Une étude de phase IIa, randomisée, parallèle, en double aveugle et contrôlée par placebo pour évaluer l'efficacité et l'innocuité d'Enpatoran chez les participants atteints de dermatomyosite et de polymyosite recevant des soins standard (NEPTUNIA)
Le but de cette étude est d'évaluer l'efficacité et l'innocuité du M5049 administré par voie orale dans les myopathies inflammatoires idiopathiques, en particulier les participants à la dermatomyosite (DM) et à la polymyosite (PM) pendant 24 semaines.

Critère d'intégration:
- Diagnostic de DM ou MP probable ou certain selon les critères de classification ACR/EULAR 2017, avec un statut auto-anticorps positif. Les participants atteints de PM doivent remplir les critères de classification avec une biopsie musculaire. Les participants au syndrome anti-synthétase (ASyS) qui répondent aux critères de classification sont autorisés
- La maladie active selon la norme de soins (SoC) doit répondre à l'un des critères dans les 6 mois précédant le dépistage : Preuve pathologique de myosite active dans la biopsie musculaire ; Preuve de myosite active par électromyographie (EMG); Imagerie par résonance magnétique (IRM) avec preuve de myosite active ; ou toute enzyme musculaire supérieure ou égale à (>=) 4 × limite supérieure de la normale (LSN) au moment du dépistage ; Éruption cutanée active PM / DM selon la zone de dermatomyosite cutanée et l'indice de gravité-A (CDASI-A)> = 7 au moment du dépistage
- Sévérité minimale de la maladie définie par : myopathie modérée à sévère avec test musculaire manuel 8 (MMT-8) > 80 et inférieure ou égale à (= 2 centimètres (cm) ; Activité globale du médecin (PGA) dérivée de l'évaluation de l'activité de la maladie liée à la myosite (MDAAT) >= 2 cm ; évaluation de l'activité extramusculaire dérivée de la MDAAT >= 2 cm ; au moins une enzyme musculaire > 1,5 fois la LSN ; questionnaire d'évaluation de la santé - indice d'incapacité (HAQ-DI) >= 0,25 OU éruption cutanée modérée à sévère et myopathie légère avec MMT-8 entre 137 et = 14 ET au moins 2 des anomalies CSM suivantes : PtGA >= 2 cm ; PGA dérivé de MDAAT >= 2 cm ; Évaluation de l'activité extramusculaire dérivée de MDAAT >= 2 cm ; Au moins une enzyme musculaire > 1,5 fois LSN ; HAQ-DI >= 0,25
- Doses stables de corticostéroïdes oraux (CS) et/ou maximum de 1 médicament immunosuppresseur/immunomodulateur non corticostéroïde (méthotrexate, 6 mercaptopurine, sulfasalazine, mycophénolate mofétil ou sodium, azathioprine, léflunomide, cyclosporine, tacrolimus oral) pour le DM ou le PM
- Les participants ont un indice de masse corporelle (IMC) compris entre 18,5 et 35,0 kilogrammes par mètre carré (kg/m^2) (inclus)
- D'autres critères d'inclusion définis par le protocole pourraient s'appliquer
Critère d'exclusion:
- Diagnostic principal de myosite à corps d'inclusion (IBM), myosite associée à une tumeur maligne (définie comme un diagnostic de myosite dans les 3 ans suivant le cancer), myopathie nécrosante à médiation immunitaire (IMNM) avec une biopsie caractérisée comme une biopsie nécrosante ou IMNM avec une particule de reconnaissance anti-signal positive (SRP) ou auto-anticorps anti-3-hydroxy-3-méthylglutaryl-coenzyme A réductase (HMGCR). Les participants avec un anticorps gamma anti-facteur intermédiaire de transcription 1 (TIF1) ou un anticorps anti MDAT5 nouvellement diagnostiqué (dans un délai d'un an) doivent avoir subi un dépistage adéquat du cancer dans les 12 mois suivant le jour 1. Un dépistage adéquat du cancer est défini comme étant à jour dépistage adapté à l'âge et au sexe conformément aux directives nationales
- Diagnostic principal de DM juvénile ou participants adultes ayant déjà reçu un diagnostic de DM juvénile
- Toute autre maladie concomitante active du tissu conjonctif associée à une myopathie inflammatoire de l'avis de l'investigateur. L'admissibilité des participants ayant reçu un diagnostic de maladie(s) du tissu conjonctif concomitante sera examinée et approuvée par un comité d'experts sur les myopathies inflammatoires idiopathiques (IIM)
- Pneumopathie interstitielle sévère définie comme un supplément d'oxygène nécessaire au repos ou une capacité vitale forcée (CVF) de
- Toute maladie non contrôlée (par exemple [par exemple], respiratoire grave, cardiovasculaire, gastro-intestinale, neurologique, psychiatrique, hématologique, métabolique [y compris thyroïdite avec augmentation/diminution de la thyréostimuline (TSH)], rénale, hépatique, maladie endocrinienne/des organes reproducteurs) autre que DM / PM, qui, de l'avis de l'investigateur ou du sponsor / représentant, constitue un risque inapproprié ou une contre-indication à la participation à l'étude ou qui pourrait interférer avec les objectifs, la conduite ou l'évaluation de l'étude

