Description détaillée

Les principaux objectifs de l'étude sont de confirmer l'innocuité de l'acétaminophène oral chez les nourrissons de naissance extrêmement faible, administré en même temps que l'ibuprofène intraveineux et également de déterminer son efficacité à augmenter de manière significative les taux de fermeture canalaire par rapport à la seule thérapie intraveineuse à l'ibuprofène. Par conséquent, la principale variable de résultat comprend le taux de réussite de la fermeture du canal artériel perméable, sur la base des preuves échocardiographiques transthoraciques 2D.

Cette étude est un essai monocentrique, randomisé, en double aveugle, contrôlé par placebo. Les nourrissons prématurés dont l'âge gestationnel est ≤ 27 6/7 semaines selon la meilleure estimation obstétricale sont éligibles pour l'inscription.

Pour la randomisation, la population de l'étude sera stratifiée en deux sous-groupes en fonction de l'âge gestationnel (AG ≤ 24 semaines et > 25 semaines). Critère d'intégration:

  1. Nourrisson prématuré ≤27 6/7
  2. Le consentement écrit des parents est obtenu
  3. Le nourrisson a besoin d'une assistance respiratoire
  4. diagnostic d'un canal artériel perméable persistant après l'âge de 5 jours (défini comme au moins deux des éléments suivants :

    1. Taille du canal ≥ 1,5 mm

    4. Large pression d'impulsion

Critère d'exclusion:

  1. Pas de consentement parental
  2. Nourrissons > 21 jours d'âge postnatal
  3. Anomalies congénitales telles que des anomalies cardiaques ou multiples
  4. Infection (p. ex., septicémie, pneumonie)
  5. Trouble hémorragique ou numération plaquettaire < 50 000/ml

Expérimental: ibuprofen and acetaminophen arm (intervention arm)
ibuprofen and acetaminophen will be administered concomitantly. Ibuprofen will be administered through intravenous route. The dose of ibuprofen will be the standard dosing regimen used in our unit: dosing based on the postnatal age. For Infants > 108 h of postnatal age: 18 mg/kg/dose loading dose followed by 9 mg/kg/dose, two doses at q24 intervals, started 24 h after the loading dose. Acetaminophen will be administered as oral formulation. Acetaminophen is given at a dose of 15 mg/Kg/dose, q 6 hours, for 3 days (total of 12 doses).
