Descrizione dettagliata
Il trattamento dei pazienti con restenosi da stent metallico nudo rimane una delle principali sfide cliniche e tecniche. Attualmente, gli stent a rilascio di farmaco sono ampiamente utilizzati nella gestione di questi pazienti. Recentemente, è stato suggerito il valore del palloncino a rilascio di paclitaxel in questo contesto clinico. Tuttavia, ci sono solo scarsi dati osservazionali sul valore di questo nuovo palloncino nei pazienti con restenosi da stent metallico nudo.
Questo studio clinico prospettico, multicentrico, randomizzato metterà a confronto due diverse strategie interventistiche (vale a dire, palloncino a rilascio di paclitaxel vs stent a rilascio di everolimus) nel trattamento di pazienti con restenosi da stent metallico nudo.

Descrizione
Criterio di inclusione:
PAZIENTE:
- Età compresa tra i 20 e gli 85 anni
- Consenso informato firmato
- Accettazione del controllo angiografico tardivo (6-9 mesi)
- Angina e/o evidenza obiettiva di ischemia
LESIONE:
- Ristenosi intra-stent (> 50% visiva) qualsiasi stent metallico nudo
- Conoscenza della precedente posizione dello stent
Criteri di esclusione:
PAZIENTE:
- Inclusione in un altro protocollo di ricerca clinica
- Donne in età fertile
- Grave malattia sistemica associata (inclusa insufficienza renale o epatica)
- Grave depressione della frazione di eiezione ventricolare sinistra (LVEF <25%)
- Malattia che colpisce l'aspettativa di vita
- Infarto miocardico recente (<7 giorni)
- Tempo di impianto del precedente BMS <1 mese
- Gravi difficoltà attese per lo studio angiografico tardivo
LESIONE:
- Fallimento angiografico durante l'implementazione dello stent iniziale (stenosi residua> 50%)
- Immagine di un grosso trombo nello stent (> diametro del vaso)
- Tortuosità o Ca + + con difficoltà molto gravi durante il precedente posizionamento dello stent
- Diametro del vaso <2 mm (stima visiva)
- Restenosi solo "fuori" dallo stent (il bordo dello stent non è interessato)
- Restenosi completamente occlusiva (100%, TIMI 0)
- Restenosi molto diffusa (lunghezza > 30 mm)
