Criterio di inclusione:
- Peso corporeo >= 5 kg allo screening.
- Vaccinazione contro Neisseria meningite sierotipi A, C, W e Y; vaccinazione contro il sierotipo B, secondo le raccomandazioni vaccinali nazionali.
- Vaccinazione contro Haemophilus influenzae di tipo B e Streptococcus pneumoniae, secondo le raccomandazioni vaccinali nazionali.
- Per i pazienti che continuano a ricevere altre terapie in concomitanza con crovalimab (ad es. immunosoppressori, corticosteroidi, mTORi o inibitori della calcineurina): dose stabile per >=28 giorni prima dello screening e fino alla prima somministrazione di crovalimab.
- Per le partecipanti di sesso femminile in età fertile: un accordo per rimanere astinenti o usare la contraccezione.
- I partecipanti con un precedente trapianto di rene sono idonei se hanno una storia nota di SEUa mediata dal complemento prima del trapianto di rene.
- Insorgenza della presentazione iniziale di TMA entro 28 giorni prima della prima dose di crovalimab (solo per Naive Cohort).
- Trattamento documentato con eculizumab o ravulizumab (solo per Switch Cohort).
- Evidenza clinica di risposta a un inibitore C5 (solo per Switch Cohort).
- TMA scarsamente controllata dopo il trattamento con un altro inibitore C5 (solo per i partecipanti SNP C5 nella coorte pretrattata).
- Polimorfismo C5 noto (solo per i partecipanti SNP C5 nella coorte pretrattata).
Criteri di esclusione:
- TMA associata a malattia renale non correlata a SEUa.
- Test di Coombs diretto positivo.
- Dialisi cronica e/o malattia renale allo stadio terminale.
- TMA identificato correlato all'esposizione al farmaco.
- Presenza o storia di una condizione che potrebbe scatenare la TMA, come tumore maligno, midollo osseo o trapianto di organi (diverso dal trapianto di rene) o malattia autoimmune.
- Storia di una malattia renale, diversa dalla SEUa.
- Storia di infezione da Neisseria meningitidis entro 6 mesi dall'arruolamento nello studio.
- Immunodeficienza nota o sospetta (ad es. anamnesi di frequenti infezioni ricorrenti).
- Test HIV positivo.
- Infezione batterica, virale o fungina sistemica attiva entro 14 giorni prima della prima somministrazione di crovalimab.
- Presenza di febbre (>= 38°C) entro 7 giorni prima della prima somministrazione di crovalimab.
- Disfunzione o insufficienza multisistemica degli organi.
- Recente trattamento con IVIg.
- Incinta o allattamento o intenzione di rimanere incinta.
- - Partecipazione a un altro studio di trattamento interventistico con un agente sperimentale o uso di qualsiasi terapia sperimentale entro 28 giorni dallo screening o entro cinque emivite di quel prodotto sperimentale, a seconda di quale sia maggiore.
- Uso recente dell'acido tranexamico.
- Trattamento attuale o precedente con un inibitore del complemento (solo per Naive Cohort).
- Il primo inizio della plasmaferesi/infusioni di plasma (PE/PI) non deve avvenire più di 28 giorni prima della prima somministrazione di crovalimab (solo per la coorte naive).
- PE/PI non deve essere somministrato entro 6 ore dalla prima somministrazione di crovalimab (solo per Naive Cohort).
- Ricezione di PE/PI entro 8 settimane dalla prima somministrazione di crovalimab (solo cambio di coorte).
- Positivo per infezioni attive da epatite B e/o C (HBV/HCV) (per i partecipanti della coorte di cambio e della coorte pretrattata C5 SNP che hanno recentemente ricevuto un trattamento con inibitore C5).
- Crioglobulinemia allo screening (per i partecipanti alla coorte Switch e alla coorte C5 SNP che hanno recentemente ricevuto un trattamento con inibitori C5).
- Condizione documentata che porta a TMA non aHUS: porpora trombotica trombocitopenica (TTP), Escherichia Coli produttore di tossina Shiga (STEC)-TMA, SEU pneumococcica, TMA secondaria a difetto della cobalamina C e TMA correlata alla nefropatia da diacilglicerolo chinasi ε (DGKE).

Sperimentale: Crovalimab
I partecipanti saranno arruolati in tre coorti: [1] Coorte naive - partecipanti che non sono stati precedentemente trattati con terapia con inibitori del complemento; [2] Switch Cohort - partecipanti che passano a crovalimab da un altro inibitore C5 e [3] Coorte pretrattata (include i partecipanti C5 SNP (Single Nucleotide Polymorphism)) - partecipanti che hanno ricevuto il trattamento con un altro inibitore C5 e successivamente lo hanno interrotto.

Crovalimab verrà somministrato alla dose di 1000 mg per via endovenosa (IV) (per i partecipanti che pesano => da 40 a = 100 kg) il giorno 1 della settimana 1.
Nella settimana 1, nel giorno 2 e nelle settimane 2, 3 e 4, crovalimab verrà somministrato alla dose di 340 mg per via sottocutanea (SC).
Nella settimana 5 e successivamente nella quarta settimana, verrà somministrato una dose di 680 mg SC (per partecipanti di peso => da 40 a = 100 kg).

Naïve Cohort: Percentage of Participants With Complete Thrombotic Microangiopathy Response (cTMAr) Anytime From Baseline to Week 25 (After 24 Weeks on Treatment)

cTMAr was defined by meeting all of the following criteria at two separate assessments, obtained at least 4 weeks (a minimum of 26 days) apart, and any measurement in between: Platelet count ≥ lower limit of normal (LLN); Lactate dehydrogenase (LDH) ≤ upper limit of normal (ULN); and ≥ 25% decrease in serum creatinine from baseline.
Participants with normalized creatinine (i.e., LLN ≤ creatinine ≤ ULN) at baseline were excluded from analysis.
95% confidence interval (CI) was calculated using Wilson's score method.

Crovalimab and eculizumab bind distinct epitopes on complement component C5, and formation of DTDCs comprising crovalimab, eculizumab, and C5 are formed when participants switch treatment.
