Description détaillée
L'Andhra Pradesh est l'État le plus touché par le VIH en Inde. Anantapur est un district situé à la frontière sud de l'Andhra Pradesh avec 72% de population rurale et un taux d'alphabétisation des adultes de 74,1% chez les hommes et de 54,3% chez les femmes. À Anantapur, l'épidémie de VIH est largement due à la transmission hétérosexuelle et se caractérise par de mauvaises conditions socio-économiques et des niveaux élevés d'analphabétisme.
Rural Development Trust (RDT) est une organisation non gouvernementale qui fournit gratuitement des soins médicaux aux personnes vivant avec le VIH, y compris des médicaments, des frais de consultation et d'admission. À l'hôpital Bathalapalli, des cliniques externes et 71 lits sont réservés exclusivement aux patients infectés par le VIH ou la tuberculose.
L'étude de cohorte VIH Vicente Ferrer (VFHCS) est une étude de cohorte ouverte prospective de tous les patients infectés par le VIH qui ont fréquenté l'hôpital Bathalapalli RDT. Les données cliniques de routine des patients sont collectées de manière prospective depuis septembre 2009 et saisies dans une base de données SQL-server utilisant C # comme frontal. Les détails de la voie de transmission, les facteurs de risque associés au VIH et les données sociodémographiques sont recueillis lors de l'inscription. Les données recueillies comprennent le traitement médical (traitement antirétroviral et autres médicaments), les examens de laboratoire (hémogramme, tests de la fonction rénale, tests de la fonction hépatique, numération des lymphocytes CD4, infections bactériennes) et les diagnostics standardisés.
L'objectif du VFHCS est de décrire les aspects sociaux et médicaux liés au VIH dans le contexte de l'investigateur, notamment :
- Épidémiologie : tendances de l'incidence du VIH, mécanismes de transmission, facteurs de risque socioéconomiques associés au VIH.
- Incidence et prévalence des infections opportunistes, en particulier la tuberculose et la cryptococcose.
- Espérance de vie et facteurs associés à la mortalité
- Décrire la rétention tout au long du continuum de soins et enquêter sur les facteurs associés à la perte de suivi.
- Toxicité et efficacité de la thérapie antirétrovirale.
- Efficacité des programmes de prévention de la transmission mère-enfant.
- Incidence et prévalence des maladies non transmissibles telles que le cancer ou les maladies cardiovasculaires.
