Gedetailleerde beschrijving
De behandeling van patiënten met restenose van een kale metalen stent blijft een grote klinische en technische uitdaging. Momenteel worden medicijnafgevende stents veel gebruikt bij de behandeling van deze patiënten. Onlangs is de waarde van de paclitaxel-eluerende ballon in deze klinische setting gesuggereerd. Er zijn echter slechts schaarse observationele gegevens over de waarde van deze nieuwe ballon bij patiënten met bare-metalen stentrestenose.
Deze prospectieve, multicenter, gerandomiseerde klinische studie zal twee verschillende interventionele strategieën vergelijken (namelijk paclitaxel-eluting ballon versus everolimus-eluting stent) bij de behandeling van patiënten met kale metalen stentrestenose.

Inclusiecriteria:
GEDULDIG:
- Leeftijd tussen 20 en 85 jaar
- Ondertekende geïnformeerde toestemming
- Acceptatie van late angiografische controle (6-9 maanden)
- Angina en / of objectief bewijs van ischemie
LAESIE:
- Restenose in de stent (> 50% visueel) elke kale metalen stent
- Kennis van eerdere stentlocatie
Uitsluitingscriteria:
GEDULDIG:
- Opname in een ander klinisch onderzoeksprotocol
- Vrouwen in de vruchtbare leeftijd
- Ernstige geassocieerde systemische ziekte (inclusief nier- of leverfalen)
- Ernstige depressie van de linkerventrikelejectiefractie (LVEF <25%)
- Ziekte die de levensverwachting beïnvloedt
- Recent myocardinfarct (<7 dagen)
- Tijdstip van implantatie van het vorige BVK <1 maand
- Ernstige moeilijkheden verwacht voor laat angiografisch onderzoek
LAESIE:
- Angiografische mislukking tijdens implementatie van initiële stent (reststenose> 50%)
- Afbeelding van grote trombus in stent (> bloedvatdiameter)
- Tortuositeit of Ca++ met zeer ernstige moeilijkheden tijdens eerdere plaatsing van de stent
- Vatdiameter <2 mm (visuele schatting)
- Restenose alleen "buiten" de stent (De rand van de stent wordt niet aangetast)
- Volledig occlusieve restenose (100%, TIMI 0)
- Zeer diffuse restenose (> 30 mm lengte)
