Description détaillée
Le traitement des patients atteints de resténose avec stent nu reste un défi clinique et technique majeur. Actuellement, les stents à élution médicamenteuse sont largement utilisés dans la prise en charge de ces patients. Récemment, la valeur du ballonnet à élution de paclitaxel dans ce contexte clinique a été suggérée. Cependant, il n'y a que peu de données observationnelles sur la valeur de ce nouveau ballonnet chez les patients atteints de resténose avec stent métallique nu.
Cet essai clinique prospectif, multicentrique et randomisé comparera deux stratégies interventionnelles différentes (à savoir, ballonnet à élution de paclitaxel vs stent à élution d'évérolimus) dans le traitement des patients atteints de resténose à stent métallique nu.

Critère d'intégration:
PATIENT:
- Âge entre 20 et 85 ans
- Consentement éclairé signé
- Acceptation du contrôle angiographique tardif (6-9 mois)
- Angine de poitrine et/ou preuve objective d'ischémie
LÉSION:
- Resténose intra-stent (> 50 % visuelle) tout stent en métal nu
- Connaissance de l'emplacement antérieur du stent
Critère d'exclusion:
PATIENT:
- Inclusion dans un autre protocole de recherche clinique
- Femmes en âge de procréer
- Maladie systémique associée sévère (y compris insuffisance rénale ou hépatique)
- Dépression sévère de la fraction d'éjection ventriculaire gauche (FEVG < 25 %)
- Maladie qui affecte l'espérance de vie
- Infarctus du myocarde récent (<7 jours)
- Temps d'implantation du BMS précédent <1 mois
- Difficultés sévères attendues pour une étude angiographique tardive
LÉSION:
- Echec angiographique lors de la mise en place du stent initial (sténose résiduelle > 50%)
- Image d'un gros thrombus dans l'endoprothèse (> diamètre du vaisseau)
- Tortuosité ou Ca++ avec difficultés très sévères lors du déploiement préalable du stent
- Diamètre du vaisseau <2 mm (estimation visuelle)
- Resténose uniquement "à l'extérieur" du stent (le bord du stent n'est pas affecté)
- Resténose complètement occlusive (100 %, TIMI 0)
- Resténose très diffuse (> 30 mm de longueur)
