28 ottobre 2026 aggiornato da: Tang-Du Hospital

A Prospective Cohort Study of Perioperative PD-1 Monoclonal Antibody Combined With Ipilimumab N01 and Chemotherapy in Patients With Resectable Non-Small Cell Lung Cancer

This multicenter, prospective, randomized controlled study aims to evaluate the efficacy and safety of different durations of neoadjuvant treatment with a PD-1 inhibitor, ipilimumab, and chemotherapy in patients with resectable non-small cell lung cancer (NSCLC). A total of 90 treatment-naïve patients with clinical stage IIA (primary tumor >4 cm) to stage IIIB (limited to resectable N2 disease) NSCLC will be enrolled and randomly assigned in a 1:1 ratio to Cohort A (n=45) or Cohort B (n=45).

Patients in Cohort A will receive two cycles of a PD-1 inhibitor plus chemotherapy and one cycle of ipilimumab, whereas patients in Cohort B will receive four cycles of a PD-1 inhibitor plus chemotherapy and two cycles of ipilimumab. The PD-1 inhibitor will be administered intravenously at 200 mg every three weeks, and ipilimumab at 1 mg/kg every six weeks. Chemotherapy regimens will be selected according to histological subtype: pemetrexed plus carboplatin or cisplatin for non-squamous NSCLC, and nab-paclitaxel plus carboplatin or cisplatin for squamous NSCLC.

Following neoadjuvant treatment, eligible patients will undergo curative-intent surgery within 22-42 days after the last treatment dose. Investigator-selected adjuvant therapy will begin 29-42 days after surgery. Tumor assessments will be performed according to RECIST version 1.1, with postoperative follow-up for up to five years.

The primary endpoint is the pathological complete response (pCR) rate, with treatment safety also being evaluated.

  • Patients must sign a written informed consent form (ICF) and be able to comply with protocol-specified visits and related procedures.
  • Age ≥18 years.
  • Cytologically or histologically confirmed primary non-small cell lung cancer (NSCLC), including adenocarcinoma and squamous cell carcinoma.

Clinical stage IIB (primary tumor >4 cm), IIIA, or IIIB (limited to resectable N2 disease) according to the ninth edition of the lung cancer TNM staging classification established by the International Association for the Study of Lung Cancer (IASLC) and the American Joint Committee on Cancer (AJCC). Resectable N2 disease is defined as non-bulky (short-axis diameter <3 cm), discrete, or single-station N2 involvement. Pathological confirmation is required if N2 or N3 involvement is clinically suspected.

  • Patients with non-squamous NSCLC must undergo molecular testing confirming the absence of sensitizing epidermal growth factor receptor (EGFR) mutations and anaplastic lymphoma kinase (ALK) rearrangements. Molecular testing is not mandatory for patients with squamous NSCLC; however, patients with known sensitizing EGFR mutations or ALK rearrangements must not be enrolled.

  • Patients must be considered suitable for curative-intent surgical resection. Pulmonary function must meet the requirements for the planned lung resection (forced expiratory volume in one second [FEV1] ≥50% of the predicted value and maximal voluntary ventilation [MVV] ≥50% of the predicted value), with no contraindications to surgery.

  • Patients must be able to provide sufficient qualified tumor tissue specimens for programmed death-ligand 1 (PD-L1) testing during screening.

  • At least one measurable lesion according to Response Evaluation Criteria in Solid Tumors (RECIST), version 1.1.

  • Eastern Cooperative Oncology Group Performance Status (ECOG PS) of 0 or 1.

Cosa sta misurando lo studio?

Misure di risultato primarie

Misura del risultato
Misura Descrizione
Lasso di tempo
Pathological Complete Response (pCR) Rate
Lasso di tempo: At the time of surgery, approximately 20 weeks after randomization

The pCR rate is defined as the proportion of participants with no residual viable tumor cells in the resected primary tumor and sampled regional lymph nodes (ypT0N0), as determined by histopathological examination following neoadjuvant treatment and surgical resection. The pCR rate will be assessed separately in each treatment cohort.

At the time of surgery, approximately 20 weeks after randomization

Misure di risultato secondarie

Misura del risultato
Misura Descrizione
Lasso di tempo
Major Pathological Response (MPR) Rate
Lasso di tempo: At the time of surgery, approximately 20 weeks after randomization

The MPR rate is defined as the proportion of participants with 10% or less residual viable tumor cells in the resected primary tumor, as determined by histopathological examination following neoadjuvant treatment and surgical resection. The MPR rate will be assessed separately in each treatment cohort.

At the time of surgery, approximately 20 weeks after randomization
R0 Resection Rate
Lasso di tempo: At the time of surgery, approximately 20 weeks after randomization

The R0 resection rate is defined as the proportion of participants who achieve complete surgical resection with microscopically negative resection margins, as confirmed by postoperative histopathological examination.

At the time of surgery, approximately 20 weeks after randomization
On-Schedule Surgery Completion Rate
Lasso di tempo: Within 22 to 42 days after the last dose of neoadjuvant treatment

The on-schedule surgery completion rate is defined as the proportion of participants who undergo curative-intent surgical resection within the protocol-specified window of 22 to 42 days after the last dose of neoadjuvant treatment.

Within 22 to 42 days after the last dose of neoadjuvant treatment
Safety and Tolerability of Neoadjuvant Treatment
Lasso di tempo: From the first dose of neoadjuvant treatment through 90 days after surgery

Safety and tolerability will be evaluated based on the incidence and severity of adverse events (AEs), serious adverse events (SAEs), and treatment-related adverse events (TRAEs), assessed according to NCI-CTCAE version 5.0. The relationship of adverse events to study treatment will be determined by the investigator. Perioperative complications will be assessed using the Clavien-Dindo classification, and perioperative mortality will also be recorded.

From the first dose of neoadjuvant treatment through 90 days after surgery
Health-Related Quality of Life (HRQoL)
Lasso di tempo: During the first 12 months after surgery

Health-related quality of life will be assessed using the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30) and Lung Cancer Module 29 (EORTC QLQ-LC29). These questionnaires evaluate global health status, physical and functional well-being, lung cancer-related symptoms, and treatment-related adverse effects. Scores will be transformed to a 0-100 scale according to the EORTC scoring guidelines. Higher global health and functional scores indicate better quality of life, whereas higher symptom scores indicate greater symptom burden.

During the first 12 months after surgery
