Stratégie de surveillance neurologique multimodale axée sur la perfusion cérébrale et l'oxygénation pour les patients présentant un arrêt cardiaque réfractaire hors de l'hôpital (OHCA)
Les lésions neurologiques restent une cause importante de morbidité et de mortalité chez les patients atteints d'ECPR. À l'heure actuelle, les résultats de trois études prospectives randomisées et contrôlées sur l'ECPR sont incohérents et il n'est pas concluant que l'ECPR puisse améliorer les résultats neurologiques des patients présentant un arrêt cardiaque réfractaire. Plusieurs études ont montré que les non-survivants de l'oxygénation extracorporelle par membrane peuvent entraîner des lésions cérébrales aiguës. Des recherches supplémentaires avec une surveillance neurologique systématique sont nécessaires pour définir le moment de la lésion cérébrale aiguë chez les patients soumis à une oxygénation extracorporelle par membrane. pas facile à détecter à temps en raison de l’utilisation d’analgésiques, de sédatifs et de relaxants musculaires. Étonnamment, peu d’attention a été accordée au rôle de la perfusion cérébrale et de l’oxygénation. De plus, les caractéristiques de la physiopathologie cérébrovasculaire et les stratégies de prise en charge optimales sont encore vagues.
Par conséquent, la neurosurveillance multimodale peut être un outil précieux pour détecter les lésions cérébrales chez les patients soumis à une oxygénation extracorporelle par membrane et fournir des conseils d'intervention précoce.
Le but de cette étude est de tester si la neurosurveillance multimodale améliorera la survie à 30 jours avec une issue neurologique favorable chez les patients ECPR atteints d'un OHCA réfractaire.

- Présumé ou connu comme étant âgé de 18 à 75 ans
- Témoin de l'OHCA
- Présentant initialement une FV/TV ou ayant reçu un choc de DEA
- RCR pour les spectateurs
- Le temps d'absence d'écoulement (temps entre l'AC et la RCP) était inférieur à 5 minutes.
- Ne parvient pas à atteindre un ROSC soutenu dans les 15 minutes
Critères d'exclusion :
- ROSC avec récupération hémodynamique soutenue dans les 15 minutes
- Insuffisance cardiaque terminale (NYHA III ou IV), maladie pulmonaire grave (MPOC Gold III ou IV), maladie oncologique,
- Grossesse
- Pontage fémoral bilatéral
- Score de catégorie de performance cérébrale (CPC) avant l'arrestation de 3 ou 4
- Traumatismes multiples (Injury Severity Score> 15)
- On estime que la canulation commencera 90 minutes après l'arrestation initiale.

Comparateur actif: control
Standard monitoring (including vital signs monitoring, blood gas analysis, and lactate levels) based on ECPR, along with continuous cerebral oxygenation monitoring (blinded to investigators, with no clinical interventions according to the results).
Clinical interventions are strictly guided by the 2023 American Heart Association (AHA) Guidelines for Advanced Cardiovascular Life Support in Adults (hereinafter referred to as the 2023 AHA Guidelines), including regulating ECMO blood flow, the dose of vasoactive drugs (MAP ≥65 mmHg), mechanical ventilation parameters (SaO₂ 94-98%, PaCO₂ 35-45 mmHg), sedation and analgesia plans.
Concurrently, staged target temperature management is implemented, involving maintaining the core temperature between 32 and 37.5°C within 24 hours, initiating controlled rewarming at a rate of ≤0.1°C/h after 24 hours, and continuing to prevent fever (core temperature ≤37.5°C) within 72 hours.

Expérimental: Multimodal monitoring strategy
Experimental group: In addition to standard treatment in the control group, a multimodal monitoring system of brain function is integrated: continuous rSO2 monitoring with at least once-daily TCD for cerebral blood flow velocity monitoring, ONSD ultrasound measurement, and EEG monitoring.
Based on the cerebral oxygenation target-directed management strategies, intervention measures such as the ECMO blood flow, the dose of vasoactive drugs, mechanical ventilation parameters, target temperature management, sedation and analgesia plans, and antiepileptic drugs are dynamically adjusted to ultimately achieve the goal of brain oxygenation target value (rSO2) to 58%-68%.
