Een fase IIa, gerandomiseerde, parallelle, dubbelblinde, placebogecontroleerde studie om de werkzaamheid en veiligheid van enpatoran te evalueren bij deelnemers aan dermatomyositis en polymyositis die standaardzorg ontvangen (NEPTUNIA)
Het doel van deze studie is het evalueren van de werkzaamheid en veiligheid van oraal toegediende M5049 bij idiopathische inflammatoire myopathieën, met name dermatomyositis (DM) en polymyositis (PM) deelnemers gedurende 24 weken.

Inclusiecriteria:
- Diagnose van waarschijnlijke of definitieve DM of PM volgens de ACR/EULAR-classificatiecriteria van 2017, met positieve auto-antilichaamstatus. Deelnemers met PM moeten voldoen aan classificatiecriteria met spierbiopsie. Deelnemers aan het antisynthetasesyndroom (ASyS) die aan de classificatiecriteria voldoen, zijn toegestaan
- Actieve ziekte volgens standaardzorg (SoC), moet binnen 6 maanden voorafgaand aan de screening aan een van de criteria voldoen: Pathologisch bewijs van actieve myositis in spierbiopsie; Bewijs van actieve myositis door elektromyografie (EMG); Magnetic resonance imaging (MRI) met bewijs van actieve myositis; of een spierenzym groter dan of gelijk aan (>=) 4 × bovengrens van normaal (ULN) op het moment van screening; Actieve PM/DM-huiduitslag volgens cutane dermatomyositis-zone en ernstindex-A (CDASI-A) >= 7 op het moment van screening
- Minimale ziekte-ernst gedefinieerd door: matige tot ernstige myopathie met manuele spiertest-8 (MMT-8) >= 80 en minder dan of gelijk aan (= 2 centimeter (cm); Physician Global Activity (PGA) afgeleid van myositis ziekteactiviteitsbeoordeling hulpmiddel (MDAAT) >= 2 cm; Extramuscular Activity Assessment afgeleid van MDAAT >= 2 cm; Ten minste één spierenzym > 1,5 keer ULN; gezondheidsbeoordeling vragenlijst-disability index (HAQ-DI) >= 0,25 OF matige tot ernstige huiduitslag en milde myopathie met MMT-8 tussen 137 en = 14 EN ten minste 2 van de volgende CSM-afwijkingen: PtGA >= 2 cm; PGA afgeleid van MDAAT >= 2 cm; Extramuscular Activity Assessment afgeleid van MDAAT >= 2 cm; Ten minste één spierenzym > 1,5 keer ULN, HAQ-DI >= 0,25
- Stabiele doses orale corticosteroïden (CS) en/of maximaal 1 niet-corticosteroïde immunosuppressieve/immunomodulerende medicatie (methotrexaat, 6 mercaptopurine, sulfasalazine, mycofenolaatmofetil of natrium, azathioprine, leflunomide, ciclosporine, oraal tacrolimus) voor DM of PM
- Deelnemers hebben een body mass index (BMI) in het bereik van 18,5 tot 35,0 kilogram per vierkante meter (kg/m^2) (inclusief)
- Andere in het protocol gedefinieerde opnamecriteria kunnen van toepassing zijn
Uitsluitingscriteria:
- Primaire diagnose van inclusielichaammyositis (IBM), maligniteit-geassocieerde myositis (gedefinieerd als diagnose van myositis binnen 3 jaar na kanker), immuungemedieerde necrotiserende myopathie (IMNM) met een biopsie gekenmerkt als necrotiserende biopsie of IMNM met positief antisignaalherkenningsdeeltje antilichaam (SRP) of anti-3-hydroxy-3-methylglutaryl-co-enzym A-reductase (HMGCR) auto-antilichamen. Deelnemers met anti-transcriptie intermediair factor 1 (TIF1) gamma-antilichaam of nieuw gediagnosticeerd (binnen 1 jaar) anti-MDAT5-antilichaam moeten binnen 12 maanden na dag 1 adequaat zijn gescreend op kanker. Adequate screening op kanker wordt gedefinieerd als up-to-date leeftijds- en geslachtsgeschikte screening volgens nationale richtlijnen
- Primaire diagnose van juveniele DM, of volwassen deelnemers die eerder gediagnosticeerd waren met juveniele DM
- Elke andere gelijktijdige actieve bindweefselziekte geassocieerd met inflammatoire myopathie volgens de mening van de onderzoeker. Geschiktheid van deelnemers met de diagnose gelijktijdige bindweefselziekte(n) zal worden beoordeeld en goedgekeurd door een idiopathische inflammatoire myopathieën (IIM)-expertcommissie
- Ernstige interstitiële longziekte gedefinieerd als extra zuurstof die nodig is in rust, of geforceerde vitale capaciteit (FVC) van
- Elke ongecontroleerde ziekte (bijvoorbeeld [bijv.] ernstige respiratoire, cardiovasculaire, gastro-intestinale, neurologische, psychiatrische, hematologische, metabolische [waaronder thyroïditis met verhoogd/verlaagd thyroïdstimulerend hormoon (TSH)], nier-, lever-, endocriene/voortplantingsorgaanziekte) andere dan DM/PM, dat naar de mening van de Onderzoeker of Sponsor/aangewezene een ongepast risico of contra-indicatie vormt voor deelname aan het onderzoek of dat de doelstellingen, het verloop of de evaluatie van het onderzoek zou kunnen verstoren
- Andere in het protocol gedefinieerde uitsluitingscriteria kunnen van toepassing zijn

