Inclusiecriteria:
- Lichaamsgewicht >= 5 kg bij screening.
- Vaccinatie tegen Neisseria meningitis serotypes A, C, W en Y; vaccinatie tegen serotype B, volgens de nationale vaccinatieaanbevelingen.
- Vaccinatie tegen Haemophilus influenzae type B en Streptococcus pneumoniae, volgens de landelijke vaccinatieadviezen.
- Voor patiënten die gelijktijdig met crovalimab andere therapieën blijven krijgen (bijv. immunosuppressiva, corticosteroïden, mTORi of calcineurineremmers): stabiele dosis gedurende >=28 dagen voorafgaand aan de screening en tot aan de eerste toediening van crovalimab.
- Voor vrouwelijke deelnemers in de vruchtbare leeftijd: een afspraak om onthouding te blijven of anticonceptie te gebruiken.
- Deelnemers met een eerdere niertransplantatie komen in aanmerking als ze voorafgaand aan de niertransplantatie een voorgeschiedenis hebben van complement-gemedieerde aHUS.
- Aanvang van de initiële TMA-presentatie binnen 28 dagen voorafgaand aan de eerste dosis crovalimab (alleen voor Naive Cohort).
- Gedocumenteerde behandeling met eculizumab of ravulizumab (alleen voor Switch Cohort).
- Klinisch bewijs van respons op een C5-remmer (alleen voor Switch Cohort).
- Slecht gecontroleerde TMA na behandeling met een andere C5-remmer (alleen voor C5 SNP-deelnemers in het voorbehandelde cohort).
- Bekend C5-polymorfisme (alleen voor C5 SNP-deelnemers in het voorbehandelde cohort).
Uitsluitingscriteria:
- TMA geassocieerd met niet-aHUS-gerelateerde nierziekte.
- Positieve directe Coombs-test.
- Chronische dialyse en/of nierziekte in het eindstadium.
- Geïdentificeerde drugsblootstellingsgerelateerde TMA.
- Aanwezigheid of geschiedenis van een aandoening die TMA zou kunnen veroorzaken, zoals maligniteit, beenmerg- of orgaantransplantatie (anders dan niertransplantatie) of auto-immuunziekte.
- Geschiedenis van een nierziekte, anders dan aHUS.
- Geschiedenis van infectie met Neisseria meningitidis binnen 6 maanden na inschrijving in het onderzoek.
- Bekende of vermoede immuundeficiëntie (bijv. geschiedenis van frequent terugkerende infecties).
- Positieve hiv-test.
- Actieve systemische bacteriële, virale of schimmelinfectie binnen 14 dagen vóór de eerste toediening van crovalimab.
- Aanwezigheid van koorts (>= 38°C) binnen 7 dagen vóór de eerste toediening van crovalimab.
- Disfunctioneren of falen van meerdere systemen.
- Recente IVIg-behandeling.
- Zwanger bent of borstvoeding geeft of van plan bent zwanger te worden.
- Deelname aan een andere interventionele behandelingsstudie met een onderzoeksmiddel of gebruik van een experimentele therapie binnen 28 dagen na screening of binnen vijf halfwaardetijden van dat onderzoeksproduct, afhankelijk van welke van de twee groter is.
- Recent gebruik van tranexaminezuur.
- Huidige of eerdere behandeling met een complementremmer (alleen voor Naive Cohort).
- De eerste start van plasma-uitwisseling/plasma-infusies (PE/PI) mag niet meer dan 28 dagen vóór de eerste toediening van crovalimab plaatsvinden (alleen voor Naive Cohort).
- PE/PI mag niet binnen 6 uur na de eerste toediening van crovalimab worden toegediend (alleen voor Naive Cohort).
- PE/PI krijgen binnen 8 weken na de eerste toediening van crovalimab (alleen Switch Cohort).
- Positief voor actieve hepatitis B- en/of C-infecties (HBV/HCV) (voor Switch Cohort en switchende C5 SNP Pretreated Cohort-deelnemers die onlangs een behandeling met C5-remmers hebben gekregen).
- Cryoglobulinemie bij screening (voor deelnemers aan het Switch Cohort en C5 SNP Cohort die onlangs een behandeling met C5-remmers hebben ondergaan).
- Gedocumenteerde aandoening die leidt tot non-aHUS TMA: trombotische trombocytopenische purpura (TTP), shigatoxineproducerende Escherichia Coli (STEC)-TMA, pneumokokken-HUS, TMA secundair aan cobalamine C-defect en TMA gerelateerd aan diacylglycerolkinase ε (DGKE)-nefropathie.

Experimenteel: Crovalimab
Deelnemers worden ingeschreven in drie cohorten: [1] Naïef cohort - deelnemers die niet eerder zijn behandeld met complementremmertherapie; [2] Switch Cohort - deelnemers die van een andere C5-remmer overstappen op crovalimab en [3] Voorbehandeld cohort (inclusief C5 SNP-deelnemers (Single Nucleotide Polymorphism)) - deelnemers die een behandeling met een andere C5-remmer kregen en deze vervolgens stopten.

Crovalimab wordt toegediend in een dosis van 1000 mg intraveneus (IV) (voor deelnemers die => 40 tot =100 kg wegen) op week 1, dag 1.
Op week 1, dag 2 en in week 2, 3 en 4 wordt crovalimab toegediend in een dosis van 340 mg subcutaan (SC).
In week 5 en Q4W daarna wordt het toegediend in een dosis van 680 mg SC (voor deelnemers die => 40 tot =100 kg wegen).
