fejlesztése közegészségügyi prioritás. Az osteoarthritis a leggyakoribb ízületi megbetegedés, és a sporttal kapcsolatos sérülések több mint 30%-a ínhártyagyulladással jár.
Az osteoarthritis kezelésének leggyakoribb konzervatív kezelési módjai közé tartoznak a fájdalomcsillapítók, az aktív fizikai terápiák, az ortopédia, a kortikoszteroidok, a hialuronsav (HA) és a vérlemezkében gazdag plazma (PRP) beszűrődése.
A PRP jótékony hatású lehet tendinopathia és osteoarthritis esetén azáltal, hogy megzavarja a katabolikus és gyulladásos eseményeket, és ezt követően elősegíti az anabolikus válaszokat. A PRP aktiválása biológiailag aktív komponenseket szabadít fel, beleértve a thrombocyta eredetű növekedési faktort (PDGF), a transzformáló növekedési faktor-β-t (PGF-β), az I. típusú inzulinszerű növekedési faktort (IGF-1) és a vaszkuláris endoteliális növekedési faktort (VEGF). Ezek a fehérjék felelősek számos kritikus szövetgyógyító szerepért, mint például a porcsejtek és a mezenchimális őssejtek proliferációjáért, a csontok és az erek átépüléséért, a gyulladás modulálásáért és a kollagén szintéziséért.
Az osteoarthritis esetében a klinikai eredmények javulása egyértelműen megállapítható, feltehetően a PRP chondroprotektív hatásával összefüggésben. Mindazonáltal az emberi porc regenerációjára kifejtett in vivo hatást még nem mutatták ki, annak ellenére, hogy számos tanulmány közelíti meg a témát.
A tendinopátiák leggyakoribb konzervatív kezelései közé tartoznak a fájdalomcsillapítók, a merevítés, az aktív fizikai terápiák, az extrakorporális lökéshullám-terápia, egyéb specifikus terápiák (krioterápia, vörös fényterápia, helyi glicerin-trinitrát, PRP és ínhosszabbítás). Jelenleg az aktív gyakorlatok és az extrakorporális lökéshullám-terápiák egyértelmű klinikai előnyöket mutatnak. Az íntűzés, más néven ínfenestráció vagy perkután tűtenotómia, a krónikus gyulladásos és degeneratív folyamatok megzavarását célozza a lokalizált vérzés, a fibroblaszt proliferáció és a szervezett kollagénszintézis révén. A beavatkozó ín bizonyos esetekben megnyúlhat.
A preklinikai modellek megvilágították, hogy az injektált zsírból származó-mezenchimális őssejtek (AD-MSC) hogyan koordinálják a porc regenerációs folyamatát parakrin mechanizmusokon keresztül, citokineket és trofikus bioaktív faktorokat termelnek, amelyek stimulálják a sejtproliferációt, csökkentik a gyulladást, a fibrózist, az oxidatív stresszt és a porcsejtek öregedését.
A Stromal Vascular Fraction (SVF) egy speciális zsírszövet-feldolgozásból származó termék, amely mezenchimális őssejteket, endoteliális prekurzor sejteket, T szabályozó sejteket, makrofágokat, simaizomsejteket, pericitákat és preadipocitákat tartalmaz. Az SVF extrakciós és injekciós technikákat a közelmúltban az AD-MSC begyűjtésének alternatívájaként alkalmazták logisztikai egyszerűsége és a klinikai gyakorlatban való megvalósíthatósága miatt.
Az SVF injekciók klinikailag jelentős hatást fejtenek ki a térd osteoarthritis kezelésében, és javítják a porc minőségét. Ígéretes eredményeket figyeltek meg az Achilles-ín esetében.
Ennek a klinikai vizsgálatnak az a célja, hogy felmérje az SVF klinikai hatékonyságát a PRP adjuváns terápiájaként az osteoarthritis és az tendinopathiák funkcionalitására és szövetregenerációjára vonatkozóan.

