La mobilizzazione del nervo mediano e l'ibuprofene alleviano entrambi il dolore cervicobrachiale, ma ciò avviene attraverso meccanismi diversi e con una diversa insorgenza di effetti collaterali. La mobilizzazione neurale del nervo mediano è una tecnica di terapia fisica che ottiene sollievo dal dolore attraverso la stimolazione meccanica del nervo mediano e del plesso brachiale. La corretta stimolazione meccanica del nervo e del tessuto neurale circostante induce un'ampia varietà di risposte fisiologiche che possono ridurre il dolore cervicobrachiale, inclusa l'attivazione dei meccanismi di modulazione del dolore del sistema nervoso discendente, sebbene l'intero insieme di ragioni alla base di questa riduzione del dolore non sia completamente inteso. Tra gli effetti causati dalla mobilizzazione del tessuto neurale che producono sollievo dal dolore ci sono: riduzione dell'edema intraneurale, modifiche della pressione del nervo intraneurale, dispersione di sostanze pro-infiammatorie e aumento della mobilità nervosa
L'ibuprofene è un farmaco antinfiammatorio non steroideo utilizzato in tutto il mondo per controllare il dolore, la febbre e l'infiammazione. L'assunzione orale di ibuprofene produce un effetto ipoalgesico dovuto all'inibizione biochimica degli enzimi COX, che convertono l'acido arachidonico in prostaglandina H2 (PGH2). PGH2, che a sua volta viene convertito da altri enzimi in diversi tipi di prostaglandine e trombossani (che sono mediatori del dolore e dell'infiammazione).
Gli effetti collaterali derivati dall'assunzione orale di ibuprofene sono correlati alla sua azione sistemica e possono essere gravi in alcuni pazienti. Questo fatto suggerisce che l'ibuprofene orale potrebbe non essere adatto all'uso in tutti i tipi di pazienti che soffrono di dolore cervicobrachiale, mentre la mobilizzazione neurale del nervo mediano ha pochi o nessun effetto collaterale noto. Ciò rivela un punto di interesse nel determinare l'efficacia comparativa tra queste due opzioni terapeutiche nel trattamento del dolore cervicobrachiale.

Descrizione
Criterio di inclusione:
- Diagnosi clinica del dolore cervicobrachiale confermata dalla risonanza magnetica
- Presenza di sintomi unilaterali di dolore al braccio, parestesia o intorpidimento dell'arto superiore durante almeno 3 mesi continuativi prima dell'applicazione del trattamento.
- Risultati positivi in tutti i seguenti test: Spurling, Distraction e Upper Limb
Criteri di esclusione:
- Controindicazione all'uso di farmaci antinfiammatori non steroidei (FANS)
- L'uso di qualsiasi tipo di trattamento, terapia, procedura o farmaco per alleviare il dolore
- Pazienti che sono sotto farmaci anticonvulsivanti, antidepressivi o psicotropi
- Instabilità vertebrale
- Osteoporosi vertebrale
- Infezione vertebrale o della colonna vertebrale.
- Malattie neurologiche di origine genetica, infettiva o neoplastica
- Mielopatia da stenosi cervicale
- Gravidanza
- Kinesiofobia
- Patologie endocrine e menopausa
- Storia della chirurgia della colonna vertebrale
- Disabilità intellettiva, grave malattia mentale, intossicazione, grave privazione del sonno, morbo di Alzheimer.

SPERIMENTALE: Mobilizzazione neurale del nervo mediano
Sessioni di mobilizzazione neurale del nervo mediano di 15 minuti 5 volte a settimana (dal lunedì al venerdì) per 6 settimane continue Il grado massimo di estensione del gomito applicato nella procedura di mobilizzazione neurale del nervo mediano è stato determinato mediante l'uso di un dispositivo goniometro universale.

La procedura di mobilizzazione neurale del nervo mediano non chirurgica non invasiva è stata applicata da un fisioterapista continuamente per 2 minuti in 5 diverse occasioni con 1 minuto di riposo tra ogni 2 minuti di applicazione della tecnica di mobilizzazione neurale.
L'intervento è stato applicato durante un periodo di 6 settimane.
Il livello massimo del grado di movimento di estensione del gomito senza la riproduzione dei sintomi durante l'applicazione del trattamento di mobilizzazione del nervo mediano è stato determinato attraverso l'uso di base di un dispositivo goniometro universale.

Una singola dose giornaliera di 400 mg di ibuprofene orale che può essere aumentata a seconda della tolleranza del paziente fino a una dose massima di 1200 mg/giorno equamente suddivisa in 3 assunzioni orali del farmaco ogni 8 ore durante 6 settimane continue.

Il gruppo di confronto attivo ha ricevuto un trattamento orale con ibuprofene.
La dose iniziale era di 400 mg/giorno ed è stata aumentata fino a un massimo di 1200 mg/giorno suddivisi in 3 assunzioni orali ogni 8 ore a seconda della tolleranza del paziente. L'intervento è stato applicato per un periodo di 6 settimane.