Serious AE was defined AE that resulted in any of the following outcomes: death; life threatening; persistent/significant disability/incapacity; initial/prolonged inpatient hospitalization; congenital anomaly/birth defect.
TEAE was defined as events with onset date or worsening during the on-treatment period.

Extra-muscular activity ranged between 0 and 10, where, 0 cm = absent and 10 cm = maximum disease activity.

MDAAT was a combined tool capturing physician's assessment of disease activity of various organ systems- constitutional, cutaneous, pulmonary, gastrointestinal, joints and cardiovascular, muscular using: Myositis Intention to Treat Activity Index (MITAX), a scale from 0("Not present in the last 4 weeks") to 4("New - in the last 4 weeks [compared to the previous 4 weeks]") and Myositis Disease Activity Assessment Visual Analogue Scales (MYOACT), a 10 cm VAS assessing disease activity in the last 4 weeks of various organ systems from 0 cm("No evidence of disease activity") to 10 cm("Extremely active or severe disease activity"), disease activity being defined as potentially reversible pathology or physiology resulting from the myositis.
Left end of line = no evidence of disease activity, midpoint of line = moderate disease activity, and right end of line = extreme or maximum disease activity.
An assessment of extramuscular activity assessment was taken from MDAAT using a 10cm VAS scale.

DBPC Period: Change From Baseline in Manual Muscle Testing-8 (MMT-8) Score

MMT-8 was an International Myositis Assessment and Clinical Studies (IMACS) Core Set Measure used in the calculation of the IMACS Definition of Improvement (DOI) and ACR/EULAR TIS.
It is a set of 8 designated muscles tested unilaterally (generally on right side) -which include 1 axial, 5 proximal (2 upper extremity, 3 lower extremity), and 2 distal muscles (1 upper, 1 lower extremity).
Total score for unilateral testing ranges from 0 to 80, and bilateral testing ranges from 0 to 150 with normal strength represented by a higher score at or near the top of the scale.
The total score for bilateral testing (item "MMT8 score (0 - 150)" from the MANUAL MUSCLE TESTING-8 (mmt8) form) is used in the derivation of the Total Improvement Score.

HAQ-DI was an IMACS Core Set Measure used in the calculation of the IMACS DOI and ACR/EULAR TIS.
It is a generic rather than a disease-specific instrument comprised of 8 sections (dressing, arising, eating, walking, hygiene, reach, grip, and activities), with 2 to 3 questions per section.
Scoring within each section is from 0 (without any difficulty) to 3 (unable to do).
For each section the score given to that section is the worst score within the section.
The 8 scores of the 8 sections are summed and divided by 8. Overall score ranges from 0-3.

For each participant, the most abnormal muscle-associated enzyme was defined as the baseline muscle-associated enzyme with the highest ratio of the observed value to the upper limit of the normal range (ULN) among creatinine kinase, lactate dehydrogenase, aspartate aminotransferase, alanine aminotransferase, and aldolase.
The relative abnormality could be expressed as a percentage of ULN, calculated as (enzyme value divided by ULN) multiplied by 100.

CDASI is a clinician-scored single page instrument that separately measures activity and damage in the skin of DM participants.
CDASI has 3 activity measures (CDASI-A: erythema, scale, and erosion/ulceration) and 2 damage measures (CDASI-D: poikiloderma and calcinosis) which are assessed over 15 body areas.
In addition, Gottron's papules on the hands, periungual changes and alopecia are evaluated separately both for activity and damage.
Activity and Damage Subscale scores range from 0 to 100 and 0 to 32, respectively, where higher scores indicate greater disease severity.
