La movilización neural del nervio mediano y el ibuprofeno alivian el dolor cervicobraquial, pero esto sucede a través de diferentes mecanismos y con una aparición diferente de efectos secundarios. La Movilización Neural del Nervio Mediano es una técnica de fisioterapia que consigue el alivio del dolor mediante la estimulación mecánica del nervio mediano y del plexo braquial. La estimulación mecánica adecuada del nervio y el tejido neural que lo rodea induce una amplia variedad de respuestas fisiológicas que pueden reducir el dolor cervicobraquial, incluida la activación de los mecanismos de modulación del dolor del sistema nervioso descendente, aunque el conjunto completo de razones subyacentes para esta reducción del dolor no está completamente aclarado. comprendido. El ibuprofeno es un fármaco antiinflamatorio no esteroideo utilizado en todo el mundo para controlar el dolor, la fiebre y la inflamación. La ingesta oral de ibuprofeno produce un efecto hipoalgésico debido a la inhibición bioquímica de las enzimas COX, que convierten el ácido araquidónico en prostaglandina H2 (PGH2). PGH2, que a su vez, es convertida por otras enzimas en varios tipos de prostaglandinas y tromboxanos (que son mediadores del dolor y la inflamación).
Los efectos secundarios derivados de la ingesta oral de Ibuprofeno están relacionados con su acción sistémica y pueden ser graves en algunos pacientes. Este hecho sugiere que el ibuprofeno oral puede no ser adecuado para su uso en todos los tipos de pacientes que sufren de dolor cervicobraquial, mientras que la movilización neural del nervio mediano tiene pocos o ningún efecto secundario conocido. Esto revela un punto de interés para determinar la efectividad comparativa entre estas dos opciones terapéuticas en el tratamiento del dolor cervicobraquial.

Descripción
Criterios de inclusión:
- Diagnóstico clínico de dolor cervicobraquial confirmado por resonancia magnética
- Presencia de síntomas unilaterales de dolor en el brazo, parestesias o adormecimiento en la extremidad superior durante al menos 3 meses continuos previos a la aplicación del tratamiento.
- Resultados positivos en todas las siguientes pruebas: Spurling, Distraction y Upper Limb
Criterio de exclusión:
- Contraindicación en el uso de antiinflamatorios no esteroideos (AINE)
- El uso de cualquier tipo de tratamiento, terapia, procedimiento o medicamento para aliviar el dolor
- Pacientes que están bajo medicación anticonvulsiva, antidepresiva o psicotrópica
- Inestabilidad vertebral
- Osteoporosis vertebral
- Infección vertebral o de columna.
- Enfermedades neurológicas de origen genético, infeccioso o neoplásico
- Mielopatía por estenosis cervical
- El embarazo
- kinesiofobia
- Trastornos endocrinos y menopausia
- Historia de la cirugía de columna
- Discapacidad intelectual, enfermedad mental grave, intoxicación, privación grave del sueño, enfermedad de Alzheimer.

EXPERIMENTAL: Movilización neural del nervio mediano
Sesiones de movilización neural del nervio mediano de 15 minutos 5 veces por semana (de lunes a viernes) durante 6 semanas continuas El grado máximo de extensión del codo aplicado en el procedimiento de movilización neural del nervio mediano se determinó mediante el uso de un dispositivo de goniómetro universal.

El procedimiento de movilización neural del nervio mediano no invasivo y no quirúrgico fue aplicado por un fisioterapeuta de forma continua durante 2 minutos en 5 ocasiones diferentes con 1 minuto de descanso entre cada aplicación de 2 minutos de la técnica de movilización neural.
La intervención se aplicó durante un período de 6 semanas.
El nivel máximo de grado de movimiento de extensión del codo sin reproducción de síntomas durante la aplicación del tratamiento de movilización neural del nervio mediano se determinó mediante el uso de línea de base de un dispositivo de goniómetro universal.

COMPARADOR_ACTIVO: Ibuprofeno
Una dosis única diaria de 400 mg de Ibuprofeno oral que puede incrementarse según la tolerancia del paciente hasta una dosis máxima de 1200 mg/día repartidos a partes iguales en 3 tomas orales del fármaco cada 8 horas durante 6 semanas continuas.

El grupo de comparación activa recibió un tratamiento con ibuprofeno oral.
La dosis inicial fue de 400 mg/día y se aumentó hasta un máximo de 1200 mg/día repartidos en 3 tomas orales cada 8 horas según la tolerancia del paciente. La intervención se aplicó durante un período de 6 semanas.