MDAAT was a combined tool capturing physician's assessment of disease activity of various organ systems- constitutional, cutaneous, pulmonary, gastrointestinal, joints and cardiovascular, muscular using: Myositis Intention to Treat Activity Index (MITAX), a scale from 0("Not present in the last 4 weeks") to 4("New - in the last 4 weeks [compared to the previous 4 weeks]") and Myositis Disease Activity Assessment Visual Analogue Scales (MYOACT), a 10 cm VAS assessing disease activity in the last 4 weeks of various organ systems from 0 cm("No evidence of disease activity") to 10 cm("Extremely active or severe disease activity"), disease activity being defined as potentially reversible pathology or physiology resulting from the myositis.
Left end of line = no evidence of disease activity, midpoint of line = moderate disease activity, and right end of line = extreme or maximum disease activity.
An assessment of extramuscular activity assessment was taken from MDAAT using a 10cm VAS scale.

MMT-8 was an International Myositis Assessment and Clinical Studies (IMACS) Core Set Measure used in the calculation of the IMACS Definition of Improvement (DOI) and ACR/EULAR TIS.
It is a set of 8 designated muscles tested unilaterally (generally on right side) -which include 1 axial, 5 proximal (2 upper extremity, 3 lower extremity), and 2 distal muscles (1 upper, 1 lower extremity).
Total score for unilateral testing ranges from 0 to 80, and bilateral testing ranges from 0 to 150 with normal strength represented by a higher score at or near the top of the scale.
The total score for bilateral testing (item "MMT8 score (0 - 150)" from the MANUAL MUSCLE TESTING-8 (mmt8) form) is used in the derivation of the Total Improvement Score.

HAQ-DI was an IMACS Core Set Measure used in the calculation of the IMACS DOI and ACR/EULAR TIS.
It is a generic rather than a disease-specific instrument comprised of 8 sections (dressing, arising, eating, walking, hygiene, reach, grip, and activities), with 2 to 3 questions per section.
Scoring within each section is from 0 (without any difficulty) to 3 (unable to do).
For each section the score given to that section is the worst score within the section.
The 8 scores of the 8 sections are summed and divided by 8. Overall score ranges from 0-3.

For each participant, the most abnormal muscle-associated enzyme was defined as the baseline muscle-associated enzyme with the highest ratio of the observed value to the upper limit of the normal range (ULN) among creatinine kinase, lactate dehydrogenase, aspartate aminotransferase, alanine aminotransferase, and aldolase.
The relative abnormality could be expressed as a percentage of ULN, calculated as (enzyme value divided by ULN) multiplied by 100.

CDASI is a clinician-scored single page instrument that separately measures activity and damage in the skin of DM participants.
CDASI has 3 activity measures (CDASI-A: erythema, scale, and erosion/ulceration) and 2 damage measures (CDASI-D: poikiloderma and calcinosis) which are assessed over 15 body areas.
In addition, Gottron's papules on the hands, periungual changes and alopecia are evaluated separately both for activity and damage.
Activity and Damage Subscale scores range from 0 to 100 and 0 to 32, respectively, where higher scores indicate greater disease severity.

We zetten ons in voor het verbeteren van de volksgezondheid door op verantwoorde wijze gegevens uit klinische onderzoeken te delen.
Na goedkeuring van een nieuw product of een nieuwe indicatie voor een goedgekeurd product in zowel de VS als de Europese Unie, zullen de studiesponsor en/of zijn gelieerde bedrijven onderzoeksprotocollen, geanonimiseerde patiëntgegevens en gegevens op studieniveau en geredigeerde klinische onderzoeksrapporten delen met gekwalificeerde wetenschappelijke en medische onderzoekers, op verzoek, voor zover nodig voor het uitvoeren van legitiem onderzoek.

Binnen zes maanden na goedkeuring van een nieuw product of een nieuwe indicatie voor een goedgekeurd product in zowel de Verenigde Staten als de Europese Unie
IPD-toegangscriteria voor delen
Gekwalificeerde wetenschappelijke en medische onderzoekers kunnen de gegevens opvragen.
Dergelijke verzoeken moeten schriftelijk worden ingediend bij de portal van het bedrijf en zullen intern worden beoordeeld op criteria voor de kwalificatie van onderzoekers en de legitimiteit van het onderzoeksvoorstel.
