Descripción detallada
El tratamiento de pacientes con reestenosis de stent de metal desnudo sigue siendo un importante desafío clínico y técnico. Actualmente, los stents liberadores de fármacos son ampliamente utilizados en el manejo de estos pacientes. Recientemente, se ha sugerido el valor del balón liberador de paclitaxel en este entorno clínico. Sin embargo, solo hay datos observacionales escasos sobre el valor de este nuevo balón en pacientes con reestenosis del stent de metal desnudo.
Este ensayo clínico prospectivo, multicéntrico y aleatorizado comparará dos estrategias de intervención diferentes (es decir, balón liberador de paclitaxel frente a stent liberador de everolimus) en el tratamiento de pacientes con reestenosis de stent de metal desnudo.

Descripción
Criterios de inclusión:
PACIENTE:
- Edad entre 20 y 85 años
- Consentimiento informado firmado
- Aceptación de control angiográfico tardío (6-9 meses)
- Angina y/o evidencia objetiva de isquemia
LESIÓN:
- Reestenosis In-Stent (> 50% visual) cualquier stent de metal desnudo
- Conocimiento de la ubicación previa del stent
Criterio de exclusión:
PACIENTE:
- Inclusión en otro protocolo de investigación clínica
- Las mujeres en edad fértil
- Enfermedad sistémica grave asociada (incluyendo insuficiencia renal o hepática)
- Depresión severa de la fracción de eyección del ventrículo izquierdo (FEVI <25%)
- Enfermedad que afecta la esperanza de vida
- Infarto de miocardio reciente (<7 días)
- Tiempo de implantación del BMS anterior <1 mes
- Dificultades severas esperadas para estudio angiográfico tardío
LESIÓN:
- Fracaso angiográfico durante la implantación del stent inicial (estenosis residual > 50%)
- Imagen de un gran trombo en el stent (> diámetro del vaso)
- Tortuosidad o Ca++ con dificultades muy severas durante el despliegue previo del stent
- Diámetro del vaso <2 mm (estimación visual)
- Reestenosis solo "fuera" del stent (El borde del stent no se ve afectado)
- Reestenosis completamente oclusiva (100%, TIMI 0)
- Reestenosis muy difusa (> 30 mm de longitud)
