Einschlusskriterien:

  • Körpergewicht >= 5 kg beim Screening.
  • Impfung gegen Neisseria-Meningitis Serotypen A, C, W und Y; Impfung gegen Serotyp B gemäß den nationalen Impfempfehlungen.
  • Impfung gegen Haemophilus influenzae Typ B und Streptococcus pneumoniae gemäß den nationalen Impfempfehlungen.
  • Für Patienten, die weiterhin andere Therapien gleichzeitig mit Crovalimab erhalten (z. B. Immunsuppressiva, Kortikosteroide, mTORi oder Calcineurin-Inhibitoren): stabile Dosis für >= 28 Tage vor dem Screening und bis zur ersten Crovalimab-Gabe.
  • Für weibliche Teilnehmer im gebärfähigen Alter: eine Vereinbarung, abstinent zu bleiben oder Verhütungsmittel zu verwenden.
  • Teilnehmer mit einer vorherigen Nierentransplantation sind teilnahmeberechtigt, wenn sie vor der Nierentransplantation eine bekannte Vorgeschichte von komplementvermitteltem aHUS haben.
  • Beginn der anfänglichen TMA-Darstellung innerhalb von 28 Tagen vor der ersten Crovalimab-Dosis (nur für die naive Kohorte).
  • Dokumentierte Behandlung mit entweder Eculizumab oder Ravulizumab (nur für Switch-Kohorte).

Ausschlusskriterien:

  • TMA im Zusammenhang mit einer nicht aHUS-assoziierten Nierenerkrankung.
  • Positiver direkter Coombs-Test.
  • Chronische Dialyse und/oder Nierenerkrankung im Endstadium.
  • Identifizierte TMA im Zusammenhang mit der Arzneimittelexposition.
  • Vorhandensein oder Vorgeschichte eines Zustands, der TMA auslösen könnte, wie z. B. Malignität, Knochenmark- oder Organtransplantation (außer Nierentransplantation) oder Autoimmunerkrankung.
  • Geschichte einer Nierenerkrankung, außer aHUS.
  • Vorgeschichte einer Neisseria meningitidis-Infektion innerhalb von 6 Monaten nach Studieneinschreibung.
  • Bekannter oder vermuteter Immundefekt (z. B. häufig wiederkehrende Infektionen in der Vorgeschichte).
  • Positiver HIV-Test.

  • Kürzliche Anwendung von Tranexamsäure.
  • Aktuelle oder frühere Behandlung mit einem Komplementinhibitor (nur für naive Kohorte).
  • Der erstmalige Beginn des Plasmaaustauschs/Plasmainfusionen (PE/PI) sollte nicht mehr als 28 Tage vor der ersten Crovalimab-Verabreichung erfolgen (nur für die naive Kohorte).

Experimental: Crovalimab
Die Teilnehmer werden in drei Kohorten eingeschrieben: [1] Naive Kohorte – Teilnehmer, die zuvor nicht mit einer Komplement-Inhibitor-Therapie behandelt wurden; [2] Wechselkohorte – Teilnehmer, die von einem anderen C5-Inhibitor auf Crovalimab wechseln und [3] vorbehandelte Kohorte (einschließlich C5-SNP-Teilnehmer (Single Nucleotide Polymorphism)) – Teilnehmer, die eine Behandlung mit einem anderen C5-Inhibitor erhielten und diese anschließend abbrachen.
Crovalimab wird in einer Dosis von 1000 mg intravenös (IV) (für Teilnehmer mit einem Gewicht von => 40 bis = 100 kg) am ersten Tag der Woche 1 verabreicht.
