Critère d'intégration:
- Poids corporel >= 5 kg au dépistage.
- Vaccination contre les sérotypes A, C, W et Y de Neisseria meningitis ; vaccination contre le sérotype B, selon les recommandations nationales de vaccination.
- Vaccination contre Haemophilus influenzae type B et Streptococcus pneumoniae, selon les recommandations nationales de vaccination.
- Pour les patients continuant à recevoir d'autres traitements en concomitance avec le crovalimab (par exemple, immunosuppresseurs, corticostéroïdes, mTORi ou inhibiteurs de la calcineurine) : dose stable pendant > = 28 jours avant le dépistage et jusqu'à la première administration de crovalimab.
- Pour les participantes en âge de procréer : un accord pour rester abstinent ou utiliser une contraception.
- Les participants ayant déjà subi une greffe de rein sont éligibles s'ils ont des antécédents connus de SHUa médié par le complément avant la greffe de rein.
- Début de la présentation initiale de la MAT dans les 28 jours précédant la première dose de crovalimab (pour la cohorte naïve uniquement).
- Traitement documenté avec eculizumab ou ravulizumab (pour Switch Cohort uniquement).
- Preuve clinique de réponse à un inhibiteur de C5 (pour Switch Cohort uniquement).
- TMA mal contrôlée après un traitement avec un autre inhibiteur C5 (uniquement pour les participants SNP C5 de la cohorte prétraitée).
- Polymorphisme C5 connu (uniquement pour les participants SNP C5 de la cohorte prétraitée).
Critère d'exclusion:
- MAT associée à une maladie rénale non liée au SHUa.
- Test de Combs direct positif.
- Dialyse chronique et/ou insuffisance rénale terminale.
- MAT identifiée liée à l'exposition au médicament.
- Présence ou antécédents d'une affection pouvant déclencher la MAT, telle qu'une tumeur maligne, une greffe de moelle osseuse ou d'organe (autre qu'une greffe de rein) ou une maladie auto-immune.
- Antécédents d'une maladie rénale autre que le SHUa.
- Antécédents d'infection à Neisseria meningitidis dans les 6 mois suivant l'inscription à l'étude.
- Déficit immunitaire connu ou suspecté (par exemple, antécédents d'infections récurrentes fréquentes).
- Test VIH positif.
- Infection bactérienne, virale ou fongique systémique active dans les 14 jours précédant la première administration de crovalimab.
- Présence de fièvre (>= 38oC) dans les 7 jours précédant la première administration de crovalimab.
- Dysfonctionnement ou défaillance d'organes multi-systèmes.
- Traitement IgIV récent.
- Enceinte ou allaitante ou ayant l'intention de devenir enceinte.
- Participation à une autre étude de traitement interventionnel avec un agent expérimental ou utilisation de toute thérapie expérimentale dans les 28 jours suivant le dépistage ou dans les cinq demi-vies de ce produit expérimental, selon la plus élevée.
- Utilisation récente d'acide tranexamique.
- Traitement actuel ou antérieur avec un inhibiteur du complément (pour la cohorte naïve uniquement).
- La première initiation des échanges plasmatiques/perfusions plasmatiques (EP/IP) ne doit pas avoir lieu plus de 28 jours avant la première administration de crovalimab (pour la cohorte naïve uniquement).
- La PE/IP ne doit pas être administrée dans les 6 heures suivant la première administration de crovalimab (uniquement pour la cohorte naïve).
- Recevoir de la PE/IP dans les 8 semaines suivant la première administration de crovalimab (switch cohort uniquement).
- Positif pour les infections actives à l'hépatite B et/ou C (VHB/VHC) (pour les participants de la cohorte Switch et de la cohorte prétraitée C5 SNP qui ont récemment reçu un traitement par inhibiteur C5).
- Cryoglobulinémie lors du dépistage (pour les participants de la cohorte Switch et de la cohorte SNP C5 qui ont récemment reçu un traitement par inhibiteur C5).
- Affection documentée conduisant à une MAT non-SHUa : Purpura thrombocytopénique thrombotique (PTT), Escherichia coli producteur de Shiga Toxine (STEC)-TMA, SHU pneumococcique, MAT secondaire à un défaut de cobalamine C et MAT liée à la néphropathie à la diacylglycérol kinase ε (DGKE).

Expérimental: Crovalimab
Les participants seront inscrits dans trois cohortes : [1] Cohorte naïve - participants qui n'ont pas été précédemment traités avec un traitement par inhibiteur du complément ; [2] Switch Cohort - participants qui passent au crovalimab à partir d'un autre inhibiteur C5 et [3] Cohort prétraité (comprend les participants C5 SNP (Single Nucleotide Polymorphism)) - participants qui ont reçu un traitement avec un autre inhibiteur C5 et l'ont ensuite interrompu.

Le crovalimab sera administré à la dose de 1 000 mg par voie intraveineuse (IV) (pour les participants pesant => 40 à = 100 kg) la semaine 1 jour 1.
La semaine 1, jour 2 et les semaines 2, 3 et 4, le crovalimab sera administré à la dose de 340 mg par voie sous-cutanée (SC).
À la semaine 5 et toutes les 4 semaines par la suite, il sera administré à la dose de 680 mg SC (pour les participants pesant => 40 à =100 kg).

Naïve Cohort: Percentage of Participants With Complete Thrombotic Microangiopathy Response (cTMAr) Anytime From Baseline to Week 25 (After 24 Weeks on Treatment)

cTMAr was defined by meeting all of the following criteria at two separate assessments, obtained at least 4 weeks (a minimum of 26 days) apart, and any measurement in between: Platelet count ≥ lower limit of normal (LLN); Lactate dehydrogenase (LDH) ≤ upper limit of normal (ULN); and ≥ 25% decrease in serum creatinine from baseline.
Participants with normalized creatinine (i.e., LLN ≤ creatinine ≤ ULN) at baseline were excluded from analysis.
95% confidence interval (CI) was calculated using Wilson's score method.
