De primära syftena med studien är att bekräfta säkerheten av oral acetaminophen hos spädbarn med extremt låg födsel, givet samtidigt med intravenös ibuprofen och även att fastställa dess effektivitet för att avsevärt öka graden av ductal stängning jämfört med endast intravenös ibuprofenbehandling. Följaktligen inkluderar den primära utfallsvariabeln patent ductus arteriosus closure rate, baserat på 2-D transthorax ekokardiografiska bevis.
Denna studie är en enkelcenter, randomiserad, dubbelblind, placebokontrollerad studie. För tidigt födda barn med en graviditetsålder på ≤27 6/7 veckor enligt bästa obstetriska uppskattning är berättigade till inskrivning.
För randomisering kommer studiepopulationen att stratifieras till två undergrupper baserat på graviditetsålder (GA ≤24 veckor och > 25 veckor). Randomisering kommer att ske genom att använda datorgenererad slumpmässig sekvens, med en 4-blocksdesign, med 1:1 parallell allokering. Tilldelningar inkluderar behandling och kontrollarm. Behandlingsarmen kommer att få en kombination av intravenöst ibuprofen och oralt paracetamol, medan kontrollarmen kommer att få intravenöst ibuprofen och oralt placebo.

Beskrivning
Inklusionskriterier:
- För tidigt födda barn ≤27 6/7
- Skriftligt medgivande från föräldrarna erhålls
- Spädbarn behöver andningsstöd
diagnos av en ihållande patenterad ductus arteriosus efter 5 dagars ålder, (definierad som minst två av följande:
- Ductus storlek ≥ 1,5 mm
- Maximal flödeshastighet genom kanalen ≤ 2 m/s
- Vänster förmak till aorta förhållande ≥ 1,4
- Brett pulstryck
- B-typ natriuretisk peptid (BNP)
- Behandlande neonatolog fattade beslutet att behandla patent ductus arteriosus
Exklusions kriterier:
- Inget förälders samtycke
- Spädbarn > 21 dagar i postnatal ålder
- Medfödda anomalier såsom hjärt- eller multipla anomalier
- Infektion (t.ex. septikemi, lunginflammation)
- Blödningsrubbning eller trombocytantal < 50 000/ml
- Akut njurskada (AKI) definieras som oliguri (urinproduktion < 0,5 ml/kg/timme i 16 timmar) och/eller serumkreatinin > 1,5 mg/dl
- Förhöjda leverenzymer (>2 gånger från övre normala gränser)
- Pulmonell hypertoni eller höger till vänster shunt genom ductus arteriosus
- Diagnos av nekrotiserande enterokolit
- Kan inte tolerera orala mediciner vid tidpunkten för inskrivningen.

Experimentell: ibuprofen and acetaminophen arm (intervention arm)
ibuprofen and acetaminophen will be administered concomitantly.
Ibuprofen will be administered through intravenous route.
The dose of ibuprofen will be the standard dosing regimen used in our unit: dosing based on the postnatal age.
For Infants > 108 h of postnatal age: 18 mg/kg/dose loading dose followed by 9 mg/kg/dose, two doses at q24 intervals, started 24 h after the loading dose.
Acetaminophen will be administered as oral formulation.
Acetaminophen is given at a dose of 15 mg/Kg/dose, q 6 hours, for 3 days (total of 12 doses).

Ibuprofen will be administered through intravenous route.
The dose of ibuprofen will be the standard dosing regimen used in our unit: dosing based on the postnatal age.
For Infants > 108 h of postnatal age: 18 mg/kg/dose loading dose followed by 9 mg/kg/dose, two doses at q24 intervals, started 24 h after the loading dose.
Placebo will be sterile water, with similar volume and color as acetaminophen, will be given through the oro-gastric tube, for three days at 6 h intervals.
