Andhra Pradesh é o estado com maior carga de HIV na Índia. Anantapur é um distrito situado na fronteira sul de Andhra Pradesh com 72% de população rural e taxa de alfabetização de adultos de 74,1% em homens e 54,3% em mulheres. Em Anantapur, a epidemia de HIV é em grande parte impulsionada pela transmissão heterossexual e é caracterizada por más condições socioeconômicas e altos níveis de analfabetismo.
A Rural Development Trust (RDT) é uma organização não-governamental que oferece assistência médica gratuita a pessoas vivendo com HIV, incluindo medicamentos, consultas e taxas de admissão. No Hospital Bathalapalli, ambulatórios e 71 leitos são alocados exclusivamente para pacientes infectados pelo HIV ou tuberculose.
O Vicente Ferrer HIV Cohort Study (VFHCS) é um estudo prospectivo de coorte aberto de todos os pacientes infectados pelo HIV que frequentaram o Bathalapalli RDT Hospital. Os dados clínicos de rotina dos pacientes são coletados prospectivamente desde setembro de 2009 e inseridos em um banco de dados SQL-server usando C# como front-end. Detalhes da via de transmissão, fatores de risco associados ao HIV e dados sociodemográficos são coletados no momento da inscrição. Os dados coletados incluem tratamento médico (tratamento antirretroviral e outros medicamentos), investigações laboratoriais (hemograma, testes de função renal, testes de função hepática, contagem de linfócitos CD4, infecções bacterianas) e diagnósticos padronizados.
O objetivo do VFHCS é descrever aspectos sociais e médicos relacionados ao HIV no contexto dos investigadores, incluindo:
- Epidemiologia: tendências na incidência do HIV, mecanismos de transmissão, fatores de risco socioeconômicos associados ao HIV.
- Incidência e prevalência de infecções oportunistas, especialmente tuberculose e criptococose.
- Expectativa de vida e fatores associados à mortalidade
- Descrever a retenção em todo o continuum de cuidados e investigar os fatores associados à perda de acompanhamento.
- Toxicidade e eficácia da terapia antirretroviral.
- Eficácia dos programas de prevenção da transmissão de mãe para filho.
- Incidência e prevalência de doenças não transmissíveis, como câncer ou doenças cardiovasculares.
