Multimodale neurologische Überwachungsstrategie nach Erhalt der ECPR (TDMCO-ECPR)
Auf zerebrale Perfusion und Oxygenierung ausgerichtete multimodale neurologische Überwachungsstrategie für Patienten mit refraktärem Herzstillstand außerhalb des Krankenhauses (OHCA)
Neurologische Verletzungen bleiben eine wichtige Ursache für Morbidität und Mortalität bei Patienten mit ECPR. Derzeit sind die Ergebnisse von drei prospektiven randomisierten kontrollierten Studien zu ECPR inkonsistent und es ist nicht schlüssig, ob ECPR die neurologischen Ergebnisse von Patienten mit refraktärem Herzstillstand verbessern kann. Mehrere Studien ergaben, dass Nichtüberlebende einer extrakorporalen Membranoxygenierung zu einer akuten Hirnschädigung führen können. Weitere Untersuchungen mit einer systematischen neurologischen Überwachung sind erforderlich, um den Zeitpunkt einer akuten Hirnschädigung bei Patienten mit extrakorporaler Membranoxygenierung zu bestimmen Aufgrund der Verwendung von Analgetika, Beruhigungsmitteln und Muskelrelaxantien ist es nicht einfach, sie rechtzeitig zu erkennen. Überraschenderweise wurde der Rolle der zerebralen Durchblutung und Sauerstoffversorgung bisher wenig Aufmerksamkeit geschenkt. Darüber hinaus sind die Merkmale der zerebrovaskulären Pathophysiologie und optimale Managementstrategien noch unklar.
Daher kann multimodales Neuromonitoring ein wertvolles Instrument zur Erkennung von Hirnverletzungen bei Patienten mit extrakorporaler Membranoxygenierung und zur frühzeitigen Interventionsberatung sein.
Ziel dieser Studie ist es zu testen, ob multimodales Neuromonitoring das 30-Tage-Überleben mit einem günstigen neurologischen Ergebnis bei ECPR-Patienten mit einem refraktären OHCA verbessert.

Beschreibung
Einschlusskriterien:
- Vermutlich oder bekanntermaßen 18–75 Jahre alt
- Zeuge der OHCA
- Sie stellen sich zunächst mit VF/VT vor oder haben einen AED-Schock erhalten
- HLW für Zuschauer
- Die Zeit ohne Fluss (Zeit von CA bis CPR) betrug weniger als 5 Minuten
- Es gelingt nicht, innerhalb von 15 Minuten einen dauerhaften ROSC zu erreichen
Ausschlusskriterien:
- ROSC mit anhaltender hämodynamischer Erholung innerhalb von 15 Minuten
- Herzinsuffizienz im Endstadium (NYHA III oder IV), schwere Lungenerkrankung (COPD Gold III oder IV), onkologische Erkrankung,
- Schwangerschaft
- Bilaterale femorale Bypass-Operation
- Vor dem Arrest: CPC-Wert (Cerebral Performance Category) von 3 oder 4
- Mehrfachtrauma (Injury Severity Score >15)
- Schätzungsweise beginnt die Kanülierung 90 Minuten nach dem ersten Stillstand.

Standard monitoring (including vital signs monitoring, blood gas analysis, and lactate levels) based on ECPR, along with continuous cerebral oxygenation monitoring (blinded to investigators, with no clinical interventions according to the results).
Clinical interventions are strictly guided by the 2023 American Heart Association (AHA) Guidelines for Advanced Cardiovascular Life Support in Adults (hereinafter referred to as the 2023 AHA Guidelines), including regulating ECMO blood flow, the dose of vasoactive drugs (MAP ≥65 mmHg), mechanical ventilation parameters (SaO₂ 94-98%, PaCO₂ 35-45 mmHg), sedation and analgesia plans.
Concurrently, staged target temperature management is implemented, involving maintaining the core temperature between 32 and 37.5°C within 24 hours, initiating controlled rewarming at a rate of ≤0.1°C/h after 24 hours, and continuing to prevent fever (core temperature ≤37.5°C) within 72 hours.

Experimental: Multimodal monitoring strategy
Experimental group: In addition to standard treatment in the control group, a multimodal monitoring system of brain function is integrated: continuous rSO2 monitoring with at least once-daily TCD for cerebral blood flow velocity monitoring, ONSD ultrasound measurement, and EEG monitoring.
Based on the cerebral oxygenation target-directed management strategies, intervention measures such as the ECMO blood flow, the dose of vasoactive drugs, mechanical ventilation parameters, target temperature management, sedation and analgesia plans, and antiepileptic drugs are dynamically adjusted to ultimately achieve the goal of brain oxygenation target value (rSO2) to 58%-68%.
