Multimodale neurologische monitoringstrategie na ontvangst van ECPR (TDMCO-ECPR)
Cerebrale perfusie en zuurstofvoorziening-georiënteerde multimodale neurologische monitoringstrategie voor patiënten met refractaire hartstilstand buiten het ziekenhuis (OHCA)
Neurologisch letsel blijft een belangrijke oorzaak van morbiditeit en mortaliteit bij patiënten met ECPR. Op dit moment zijn de resultaten van drie prospectieve gerandomiseerde gecontroleerde onderzoeken naar ECPR inconsistent, en het is niet doorslaggevend of ECPR de neurologische uitkomsten van patiënten met een refractaire hartstilstand kan verbeteren. Uit verschillende onderzoeken is gebleken dat niet-overlevenden van extracorporale membraanoxygenatie kunnen leiden tot acuut hersenletsel. Verder onderzoek met systematische neurologische monitoring is nodig om de timing van acuut hersenletsel te bepalen bij patiënten met extracorporale membraanoxygenatie. Bovendien wordt hersenletsel dat optreedt tijdens extracorporale membraanoxygenatietherapie voorkomen. niet gemakkelijk op tijd op te sporen vanwege het gebruik van pijnstillers, kalmerende middelen en spierverslappers. Verrassend genoeg is er weinig aandacht besteed aan de rol van cerebrale perfusie en oxygenatie. Bovendien zijn de kenmerken van de cerebrovasculaire pathofysiologie en de optimale behandelstrategieën nog steeds vaag.
Daarom kan multimodale neuromonitoring een waardevol hulpmiddel zijn voor het detecteren van hersenletsel bij patiënten met extracorporale membraanoxygenatie en voor het bieden van begeleiding bij vroegtijdige interventie.
Het doel van deze studie is om te testen of multimodale neuromonitoring de 30-dagen overleving met een gunstige neurologische uitkomst zal verbeteren bij ECPR-patiënten met een refractaire OHCA.

Inclusiecriteria:
- Vermoedelijk of bekend 18-75 jaar oud
- Getuige van OHCA
- In eerste instantie met VF/VT of die een AED-schok hebben gekregen
- Reanimatie door omstanders
- Geen enkele doorlooptijd (tijd van CA tot CPR) was minder dan 5 minuten
- Er wordt niet binnen 15 minuten een aanhoudende ROSC bereikt
Uitsluitingscriteria:
- ROSC met aanhoudend hemodynamisch herstel binnen 15 minuten
- Terminal hartfalen (NYHA III of IV), ernstige longziekte (COPD Gold III of IV), oncologische ziekte,
- Zwangerschap
- Bilaterale femorale bypassoperatie
- Cerebral Performance Category (CPC)-score vóór arrestatie van 3 of 4
- Meervoudig trauma (ernstscore van letsel> 15)
- Geschat wordt dat de canulatie 90 minuten na de eerste arrestatie zal beginnen.

Actieve vergelijker: control
Standard monitoring (including vital signs monitoring, blood gas analysis, and lactate levels) based on ECPR, along with continuous cerebral oxygenation monitoring (blinded to investigators, with no clinical interventions according to the results).
Clinical interventions are strictly guided by the 2023 American Heart Association (AHA) Guidelines for Advanced Cardiovascular Life Support in Adults (hereinafter referred to as the 2023 AHA Guidelines), including regulating ECMO blood flow, the dose of vasoactive drugs (MAP ≥65 mmHg), mechanical ventilation parameters (SaO₂ 94-98%, PaCO₂ 35-45 mmHg), sedation and analgesia plans.
Concurrently, staged target temperature management is implemented, involving maintaining the core temperature between 32 and 37.5°C within 24 hours, initiating controlled rewarming at a rate of ≤0.1°C/h after 24 hours, and continuing to prevent fever (core temperature ≤37.5°C) within 72 hours.

Experimenteel: Multimodal monitoring strategy
Experimental group: In addition to standard treatment in the control group, a multimodal monitoring system of brain function is integrated: continuous rSO2 monitoring with at least once-daily TCD for cerebral blood flow velocity monitoring, ONSD ultrasound measurement, and EEG monitoring.
Based on the cerebral oxygenation target-directed management strategies, intervention measures such as the ECMO blood flow, the dose of vasoactive drugs, mechanical ventilation parameters, target temperature management, sedation and analgesia plans, and antiepileptic drugs are dynamically adjusted to ultimately achieve the goal of brain oxygenation target value (rSO2) to 58%-68%.