Bevételi kritériumok:
- Aláírással dokumentált tájékoztatáson alapuló beleegyezés (Függelék Tájékoztatott beleegyező nyilatkozat)
- 16 évesnél idősebb,
- A csípő-, térd-, boka-, könyök-, vállízület tüneti osteoarthritisét MRI-vel igazolták, vagy tüneti tendinopathiát ultrahanggal igazoltak,
- Az első vonalbeli konzervatív kezelés kudarca az elmúlt 3 hónapban, beleértve az orvosi vagy infiltratív kezelést, az ortopédia használatát, az aktív rehabilitációs tervet, a sport- és munkaszokások adaptálását.
Kizárási kritériumok:
- A beteg ismeri a lipoaspirációs folyamatot
- Tentinopátia esetén: a vizsgáló megítélése szerint jelentős behatás
- A supraspinatus ín szubakromiális ütközése, Haglund-kór az Achilles-ín elülső részének eróziójával
- Az ellenoldali tag szignifikáns betegsége 80% alatti SANE pontszámmal értékelt funkcióval
- Osteoarthritis esetén: mikrokristályos betegség (pl. köszvény, álköszvény),
- Aktív gyulladásos reumás betegségek,
- Rendszeres gyulladáscsökkentő kezelés szükségessége (nem szteroid gyulladásgátlók vagy kortikoszteroidok),
- Allergia helyi érzéstelenítőkre vagy epinefrinre
- Vérzési rendellenességek vagy jelenlegi antikoaguláns terápia
- Dekompenzált veseelégtelenségben, májműködési zavarban vagy súlyos tüdő- vagy szív- és érrendszeri betegségben szenvedő betegek,
- Immunkompromittált állapotú betegek
- A vizsgálat alatt terhes vagy teherbe esni szándékozó nők
- A vizsgálati eljárások betartásának képtelensége, például a résztvevő nyelvi problémái, pszichés zavarai, demenciája stb.
- Ismert vagy feltételezett be nem tartás, kábítószerrel vagy alkohollal való visszaélés
- Korábbi beiratkozás a jelenlegi tanulmányba,
- Részvétel egy másik vizsgálatban vizsgált gyógyszerrel vagy eljárással a jelen vizsgálatot megelőző 30 napon belül és a jelen vizsgálat során

Kísérleti: SVF (Stroma vascularis frakció)
A betegek vénapunkciót kapnak, hogy PRP-t kapjanak, és lipoaspirátumot, hogy SVF-et kapjanak. Ezután ultrahangos irányított PRP+SVF injekciót végeznek. A betegek egymást követően két havi PRP injekciót kapnak. |
Az SVF elkészítésének menete: A műtőben és aszeptikus technika után helyi érzéstelenítést alkalmaznak a zsírleszívás bemetszés helyén 1%-os lidokainnal, epinefrin nélkül, szubkután.
60 ml tumeszcens oldatot fecskendezünk be.
15-20 perc várakozás után oldalanként 15 ml lipoaspirációt gyűjtünk egy dupla fecskendőbe.
Ezt 4 percig 2500 fordulat/perc sebességgel centrifugáljuk, és a maradék zsírt elválasztjuk a többi frakciótól.
Két 1,4 mm-es GEMS fecskendő van egymáshoz rögzítve, és a zsír legalább 30-szor kerül át az egyik fecskendőből a másikba.
A fecskendő tartalmát ismét centrifugáljuk 4 percig.
Az olajat kiöntjük, és körülbelül 1,5 ml SVF-frakció marad vissza.

A PRP elkészítésének menete: 15 cm vénapunkcióval nyert perifériás vért 5000 RPM-en centrifugálunk 5 percig.
A PRP Arthrex kit használatával a gyenge plazmát eldobják, és 1-3 mm PRP készen áll az injekció beadására.
